Abstract

Hospital accreditation standards require periodic reviews of medical records as part of performance improvement efforts, with review results being regularly reported to the Hospital Director. A preliminary survey in the Inpatient Department (IRNAP) at Telogorejo General Hospital, Semarang City, conducted from January to February 2022, showed that the implementation of electronic medical records (EMR) for inpatient services began on October 26, 2021. It was found that certain sections, especially in the discharge summary form, were not fully completed by doctors. This study aims to design and develop an application for monitoring and evaluating the quantitative analysis of digital medical discharge forms at SMC Telogorejo Hospital Semarang in 2022. The research employs a descriptive method, analyzing quantitative variables in the EMR. The study population consists of 1,826 inpatient medical records from 2021, with a sample of 183 records selected using purposive sampling. The result of this study is the SIKUMIS application. The development of the SIKUMIS application is crucial in supporting the assurance of medical record service quality, particularly in the analysis of inpatient discharge summaries. This application is expected to strengthen the integrity and completeness of medical record data, as well as improve efficiency and accuracy in healthcare services.

Abstract

RS PKU Muhammadiyah Gubug is a Type D hospital with full accreditation, providing Visum et Repertum (VeR) services. In 2022, there were 40 VeR requests for injury cases. However, challenges encountered included incomplete medical record documentation and VeR formats that did not meet the required elements of treatment and care. On January 13, 2023, the Udinus Community Service Team conducted knowledge-sharing sessions, interactive discussions, and mentoring on managing medical information disclosure in the form of VeR. The team also trained 13 medical record staff on creating VeR documents that comply with format requirements and essential elements. These activities involved knowledge sharing and interactive discussions. The results showed that hospital staff enthusiastically participated in the sessions. According to the Head of Medical Records, the team gained understanding and skills in creating standard-compliant VeR, which is expected to improve the quality of VeR services. Continuous mentoring is recommended to address future challenges, enhance the quality of VeR, and optimize service delivery. Keywords: Visum et Repertum, Medical Records, Service Quality

Abstract

Gold Indonesia 2045 depends on the availability of skilled labor as evidenced by the current condition of toddlers who are physically healthy. Semarang City is one of the cities that has a high distribution of stunting cases. North Semarang is the sub-district that has the most stunted toddlers, namely with 192 toddlers, of which 84 children are in Tanjung Mas District. The aim of the research is to see the effectiveness of posyandu digitalization training as an effort to increase stunting monitoring in posyandu. This research is an experimental analytical research with a cross sectional method using a pre-post test design that compares the effect of training method interventions. The number of questions in the questionnaire is 30 items related to knowledge, attitudes and practices of digitizing posyandu data. The intervention was carried out on 19 posyandu cadres to measure the level of knowledge based on the mean difference in pre-post test results using the Wilcoxon difference test. The effectiveness of the training method using the application is measured by the Gain Score formula. The statistical results obtained a p-value of 0.001, meaning that there was a significant difference in the level of digitization of cadre data after training with a gain-score of 0.99, so it can be said that the training activities carried out had high effectiveness in increasing data digitization for cadres.
