Abstract

Fracture occurs due to the damage of the shape and function of the bone, and the fractured patients generally experience pain. One of the interventions to reduce pain intensity in fracture patients is cold compress. This study aims to identify the effectiveness of cold compress on pain intensity in fracture patients in H. Adam Malik Hospital Medan. This is a quantitative study with a quasi-experimental design approach with an equivalent control group pretest-posttest design. The number of samples is 70 respondents consisting of 35 people for each intervention and control group, respectively, which was collated by consecutive sampling. Data analysis used paired t-test and independent t-test. The results showed that there was a significant influence before and after cold compress intervention on pain intensity (p value 0.001 (p = <0.05)). There is a significant difference between the intervention with cold compresses and the hospital standard intervention on pain intensity in fracture patients (p value 0.001 (p = <0.05)). It can be concluded that cold compress has a significant effect on the pain intensity in fracture patients. Cold compress is one of the nursing interventions that can help to reduce the intensity of fracture pain.

Abstract

Luka kaki diabetes (LKD) merupakan komplikasi penyakit diabetes yang disebabkan oleh gangguan sirkulasi darah vena dan arteri. Buerger allen exercise adalah terapi modalitas dengan gerakan postural aktif. Penelitian ini bertujuan untuk menilai pengaruh buerger allen exercise tehadap peningkatan sirkulasi ektremitas bawah pasien LKD. Desain penelitian adalah pre eksperimen pretest dan posttest without control. Teknik pengambilan sampel adalah consecutive sampling sebanyak sampel 43 responden LKD, analisis data menggunakan uji Wilcoxon. Peningkatan sirkulasi di ukur menggunakan Ankle Brachial Index (ABI). Latihan dilakukan sebanyak 2 kali sehari dengan waktu latihan 17 sampai 20 menit, selama 3 minggu. Hasil penelitian menunjukkan perbedaan signifikan antara nilai rata-rata ABI sebelum 0,84 dan sesudah 0,95 intervensi Buerger allen exercise dengan nilai p= 0,000. Buerger allen exercise efektif untuk meningkatkan sirkulasi LKD karena perubahan posisi dan gaya gravitasi membantu mengosongkan dan mengisi kolom darah, sedangkan kontraksi muskulus gastrocnemius sebagai muscle pump mengaktivasi pembuluh darah vena dan arteri untuk membuka jalur sirkulasi collateral lokal. Kata Kunci: Luka kaki diabetik, ankle brakial indeks, Buerger allen exercise Abstract The Influence of Buerger Allen Exercise on Lower Extremity Circulation in Patients with Diabetic Foot Ulcer. Diabetic foot ulcer (DFU) is a complication of diabetes caused by impaired venous and arterial blood circulation. Buerger allen exercise is a modality therapy with active postural movement. This study aims to assess the effect of buerger allen exercise on the circulation of lower extremities in DFU patients. The study design was pre experimental pretest and posttest without control. The sampling technique was consecutive sampling and the large sample of 43 DFU patients, analysis data was using Wilcoxon test. Increased circulation is measured using the Ankle Brachial Index (ABI). Exercise done as much as 2 times a day with practice time 17 to 20 minutes, for 3 weeks. The results showed a significant difference between the average value of ABI before 0.84 and after 0.95 intervention of Buerger allen exercise with a value of p = 0.000. Buerger allen exercise is effective for improving DFU circulation because position changes and gravitational forces help to empty and fill the blood column, and the contraction of the gastrocnemius as a muscle pump activates the venous and arteries and opens the local collateral circulatory pathway. Keyword: Diabetic foot ulcer, brachial ankle index, Buerger allen exercise

Abstract

Patients with health problems who undergo bed rest for a long time can experience various health problems. Problems that can be felt by patients on functional and hemodynamic status. This study aims to identify the effectiveness of progressive mobilization on functional status and hemodynamics in bed rest patients. The research method used a qualitative design with a randomized controlled trial method. The number of samples in this study was 70 people for each group. The sample in this study was taken by simple random sampling method with a total of 70 people in the control group and 70 people in the intervention group. Bivariate statistical test used Friedman and Mann Whitney test, while multivariate test used logistic regression. The results of this study showed that there was a significant effect of progressive mobilization on functional and hemodynamic status (p0.05). The conclusion of this study is that progressive mobilization is effective on the functional and hemodynamic status of the patient so that it can be applied in the ICU of the hospital as a nursing intervention

Abstract

Background: Incontinence Associated Dermatitis is an inflammation of the skin that occurs when urine or feces comes in contact with the perineum.  The use of a skin barrier that contains moisturizers can prevent skin inflammation.  The mechanism of action of moisturizers is found in aloe vera and olive oil, as well as accelerating wound healing, anti-inflammatory, and antimicrobial. Objective: To identify the effect of skin barrier aloe vera and olive oil on the prevention of Incontinence Associated Dermatitis in immobility patients. Methods: The study was a Quasi-Experiment post-test-only comparison group design.  The population was newly admitted immobility patients without Incontinence Associated Dermatitis. A sample of 66 people was divided into the control group and the intervention group, sampling using purposive sampling, analyzed using the Mann Whitney and Kruskal Wallis tests. Results: The results showed that there were differences in the values of the three groups, namely the aloe vera, olive oil, and control groups (p = 0.003), with the highest control mean value. Conclusion: This study revealed that immobility patients who were not given the skin barrier using aloe vera or olive oil experienced the most Incontinence Associated Dermatitis. Abstrak: Latar belakang: Incontinence Associated Dermatitis adalah peradangan kulit yang terjadi ketika urin atau tinja kontak dengan perineum. Penggunaan skin barrier yang mengandung pelembab dapat mencegah peradangan kulit. Mekanisme kerja pelembab terdapat pada aloe vera dan olive oil, sekaligus dapat mempercepat penyembuhan luka, antiinflamasi, dan antimikroba. Tujuan penelitian: Untuk mengidentifikasi pengaruh skin barrier aloe vera dan olive oil terhadap pencegahan Incontinence Associated Dermatitis pasien imobilitas. Metode: Jenis penelitian adalah quasi-eksperiment dengan post-test only comparison group design. Populasi adalah pasien imobilitas yang baru masuk tanpa incontinence associated dermatitis. Sampel sebanyak 66 orang dibagi dalam kelompok kontrol dan kelompok intervensi, pengambilan sampel menggunakan purposive sampling, dianalisa menggunakan uji Mann Whitney dan Kruskal Wallis. Hasil: Menunjukkan terdapat perbedaan nilai ketiga kelompok, yaitu kelompok aloe vera, olive oil dan kontrol (p=0,003), dengan nilai rata-rata kontrol paling tinggi. Kesimpulan: Penelitian ini mengungkapkan bahwa pasien imobilitas yang tidak diberikan skin barrier aloe vera maupun olive oil paling banyak mengalami incontinence associated dermatitis.
