Abstract

Tuberculosis is an acute infectious disease that primarily affects the lungs. Probiotics supplementation can increase the number and activity of NK cell in peripheral blood by modulation of IL-12, thus increasing IFN-γ production by Th1 response. Vitamin B1 acts on macrophages and affects neutrophil motility. Vitamin B6 is associated with the release of cytokines and the responsiveness of NK cells, while vitamin B12 affects to lymphocytes, Tcell proliferation, CD4+ ratios, and NK cell activity. To analyze the effects of probiotics and vitamin B1, B6, B12 supplementation on IFN-γ and IL-12 levels during intensive phase of antituberculosis treatment. The study was pre-post test randomised control by time series. The control group was TB patients with standard therapy of antituberculosis and vitamin B6, while the intervention group was TB patients receiving therapy plus once daily probiotics and vitamin B1, B6, B12supplementation during the intensive phase. Blood samples were withdrawn at baseline, one month, and two months after therapy to measure plasma IFN-γ and IL-12 levels using the ELISA method. Twenty two patients were divided equally into two groups. There was a tendency to greater increase of IFN-γ in the first month of the intervention group, followed by a significant decline after two-month therapy (p < 0.05). In both groups there was a rise in IL-12 levels after one month followed by a decrease in the second month (p > 0.05). However, the percentage was higher in the supplementation group. Adding probiotics and vitamins B1, B6, B12 could improve immune response through IL-12 and IFN-γ modulation during intensive phase therapy.

Abstract

Background: Uremic patients are in a pro-oxidant state and show an increased level of asymmetric dimethylarginine (ADMA), which is due to increased PRMT1 activity and reduced dimethylarginine dimethylaminohydrolase (DDAH) as degradation enzymes. Reactive oxidant species may play an important role in increasing the action of PRMT1 and in inhibiting the action of DDAH. Albuminuria and ADMA are closely correlated with progression of cardiovascular disease in chronic kidney disease (CKD) patients as well as indicators for decreasing renal function. Although ACEIs and/or ARBs reduced albuminuria in CKD patients, the results are still conflicting. Several factors in these patients may play important roles in the mechanism of albuminuria such as oxidative stress. The antioxidant N-acetylcysteine may prove to have beneficial therapeutic effect, because it can reduce oxidative stress as shown by evidence in humans, and subsequently increase ADMA. The objective of the present study is to explore the contribution of the antioxidant N-acetylcysteine (NAC) to the decrease of ADMA and albuminuria in non-diabetic CKD patients. Material and Methods: Patients with non-DM CKD stage 1–4 with albuminuria were randomized to receive ACEI and/or ARB alone (control group) or with antioxidant NAC 600 mg orally twice a day (treatment group). Observations were performed for 3 months to measure ADMA and albuminuria before and after-treatment. 80 patients in total 40 in the control group and 40 in the treatment group were used. Results: After oral treatment with NAC, the plasma level of ADMA in the treatment group increased from 0.604 µmol/l to 0.689 µmol/l, whereas ADMA level in the control group exhibited a higher increase from 0.561 µmol/l to 0.743 µmol/l. The increases in these groups were significantly different (p < 0.02). Moreover, the level of albuminuria was reduced from 148.12 µg/mg • cr to 132.7 µg/mg • cr in the treatment group, and from 75.25 µg/mg • cr to 71.85 µg/mg • cr in the control group. The difference was significant (p < 0.001). Conclusion: The anti-oxidant N-acetylcysteine can be used as adjuvant therapy to inhibit the progression of CKD in patients by decreasing the ADMA level and albuminuria.

Abstract

Selain digunakan sebagai indikator signifikan untuk diagnosis klinis gagal jantung akut, pemeriksaan NT-proBNP juga dapat membantu menentukan tingkat keparahan, memandu strategi pengobatan yang relevan, dan menilai prognosis penyakit jantung. Dari penelitian yang sudah ada sebelumnya diketahui penerapan NT-proBNP guided therapy dapat menurunkan kadar NT-proBNP selama perawatan ≥ 30%. Penelitian ini bertujuan untuk menganalisis profil kadar NT-ProBNP sebagai Langkah awal dalam NT-proBNP guided therapy. Jenis penelitian ini adalah penelitian observasional dengan desain cross sectional. Pengambilan sampel dilakukan di IGD Rumah Sakit Universitas Airlangga selama periode Juni-Agustus 2023 dengan kriteria inklusi pasien AHF berusia ≥ 18 tahun dan didiagnosis AHF secara klinis dengan adanya satu gejala pada HF (dispnea, ortopnea, atau edema) dan satu tanda (rales, edema perifer, asites, atau kongesti vaskular paru pada radiografi dada. Pemeriksaan kadar NT-proBNP menggunakan prinsip Enzyme-Linked Immunosorbent Assay (ELISA) menggunakan metode Chemiluminescent Microparticle Immunoassay (CMI). Analisis data secara deskriptif menunjukkan hasil bahwa daei 39 pasien yang masuk ke dalam kriteria inklusi, terdapat 32 sampel dengan profil kadar NT-proBNP meningkat ≥ 300 pg/mL.

Abstract

Kepatuhan terhadap terapi farmakologi menjadi kunci utama dalam pengendalian Penyakit Jantung Koroner (PJK) dimana hal ini akan sangat dipengaruhi oleh tingkat pengetahuan pasien. Penelitian ini bertujuan untuk menilai efektivitas video animasi sebagai media edukasi terhadap peningkatan pengetahuan pasien terkait penggunaan obat-obatan dan metode meningkatkan kepatuhan pada pasien PJK. Penelitian ini bersifat analisis menggunakan desain pre-post test di Rumah Sakit Universitas Airlangga, Surabaya. Bentuk intervensi yang dilakukan adalah dengan edukasi video animasi yang dibuat oleh peneliti dengan durasi 9 menit 51 detik. Video tersebut berisi materi tentang pengenalan penyakit jantung koroner, obat-obatan jantung koroner, dan cara untuk meningkatkan kepatuhan. Selanjutnya pengetahuan akan dievaluasi menggunakan kuesioner berisi 15 pertanyaan yang terdiri dari pertanyaan seputar terapi PJK, manfaat terapi, dan cara meningkatkan kepatuhan dalam pengobatan. Analisa data dilakukan dengan menggunakan uji statistik menggunakan uji Wilcoxon signed rank test (uji non parametrik) . Hasil penelitian ini menunjukkan bahwa dari total sampel 50 pasien, video animasi dapat meningkatkan kepatuhan pasien secara signifikan (p-value 0,000)  dengan nilai rata-rata pengetahuan pasien sebelum edukasi adalah 66.0 ± 1,8 dan setelah edukasi menjadi 92.7 ± 1,3. Proporsi pasien yang mempunyai pengetahuan baik juga mengalami peningkatan secara signifikan (p-value 0,000) yaitu sebanyak 14% sebelum edukasi dan 90% sesudah edukasi. Dengan demikian dapat disimpulkan bahwa terdapat peningkatan pengetahuan pasien terkait obat-obat PJK dan metode meningkatkan kepatuhan setelah dilakukan edukasi dengan menggunakan media video animasi.
