Abstract

Abstract. Background. High intensity of mobile phone use can cause Carpal Tunnel Syndrome. Repeated wrist positions and movements cause increase pressure in carpal tunnel, that condition can compress the median nerve. Objectives. To determine the correlation between mobile phone use intensity and Carpal Tunnel Syndrome in Medical Faculty North Sumatera University Student. Methods. This research used an observational analytic design with cross sectional method. The sample were 417 Medical Faculty North Sumatera University students who met the inclusion criteria and were not included in the exclusion criteria. The instrument of this research were Mobile Phone Use Questionnaire and Indonesian version of the Boston Carpal Tunnel Questionnaire. The research results were analyzed using the Statistical Package for Social Science application with the Rank Spearman Correlation Test. Results. Based on the Rank Spearman Correlation test, obtained p < 0,0001 which stated that there was a correlation between mobile phone use intensity with Carpal Tunnel Syndrome. The correlation coefficient of 0.177 indicated very weak correlation and correlation between two variables was unidirectional. Conclusion. There was correlation between mobile phone use intensity with Carpal Tunnel Syndrome in Medical Faculty North Sumatera University Student.

Abstract

Introduction: Quality of life is an important parameter to be assessed, especially in progressive chronic diseases such as patients with Parkinson's Disease (PD). Parkinson's Disease Questionnaire-39 (PDQ-39) is the most widely used measurement tool in assessing the quality of life of PD patients. In PD, a series of features can affect the quality of life including the degree of disease severity and the number of medications. The aim of this study is to determine the relationship between the severity of the disease and the number of medications with the quality of life of people with PD so that it is hoped that it can help improve the quality of life of PD patients. Method: This study used a cross-sectional design. The research subjects were PD patients at the Neurology Polyclinic at Haji Adam Malik Hospital Medan from April - November 2020 who met the inclusion criteria. Assessing the severity of the disease using the Hoehn and Yahr and measuring the quality of life using the PDQ 39. Data analysis used the Spearman correlation test for the degree of severity and the Mann- Whitney test for the number of medications. Results: Of the 47 subjects who met the inclusion criteria, most were males (68.1%), with a mean age of 64.82 ± 9.52 years old, with University graduates as the majority of education level (46.8%) and the civil servant as the majority of a profession (34.0%). Stage 2 on Hoehn and Yahr Scales is the most common severity level of disease (55.3%), and polytherapy is the majority of the number of medications (74.5%). The median value of quality of life is 35, with the lowest score being 10 and the highest score being 98. From the results of the bivariate analysis, it was found that there was a very strong, significant association between the degree of disease severity and the quality of life (p <0.001 r=0.872), and there was a significant difference between the number of medications with the quality of life of Parkinson's disease patients (p=0.041). Conclusion: There is a very strong association between the degree of severity and the quality of life of people with Parkinson's disease and there is a difference in the quality of life of PD patients between those who receive monotherapy and polytherapy so there is an effect of the number of medication given on the quality of life of Parkinson's disease patients.

Abstract

Introduction: Tension-type headache (TTH) is one of the major public health concerns globally with an incidence of 1.89 billion people. Fatigue is potentially debilitating and affects an individual's quality of life and is known to be associated with the case of TTH. The purpose of this study was To find out the relationship between fatigue and the incidence of tension-type headache in the first semester resident doctors of the Faculty of Medicine Universitas Sumatera Utara. Method: The research design was an observational study with a cross-sectional approach which was undertaken at Haji Adam Malik Hospital and Network Hospital in January to February 2022 with all first semester resident doctors as research subject at Faculty of Medicine, Universitas Sumatera Utara in the period of August 2022. Fatigue was measured through the Fatigue Severity Scale (FSS), TTH frequency was measured using the third of the International Classification of Headache Disorders (ICHD-3) criteria, and TTH pain intensity was measured using numerical rating scale (NRS). Meanwhile, to analyze the relationship between fatigue and the incidence of tension-type headache, the Spearman correlation test was used. Results: : Total subjects who met the inclusion and exclusion criteria was 98.  Regarding to the results of statistical tests, a significant relationship was obtained between fatigue and TTH frequency (r= 0,294, p= 0,003), but there was no relationship between fatigue and the pain intensity (r= 0,143, p= 0,160). Conclusion: There was a significant relationship between fatigue and TTH frequency. However, the relationship was not significant between Fatigue and the pain intensity

