Abstract

Objective: This study aims to review the usage and complication of paramedian approach for CAPD catheter insertion. Material & Methods: This is a retrospective study. All ESRD patients ongoing CAPD at the Dialysis Clinic of Zainoel Abidin General Hospital in Aceh, Indonesia between January 1, 2009 and December 31, 2018 were included in the present study. Complication outcomes included mechanical and infectious complications are documented and reviewed. Results: A total of 190 ESRD patients had been performed CAPD catheter insertion using paramedian approach in this period. Out of these patients, complication occurred in 31 cases (16.32%). All the complication that occurred in this study are 5 cases of leakage (2.63%), 9 cases of infection (4.74%), 8 cases of drainage problem (4.21%), 9 cases of malposition (4.74%), and no case of bowel perforation. From total of 9 cases of infections, 1 patient had peritonitis from the catheter insertion. The other infection was exit site infection. Conclusion: CAPD catheter insertion using paramedian approach is safe with low complication rates. It could decrease post-operative complications and can be recommended for CAPD catheter insertion technique due to its safety.

Abstract

Infertility is a global health problem affecting 1 of 10 couples in the world and its related to dyslipidemia. Dyslipidemia, as metabolic syndrome, had become global epidemic. It is estimated 12-37% Asian population and 12-26% of European population had dyslipidemia. Dyslipidemia decreased sperm count by increasing free radical oxygen species (ROS) caused of lypotoxicity and lipid peroxidation. Antioxidant is treatment choice for this problem. Drum stick (Moringa oleifera L) is medical herb that contain antioxidant compound such as alkaloid, flavonoid, tannin, vitamin C, E, and steroid. So, Moringa oleifera L leaf could protect sperm cells from free radicals of dyslipidemia and become alternative treatment for infertility. It was a post-test only control group design. Sample consist of 30 rats devided into 6 groups. It began with atherogenic diet to induce dyslipidemia. Then, Ethanol extract of Moringa oleifera L leaf was given. Sperm count was calculated microscopically with Neubauer chamber. Data analysis was using one way analysis of variance (ANNOVA) test, then continued with post hoc Duncan test. There were significant differences of total cholesterol, triglyceride, high density protein, low density protein, and sperm count between groups based on one way ANNOVA test (p < 0,05). On post hoc Duncan test, Dyslipidemia induction decreased sperm count compared to other groups. Ethanol extract of 200 mg/body weight and 400 mg/body weight leaf had no difference effect compared to atorvastatin. Ethanol extract of 800 mg/body weight leaf increased sperm count significantly compared to other groups.

Abstract

Pneumatic Lithotripsy (PL) is considered effective in fragmenting hard stones, and this procedure can also be performed transurethrally and percutaneously on large stones. This study aimed to see the percentage of Stone Free Rate (SFR) by administering force diuretics to ureteral stones that were crushed using PL. Previous studies have found that intravenous loop diuretic administration is effective, safe, and efficient in obtaining a much higher percentage of SFR and can reduce the average number of sessions per stone during extracorporeal shock wave lithotripsy (ESWL). We identified study subjects from early July-September 2022 and obtained as many as 38 study subjects (SP) with stones in the distal ureter <20mm. Subjects were randomly selected and divided into two groups: the group given diuretics (K2) and not given (K1). With a 90% confidence degree and 15% sampling error, based on Lameshow's formula, 30 SP can be observed; eight other subjects were excluded because they experienced a decrease in systolic pressure >25% (5 SP) and the remaining stones came out without splitting (3SP). The results obtained were SFR levels that were statistically significant (p<0.05) and clinically using the Mann-Whitney statistical test. Administration of diuretics to patients with distal ureteral stones undergoing URS with Pneumatic Lithotripsy is safe and effective as an anti-retropulsion method. It is the strategy of choice in centers that do not provide laser lithotripsy or flexible ureteroscope.

Abstract

Cancer is still one of the health problems around the world. Cancer is an uncontrolled (abnormal) cell division and can invade surrounding tissues and can also spread to other parts of the body through the blood and lymphatic system known as metastasis. This disease is often diagnosed in men aged 50-80 years with an average age of 73 years. Bladder cancer is divided into transitional cell carcinoma (95%), squamous cell carcinoma (3%), adenocarcinoma (2%), and less than 1% small cell tumors (paraneoplastic syndrome). Risk factors for this disease are smoking, exposure to carcinogenic substances, drugs, infection with the parasite Schistosoma haematobium, chronic irritation (stone disease), physical trauma (in the uroepithelial layer), infectious diseases and those that have not been proven to be the cause are coffee, alcohol, saccharin and cyclamate sweeteners. The prognosis of the disease depends on histologic examination to see the stage of the disease and by tissue biopsy. Methods: This paper is based on a literature search of clinical practice guidelines, scientific literature, websites, and textbooks on the topic of bladder cancer. Results and Discussion: Hematuria is the main clinical symptom in addition to other symptoms as a complaint of bladder cancer. The disease is divided into non-muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC). Non-invasive and invasive cancer conditions play an important role in the treatment and prognosis of the disease. MIBC is a disease condition with high morbidity and mortality. Conclusion: Cystoscopy followed by biopsy resection is the diagnostic standard followed by anatomic pathology examination (histology and cytology) for definitive diagnosis of the disease. The prognosis will be better if the disease is still at superficial and non-invasive stage (Ta), so that only transurethral tumor resection followed by chemotherapy drugs, intravesical and results will be more satisfactory.

Abstract

Trauma testis menjadi penyebab tersering ketiga dari nyeri skrotum akut yang dapat mengakibatkan berbagai tingkat kerusakan testis, sehingga dapat menyebabkan infertilitas. Trauma testis merupakan faktor risiko infertilitas terkait dengan adanya peningkatan radikal bebas, inflamasi yang berdampak pada penurunan kualitas sperma, dan kerusakan Blood Testis Barrier (BTB). Lima dari 9 testis yang mengalami trauma menjadi atrofi. Salah satu tatalaksana yang dapat dilakukan untuk mengatasi hal tersebut adalah dengan pemberian antioksidan. Tanaman kelor (Moringa oleifera)  mengandung beberapa senyawa antioksidan seperti senyawa golongan alkaloid, flavonoid, tanin, vitamin C, vitamin E, dan steroid. Diharapkan daun kelor juga dapat melindungi sel sperma dari oksidan bebas akibat trauma testis dan secara tidak langsung menjadi salah satu pilihan terapi infertilitas. Penelitian ini merupakan penelitian eksperimental menggunakan rancangan the post-test only control group design. Setelah dilakukan adaptasi selama 2 minggu, dibuat perlakuan torsio testis pada 30 ekor tikus rattus novergicus yang dibagi  ke dalam 5 kelompok perlakuan, kemudian dilakukan analisis sperma. Analisis data perbedaan antar kelompok menggunakan uji analisis of variance (ANNOVA) satu arah. Hasil penelitian menemukan konsentrasi spermatozoa (39.80 ± 17.88 x106 spermatozoa/ml) pada kelompok dosis 800 mg lebih tinggi dibandingkan kelompok dosis 400 mg (27.94 ± 14.76 x106 spermatozoa/ml), dan kelompok dosis 200 mg (25.32 ± 19.33 x106 spermatozoa/mL). Hal ini menunjukkan bahwa pemberian ekstrak tanaman kelor dosis 800 mg lebih berpengaruh atau mampu mempertahankan konsentrasi spermatozoa tikus wistar pada torsio testis dibanding  dosis 400 mg dan 200 mg. Penelitian ini menyimpulkan daun kelor memiliki potensi dalam tatalaksana infertilitas terkait trauma testis.
