Abstract

Introduction: The timing of puberty is a crucial developmental milestone with significant implications for adolescent physical and psychosocial health. While secular trends towards earlier pubertal onset have been observed in many populations worldwide, data from Ambonese adolescents in Indonesia remains limited. This study aimed to investigate secular trends in pubertal timing among Ambonese adolescents and explore potential influencing factors. Methods: A retrospective cross-sectional study was conducted involving Ambonese adolescents aged 10-18 years who attended health clinics in Ambon City between 2008 and 2023. Data on age at menarche for girls and testicular volume for boys, along with socioeconomic and lifestyle factors, were collected from medical records. Age at menarche and testicular volume were used as markers of pubertal onset. Statistical analyses were performed to assess trends over time and identify associations with potential influencing factors. Results: A total of 1,542 adolescents (785 girls and 757 boys) were included in the study. The mean age at menarche decreased significantly from 12.8 years in 2008 to 12.3 years in 2023 (p < 0.001). Similarly, the mean testicular volume at age 14 increased significantly from 12.5 ml in 2008 to 14.2 ml in 2023 (p < 0.001), suggesting earlier pubertal onset in boys. Improved socioeconomic status and increased body mass index (BMI) were associated with earlier pubertal onset in both sexes. Conclusion: This study provides evidence of a secular trend towards earlier pubertal onset among Ambonese adolescents. Socioeconomic factors and BMI appear to play a role in influencing pubertal timing. Further research is needed to explore the underlying mechanisms and potential health implications of these trends.

Abstract

Measles is an infectious disease caused by a viral infection. The measles virus is known as Morbilivirus. Measles is a disease that causes a systemic rash and there are several clinical manifestations that can cause complications. The purpose of this study was to determine factors related to advanced measles-rubella immunization status in children aged 20-59 months in Labuha Village, Bacan District in 2023. This study was an observational analytic study with a cross-sectional approach using primary data obtained from 143 respondent. The sampling technique used is Simple Random Sampling. The results of this study showed that out of 143 children aged 20-59 months in Labuha Village who had received advanced measles-rubella immunization, 67 children (46.9%), while children who did not receive advanced measles-rubella immunization, were 76 children (53, 1%). The results showed that the related factors were education (p=0.004), knowledge (p=0.002), basic immunization status (p=0.034). The variable that has no relationship is the mother's employment status (p=0.102). The conclusion of this study is that there is a relationship between education level, level of knowledge, and children's basic immunization status. However, there was no relationship between the mother's employment status and the measles-rubella immunization status. It is hoped that the Labuha health center will improve education and outreach regarding immunization so that it can motivate people to take part in routine immunization programs.

Abstract

The process of sexual maturation, or puberty, is marked by the development of secondary sexual characteristics, rapid body growth, and, ultimately, the ability to reproduce. Various congenital and endocrine disorders can interfere with the timing of sexual maturation, causing delayed puberty or precocious puberty. This literature review aimed to describe puberty and its disorders and influencing factors. The age of puberty is multifactorial, involving genetic and environmental components. Delayed puberty is a physiological (constitutional) delay in which hormonal levels are normal, and the hypothalamic-pituitary-gonadal (HPG) axis is intact, but maturation occurs slowly. Precocious puberty can be partial, complete, or mixed type and can be further categorized into central (GnRH dependent) and peripheral (GnRH dependent). In conclusion, congenital and endocrine disorders can interfere with the timing of sexual maturation, causing delayed puberty or precocious puberty.
