Abstract

Abstract: Basic immunization is a strategic step in preventing polio, diarrhea, congenital defects, and delays in physical and cognitive development. Tomuan Health Center is one of the health centers in Pematang Siantar City with low immunization achievements. The purpose of the study was to determine the factors associated with the completeness of basic immunization of infants. The research design used in this study was cross sectional. The research was conducted in the Tomuan Health Center working area. The research was conducted in October 2023. The study population was all mothers who have babies who have reached the age of 12 months who live around the Tomuan Health Center working area. The research sample amounted to 50 respondents who were selected using simple random sampling. The research instrument used a research questionnaire. Data analysis was conducted univariate and bivariate. The results showed family support (p value: 0.001), maternal confidence (p value: 0.001) on the provision of complete basic immunization. It is recommended that the puskesmas provide information in counseling and increase the motivation of mothers regarding the importance of immunization and monitor the completeness of immunization so as to create trust in doing this and for mothers who have infants and toddlers should be more active in seeking information about basic immunizations that should be given to children because this is very influential with the mother's knowledge. Keywords: Toddlers, Family Support, Basic Immunization.

Abstract

In the world of obstetrics and gynecology, narrow pelvis or narrow pelvimetry often causes a mismatch between the size of the fetus and the birth canal called cephalopelvic disproportion (CPD). A narrow pelvis is one of the main causes of CPD, which is often an indication for cesarean section. In cases like this with a head presentation and narrow maternal pelvis, the risk of prolonged labor and complications increases significantly. Cesarean section is the main intervention of choice to save the life of the mother/baby so that it has an impact on reducing morbidity and mortality rates. However, if the implementation is not carried out on time or is repeated, it has significant physiological and psychosocial impacts. Various studies have shown that cesarean section intervention in cases of narrow pelvis significantly reduces the risk of obstetric complications in both the mother and the baby. Although cesarean section is effective in preventing morbidity and mortality, this procedure can also be at risk of causing postoperative complications and perinatal disorders if not carried out in a timely manner. This case study aims to describe the handling of cases of narrow pelvis with head presentation through cesarean section and the clinical impact on the mother and baby. Cesarean section in cases of narrow pelvis with cephalic presentation has been proven effective in preventing labor complications. Early diagnosis and appropriate action decisions are essential to ensure the safety of the mother and baby.

Abstract

Tulehu Community Health Centers is one of the community health centers in these work areas of the Central Maluku Regency Health Services. has 17 Posyandu with 85 active health cadres. There are IYCF Cadres who are representatives of the 17 Posyandu Health cadres. they were formed and trained in 2019. Exclusive breastfeeding coverage in the Tulehu Health Center working area is 32%, which is not yet according to the target of 40%. There were  129 babies, but only 41 people received breast milk. This happens because some mothers say that breast milk production is lacking (insufficient breast milk syndrome), so they give formula milk and additional food before  6 months,  the activity aims is to increase the knowledge and skills of cadres in helping overcome breastfeeding problems in postpartum mothers.  20 cadres were targeted in the activities. The activities carried out consisted of providing oketani massage material demonstrations and re-demonstrations followed by independent practice a home using the learning models/media provided. The post-test results showed that there was an increase in knowledge from 7.1 to 92.26 and the skills assessment results were very good, namely 97.1. This indicates a very good achievement for the participants. It is hoped that regular monitoring and evaluation by midwives of the results of the oketani massage carried out by cadres will result in the problem of breastfeeding being resolved and the coverage of exclusive breastfeeding increasing.
