Abstract

Dermatitis atopi merupakan inflamasi kulit yang sering terjadi pada berbagai kelompok usia. Defek fungsi barier kulit, genetik, disregulasi sistem imun, dan lingkungan saling berinteraksi sebagai faktor penyebab. Manajemen meliputi perawatan kulit optimal, salah satunya dengan penggunaan rutin pelembap. Makalah ini akan memaparkan peran dan fungsi pelembap dalam dermatitis atopi.Atopic dermatitis is a common skin inflammation in any age group. Many factors, such as skin barrier function defect, genetic, immune system dysregulation, and environment, are related as contributing factors. Management includes optimal skin care, including routine moisturizer use. This article discuss the moisturizer’s role in atopic dermatitis.

Abstract

Abstract

Background: Henoch-Schönlein purpura (HSP) is a cutaneous small vessel vasculitis that typically affects children under ten but may also occur in adults. This study aims to report a rare instance of an adult patient with HSP to expand our knowledge and prevent delayed diagnosis and treatment. Case: A 21-year-old female patient presented with palpable purpuric lesions on the lower abdomen, buttocks, and lower limbs. In addition, the patient complained of abdominal pain and joint pain. The patient had suffered an upper respiratory tract infection with spontaneous resolution three days before the skin lesions. Laboratory tests showed leukocytosis and elevated anti-Streptolysin O titer. The patient was diagnosed with HSP and given oral methylprednisolone, antibiotics, and symptomatic treatment. She showed clinical improvement after taking medication. Conclusion: Henoch-Schönlein purpura is typically a self-limiting disease and infrequent in adults, but it has the potential to manifest into life-threatening conditions such as end-stage renal failure. An integrated multidisciplinary approach is needed for early diagnosis and management.

Abstract

Herpes zoster (HZ) results from the reactivation of latent varicella-zoster virus (VZV), typically triggered by aging or immunosuppression. Herpes zoster ophthalmicus (HZO) affects the ophthalmic branch of the trigeminal nerve (V1), posing a risk of ocular complications. Disseminated herpes zoster (DHZ), a rare form of HZ, is defined by ?20 vesicular lesions beyond the primary and adjacent dermatomes, potentially leading to systemic complications and increased morbidity. Case Presentation: A 36-year-old immunocompetent male presented with disseminated vesicular eruptions across the body, sparing the knees and lower legs, four days after the initial appearance of vesicles along the left ophthalmic (V1) branch of the trigeminal nerve. Notably, the patient had a history of minor trauma at the same site one week prior. A comprehensive evaluation and multidisciplinary management were undertaken, involving ophthalmologists, dermatologists, and neurologists. By the third day of treatment, his pain had significantly subsided, no new lesions had developed, and was discharged. Conclusion: DHZ can occur in immunocompetent individuals at any age, but is more common in older patients. A meticulous history and physical examination are essential, as dermatomal herpes zoster may progress to dissemination. Additionally, the potential link between recent trauma and zoster reactivation may play a contributory role, highlighting the importance of early recognition for prompt intervention.

Abstract

Tattoos are a form of body modification that involves inserting indelible ink into the dermis to alter skin pigmentation. Based on the duration of pigment retention, tattoos are classified into two types: temporary tattoos and permanent tattoos. The increase in the prevalence of tattooing has raised awareness of the adverse reactions associated with tattoos, which are not limited to the risk of dermatological complications, but also systemic complications. Adverse reactions related to tattoos are classified into infection, inflammatory, and neoplastic complications. Risks of infection and neoplasia are only observed in permanent tattoos. Hypersensitivity reactions to tattoo pigments are the most common adverse effects for both permanent and temporary tattoos. Risk of infection during permanent tattooing varies based on the causative pathogens, which may include bacterial, viral, fungal, and parasitic infections, manifesting as both localized and systemic issues. Adverse reactions have also been reported in relation to the pigment colors used in tattoos, suggesting that certain pigment components may be associated with harmful effects. A standardized regulatory framework is necessary to ensure the safety of tattoo inks and to prevent adverse health outcomes.

Abstract

Peningkatan kasus penyakit bulosa pada lanjut usia terkait berbagai faktor. Penuaan menyebabkan penurunan struktur dan integritas kulit, sehingga lebih rentan terhadap penyakit bulosa. Penyakit ini terjadi karena proses penuaan menyebabkan disregulasi system imun yang meningkatkan risiko kondisi autoimun. Penyakit bulosa yang sering terjadi pada lanjut usia biasanya adalah pemfigoid bulosa (kondisi autoimun bulosa) dan pemfigus vulgaris (penyakit kulit yang menyebabkan lepuhan) yang berhubungan dengan morbiditas dan mortalitas yang signifikan. Pemfigoid bulosa ditandai dengan bula subepidermal tegang, sedangkan pemphigus vulgaris ditandai dengan bula suprabasal kendur yang mudah pecah. Pemeriksaan histopatologi dan imunofluoresensi langsung merupakan baku emas diagnosis. Tata laksana penyakit bulosa autoimun pada lansia umumnya sama dengan penyakit bulosa pada umumnya, seperti corticosteroid dan agen imunosupresif. Namun, perlu diberikan tata laksana yang tepat dan efektif serta diberikan perhatian lebih lanjut terhadap risiko polifarmasi dan efek samping obat yang dapat memberikan masalah baru pada pasien lansia, termasuk peningkatan risiko infeksi, steoporosis, dan komplikasi metabolik.
