Abstract

Cardiovascular disease (CVD) is a growing burden in Indonesia, making primary prevention of utmost importance. High sensitivity cardiac troponin I (hsTnI) has been known as one of the biomarkers of choice for diagnosing acute myocardial infarction. Nonetheless, recent studies indicate that hsTnI assay has potential as a predictor of cardiac risk in asymptomatic individuals. An advisory board consisting of renowned cardiologists from the Indonesian Heart Association was convened in Jakarta in March 2023. The meeting aimed to explore the appropriate use of hsTnI for cardiovascular (CV) risk stratification in apparently healthy adults in Indonesia. The board reviewed relevant literature and developed key consensus statements, including hsTnI cut-off for identifying high-risk asymptomatic patients, the proposed algorithm, and monitoring after aggressive risk factor control. This article presents the resulting consensus statements to provide clinicians with a practical tool for planning primary prevention strategies. Furthermore, it is expected to raise awareness and advocacy among stakeholders in the healthcare infrastructure regarding the use of hsTnI as a guide for assessing CV risk in Indonesia.

Abstract

Selain digunakan sebagai indikator signifikan untuk diagnosis klinis gagal jantung akut, pemeriksaan NT-proBNP juga dapat membantu menentukan tingkat keparahan, memandu strategi pengobatan yang relevan, dan menilai prognosis penyakit jantung. Dari penelitian yang sudah ada sebelumnya diketahui penerapan NT-proBNP guided therapy dapat menurunkan kadar NT-proBNP selama perawatan ≥ 30%. Penelitian ini bertujuan untuk menganalisis profil kadar NT-ProBNP sebagai Langkah awal dalam NT-proBNP guided therapy. Jenis penelitian ini adalah penelitian observasional dengan desain cross sectional. Pengambilan sampel dilakukan di IGD Rumah Sakit Universitas Airlangga selama periode Juni-Agustus 2023 dengan kriteria inklusi pasien AHF berusia ≥ 18 tahun dan didiagnosis AHF secara klinis dengan adanya satu gejala pada HF (dispnea, ortopnea, atau edema) dan satu tanda (rales, edema perifer, asites, atau kongesti vaskular paru pada radiografi dada. Pemeriksaan kadar NT-proBNP menggunakan prinsip Enzyme-Linked Immunosorbent Assay (ELISA) menggunakan metode Chemiluminescent Microparticle Immunoassay (CMI). Analisis data secara deskriptif menunjukkan hasil bahwa daei 39 pasien yang masuk ke dalam kriteria inklusi, terdapat 32 sampel dengan profil kadar NT-proBNP meningkat ≥ 300 pg/mL.

Abstract

Background: ST-elevation myocardial infarction (STEMI) has been traditionally associated with elderly males; however, its prevalence is on the rise among young women.  Women often manifest with non-atherosclerotic causes of ischemic heart disease, including plaque erosion and spontaneous coronary artery dissection (SCAD); while atypical symptoms and other pathophysiological processes often result in delay in diagnosis and diagnostic or treatment differences. This case underscores the crucial role of early recognition of atypical symptoms, careful assessment of risk factors, and thoughtful selection of the best management strategies. Case Presentations: A 37-year-old woman with inadequately controlled hypertension, who did not have any traditional risk factors, presented to the emergency department with diaphoresis and epigastric pain. Initial electrocardiogram (ECG) showed inferoposterior STEMI, and coronary angiography revealed total mid RCA occlusion with nonsignificant LAD stenosis. She was immediately taken to the cath lab and primary PCI was performed with direct stenting to improve myocardial perfusion. After PCI, the patient experienced uneventful TIMI III flow restoration and was discharged on DAPT and optimal medical therapy. She was asymptomatic at the one-year follow-up, which underscores the efficacy of direct stenting in specific STEMI. Conclusions: Young women with STEMI represent a distinct and under-recognized group requiring sex specific risk stratification and tailored management to improve outcomes. Further research is needed to identify optimal therapeutic strategies for this population.