Abstract

Introduction: Cerebral atrophy is a condition associated with brain volume reduction and is a common manifestation of aging. Age is an important factor in the incidence of cerebral atrophy. This research aims to know the relationship between age and the incidence of cerebral atrophy in patients at H. Adam Malik General Hospital Medan. Methods: This study is an observational analytic study with a cross-sectional design conducted at the Haji Adam Malik General Hospital Medan by collecting head CT scan data from the electronic medical records of 91 patients with the period January 1st 2021 to December 31st 2022. The research time will be carried out from October 2022 to December 2023. Results: Cerebral atrophy began to be found in the age group of 20-39 years, the largest gender was male (56%), the highest level of education in patients with cerebral atrophy was elementary school / equivalent (66.7%) and D1 to D3 (66.7%), and from the area of residence, patients who experienced cerebral atrophy mostly lived in urban areas (58.7%). The age of onset of cerebral atrophy was found to be 20 years old. The types of cerebral atrophy found in the study subjects were cortical atrophy as many as 49 people (53.8%) and global atrophy as many as 10 people (11%). There was no significant relationship between age and the incidence of cerebral atrophy in patients at H. Adam Malik General Hospital (p> 0.05), but there was a significant relationship between age group and the incidence of cerebral atrophy in patients at H. Adam Malik General Hospital (p < 0.05). Conclusion: There is no significant association between age and the incidence of cerebral atrophy in patients at Adam Malik General Hospital. Additional research is needed to gain a comprehensive understanding of cerebral atrophy and its relationship with other risk factors.

Abstract

COMPARATIVE ANALGESIC EFFECTS OF AMITRIPTYLINE, GABAPENTIN, AND PREGABALIN IN DIABETIC NEUROPATHY AND TRIGEMINAL NEURALGIAABSTRACTIntroduction: The management of neuropathic pain is a challenge for clinicians because of its nonspecific and difficult clinical to treat characteristics. The e use of antidepressant drugs such as amitriptyline and anticonvulsants such as gabapentin and pregabalin has shown various efficacy overcoming neuropathic pain.Aim: To compare the analgesic  efficacy of amitriptyline, gabapentin, and pregabalin  in patients with diabetic neuropathy and trigeminal neuralgia.Methods: This is an experimental pre and post test study on patients with diabetic neuropathy and trigeminal neuralgia in neurology clinic Haji Adam Malik Hospital, Medan, from April 2015 to march 2017. in each disease, subjects were divided into three groups, each was treated either with oral amitriptyline 12.5mg, gabapentin 100mg, or pregabalin 75mg twice daily. The Numeric Rating Scale to assess pain intensity were examined before and after two weeks after treatment.Results: The number of diabetic neuropathy subjects was 75, while trigeminal neuralgia subjects was 30, each were divided into three groups treated either with amitriptyline, gabapentin, or pregabalin. There were no differences on pain intensity changes in diabetic neuropathy groups but significant differences were shown in trigeminal neuralgia groups.Discussion: Amitriptyline, gabapentin, and pregabalin effective to lower pain intensity in trigeminal neuralgia significantly compare to diabetic neuropathy.Keywords: Amitriptyline, diabetic neuropathy, gabaptentin, pregabalin, trigeminal neuralgiaABSTRAKPendahuluan: Pengelolaan nyeri neuropatik merupakan tantangan bagi klinisi karena karakteristik klinisnya yang nonspesifik dan tatalaksananya yang sulit. Penggunaan antidepresan seperti amitriptilin dan antikonvulsan seperti gabapentin dan pregabalin mempunyai efikasi yang berbeda-beda dalam mengatasi nyeri neuropatik.Tujuan: Mengetahui perbedaan efek analgesik dari amitriptilin, gabapentin, dan pregabalin pada penderita neuropati diabetik dan neuralgia trigeminal.Metode: Studi eksperimental pre dan post test terhadap pasien neuropati diabetik atau neuralgia trigeminal yang berobat ke Poliklinik Neurologi RSUP Haji Adam Malik, Medan, sejak bulan April 2015 hingga Maret 2017. Semua subjek dibagi menjadi tiga kelompok untuk setiap penyakit, yang masing-masing mendapatkan amitriptilin 12,5mg, gabapentin 100mg, dan pregabalin 75mg, dengan frekuensi pemberian obat dua kali sehari setiap kelompok. Pengukuran intensitas nyeri dengan menggunakan Numeric rating Scale dilakukan sebelum dan setelah dua minggu pengobatan.Hasil: Didapatkan subjek dengan neuropati diabetik sebanyak 75 orang dan neuralgia trigeminal 30 orang yang masing-masing dibagi menjadi 3 kelompok dengan terapi amitriptilin, gabapentin, dan pregabalin. Tidak terdapat perbedaan rerata perubahan intensitas nyeri yang bermakna pada kelompok neuropati diabetik, namun bermakna pada subjek neuralgia trigeminal.Diskusi: Amitriptilin, gabapentin, dan pregabalin memiliki efikasi dalam menurunkan intensitas nyeri pada neuralgia trigeminal secara bermakna dibandingkan pada neuropati diabetik.Kata kunci: Amitriptilin, gabapentin, neuralgia trigeminal, neuropati diabetik,pregabalin
