Abstract

Hypertension is a major risk factor for cardiovascular disease and remains a persistent challenge in health systems, including in Indonesia. The Chronic Disease Management Program (Prolanis) under the National Health Insurance scheme (Jaminan Kesehatan Nasional, JKN) aims to improve the control of chronic diseases. However, the achievement of blood pressure control indicators at a clinic affiliated with JKN remains suboptimal. This study aimed to identify factors associated with blood pressure control among hypertensive patients enrolled in the JKN program. A cross-sectional quantitative design was applied to 120 hypertensive patients selected through quota sampling. Data were collected using standardized questionnaires (IPAQ, PSS-10, MMAS-8, and MSPSS) and analyzed using Chi-Square tests and multiple logistic regression. Results showed that 50.8% of respondents had controlled blood pressure. Among all variables examined, only physical activity was statistically associated with blood pressure control (p = 0.020; OR = 9.5; 95% CI: 1.74–51.53; p = 0.009). These findings highlight the importance of strengthening promotive and preventive approaches—particularly through structured physical activity and health education—as part of the Prolanis service to support sustainable blood pressure control.

Abstract

Introduction: Universal Health Coverage (UHC) is a significant agenda outlined in the SDG 2030. In order to achieve UHC, Indonesia established the Social Security Management Agency (BPJS) in 2014. Nearly a decade after its establishment, BPJS experienced a financial deficit in its first six years, followed by a surplus during the COVID-19 pandemic. This has impacted the sustainability of BPJS Health. One of the factors contributing to the financial deficit is the non-compliance of participants in paying BPJS contributions, particularly in the Non-Wage Earner/Self-Employed/Independent (PBPU) and Non-Worker segments who are required to pay contributions individually. When participants fail to pay contributions for six consecutive months, their membership status is converted to inactive. From the 2022 BPJS Sample Data, it was found that 799,384 out of 2,305,435 (34.67%) participants had inactive membership status. Objective: This study aims to analyze the factors influencing BPJS membership status in the Independent and Non-Worker segments. Method: The method that used in this study is quantitative analysis of secondary data from the 2022 BPJS Sampel Data with the Linear Probability Model (LPM), Logit and Probit Models. The analysis is conducted using STATA to obtain univariate, bivariate and multivariate analyses. Result: From a sample of 545,507 participants, if was found that 54.20% had inactive membership status. Factors such as age, gender, marital status, treatment class, healthcare facility type, and participant segmentation affect membership status. The LPM regression results indicate that the factors leading to inactive membership status are being of productive age, male, belonging to class 1 and 2, utilizing primary healthcare facilities, and being in the Independent segment. Conclusion: Therefore, stateholders such as the central government and BPJS need to collaborate in efforts to reduce the number of inactive membership statuses.

Abstract

Developing Asian countries continue to face financial challenges in ensuring equitable access to HIV services. Despite numerous initiatives to expand coverage and improve treatment outcomes, affordability remains a major barrier. This systematic review, conducted using PRISMA methodology, analyzes willingness to pay (WTP) and readiness to pay (RTP) for HIV-related services, based on 14 studies selected from an initial pool of 6,773 articles identified through three major databases (PubMed, Scopus, and ProQuest) covering peer reviewed publications between 2015 and 2025. Focusing on at-risk populations in India, China, Vietnam, Iran, and the Philippines, the review examines services such as HIV self-testing kits, antiretroviral therapy (ART), pre-exposure prophylaxis (PrEP), and viral load monitoring. Findings reveal significant variation in WTP/RTP influenced by education, income, HIV awareness, stigma, and privacy concerns. For instance, 69% of respondents in India were willing to pay for self-testing kits, compared to only 6.8% of MSM in China willing to pay market rates for PrEP. RTP extends beyond financial capacity to include behavioral, social, and cultural readiness, which plays a critical role in service uptake. The review concludes that WTP/RTP alone is insufficient for achieving universal HIV care. It recommends adaptive policies such as hybrid financing models, targeted health education, and government subsidies to ensure sustainable and inclusive HIV service delivery.

Abstract

Digital transformation has become a prominent topic in the era of globalization. Despite the growing digital transformation trend, many health service users in Indonesia still prefer face-to-face interactions, indicating a gap in digital literacy and service adoption. In response to this issue, the Social Security Administrative Body for Health launched a digital administrative service called Pandawa (Administrative Service via WhatsApp). This study aims to explore the perceptions of National Health Insurance participants who visited Social Security Administrative Body for Health Depok in 2025, focusing on the perceived ease of use and usefulness of the Pandawa service. The research employs a quantitative approach using the Technology Acceptance Model (TAM) through a cross-sectional study design. The secondary data used in this study consists of participant visits to the agency in 2024. Primary data will also be needed through distributed questionnaires. The sample consists of 109 National Health Insurance participants who are users of the Pandawa service and visited the agency during the study, conducted in February 2025. The analysis techniques of the research used the Chi-Square Test and the Binary Logistic Regression Test. The multivariate analysis shows that age, education, perceived ease of use, perceived usefulness, and attitude are significantly related to the intention to use the Pandawa service, with attitude being the dominant factor. Meanwhile, sex does not show a significant relationship to the intention to use the Pandawa service. Overall, user acceptance of the Pandawa service is relatively good, although further improvements in service quality are necessary to optimize its implementation.
