Abstract

Junior High Schools in Srandakan had already implemented Trias UKS/M policies. However, there are still found some problems and cases in reality dealing with the implementation process, seen from the aspect communication, resources, disposition, and bureaucratic structure. This research was located in Junior High Schools Srandakan, Bantul. The research type was qualitative using descriptive approach. The data collection was done by intensive interviews using interview guidelines and observation using checklist sheet for multiple variables. The informants in this research were the Headmaster, UKS teachers, and Chairman of the PMR at each school, Head of Community Welfare Section, Head of TU UPT PPD Srandakan as well as Head of Puskesmas Srandakan. The results showed that the policy of implementation Trias UKS/M was not optimal because the implementation has reached 10 out of 21 indicators. It was influenced by communication among the executive teams in the schools was not going well, especially in transmission. The resource variables had not fully support the implementation of Trias UKS/M policies yet, the human resources already well, but the financial aspect was not enough and there was no Decree and the facilities were inadequate. The attitude/disposition variables had not entirely support                                                                                                              the policies because the support of the relevant authorities was still weak. The bureaucratic structure variables had not totally support because there was no SOP. Suggestions in this research are to improve the communications among the executive teams and the mentor teams as well as involve Chairman of PMR in coordination meetings.

Abstract

Demand for of goods in the dr. Loekmono Hadi Kudus Hospital with ahigh, abundance quantity and type of goods as well as fast mobility was needed. Ultimately, the planning and procurementof goods as well as the goods inventory in the hospital are not optimal yet. The purpose of this study was to determine the readiness of input, process and output in the hospital dealing with the management of goods inventory with a barcode system in dr. Loekmono Hadi Hospital Kudus. This was a qualitative research using intensive interviews and observation with the main informants of Goods Management Staff, Goods Storage Coordinator, Goods Storage Staff and triangulation informants of Household subsection Head and IPSRS Head. The results showed that dealing with input, the human resources are lacking in competence. Furthermore,regarding to infrastructure, therewas the unavailability of label printers and barcode scanners, the information system is not good enough, the absence job descriptionsof officers duties and function, and the lack of SOP. Dealing with the process, the planning aspect included the socialization that had been conducted, the organizing aspect covered the duties and function and SOP that had not been organized, the implementation of education and training that had not been conducted specifically by the hospital but had been given by the programmer, and the accountability report assessment of barcode system preparation. Regarding to the output, the readiness of hospitals was not well prepared due to the lack of human resources, the infrastructure was not complete, and the information system, the duties and function officers and SOP was unavailable. The suggestions in this research are to increase the understanding and competences of officers, develop the descriptions of duties and function and SOP as well as conduct specific training by the hospital regarding to the goods inventory management with barcode system.

Abstract

Abstract: One of the attempts in perform the function of financing in the Health System through Health Account. Health Account is a respresentative and innovative resources to describe the utilization of the flow of funds/ financing used for planning appropriate health budgets to solving health problems. In the implementation of drafting the Provincial Health Account in Central Java was still find problems. They are the absence of formal team, result of PHA have not been disseminated, the difficulty of collecting data, and the difficulty of presenting  relevant stakeholders. The purpose of this study was to analyze the decision-making adoption of stakeholders in the making PHA in Central Java province. The variables of this study is Knowledge Stage, Persuassion Stage, and Adoption Decision Stage. The study used a qualitative method with descriptive approach and interview guide was used as an instrument. Interviews were conducted with Health Department of Central Java, Planning and Regional Development Agencies of Central Jawa, Central Bureau of Statistics, Tugurejo Hospital, and Ministry of Health. The findings indicate that Knowledge Stage of stakeholders is good because all stakeholders have to known the concept and elements of the PHA. Persuasion Stage of stakeholders has been good enough for stakeholders expressed the making PHA has a relative advantage, compatibility with regional health system, can be made by Central Java, and can be seen the results even though the making PHA is considered quite difficult to do. Adoption Decision stage, all stakeholders declared accept innovation of the making PHA because it has advantages and stakeholders have made various preparations for the making PHA. Based on that all stages sufficient to support the decision to accept innovation of the making PHA.

Abstract

PONED is a health service to cope with cases of emergency obstetric and neonatal at the primary healthcare center level. Brebes that has the highest maternal mortality rate in recent years set 22 the primary healthcare center become PONED in 2015, one of which is Sitanggal. This purpose of this study is to describe and analyze the system implementation of PONED in the primary healthcare center Sitanggal from the aspects of input, process, output, feedback and environment. The type of observational study is a qualitative research with indepht interview. The study population were 4 key informants consisted of one team PONED (1 doctor and 2 nurses) with the heads of primary healthcare centers and 6 informants triangulation consisted of the kabid kesga DKK Brebes, 3 officers PONED (midwife) and 2 targets of PONED. The result shows that in the system implementation of PONED, from the aspect of input, there was no the assertiveness rules and an installed SOP as reference in PONED, there was a shortage of staff, there was only doctor in the morning shift, there was no specialized administrative staff, the condition of facilities and infrastructure were not supported the implementation of PONED yet. From the aspect of the process, there was no written document about organizing of PONED, some staffs did not obey the rules and another non-health staff still doubles as an staff in the primary healthcare center, the staff commitment was still difficult, there was no complete reporting document about the implementation of PONED in DKK. The aspect of output, the service by PONED in the primary healthcare center Sitanggal was low. From the aspect of feedback, there was no special evaluation that perfomed routinely by PONED. From the aspect of the environment, the support from the related parties was not maximed: The recommended suggestion are: to further reinforce the existing rules, the need for training and apprentice, procurement of equipment and better medicine, fostering the commitment to all related parties, reschedule the picket schedule, the need for coaching and routine evaluation in the implementation of PONED.

Abstract

Elderly is someone aged 60 years and over. The increasing number of elderly has received the attention of the government by making the elderly health as a priority by the issuance of PMK No. 67 of 2015. The report of the elderly health service coverage in Semarang City 2017 showed that there were 8 puskesmas that didn’t reach the target (67%). Puskesmas Bandarharjo has the lowest coverage and Krobokan has the highest coverage. To increase this coverage, puskesmas developed posyandu for elderly. A preliminary study at Puskesmas Bandarharjo showed that the coverage of pre-elderly and elderly services  was below the target, the elderly who were served in the posyandu were only 41,87%, the coverage of elderly who measured blood pressure at posyandu was fluctuated, peaking at 30.5% of the total elderly who were served. This study aims to analyze the implementation of elderly posyandu in Puskesmas Bandarharjo and Krobokan working area with a system theory approach. This study uses a qualitative research method with in-depth interview. Cadres are the main informants, while stakeholder of elderly programs at puskesmas, stakeholder of elderly program at  Semarang Health Department and the elderly are the triangulation informants. The result of this study shows that there were similarities in the problems between the two puskesmas including inadequate human resources, the lack of trainings for cadres, puskesmas staffs’ tardiness or absence, insufficient funds, incomplete facilities, the fact that the target of some posyandu is only elderly and doesn’t include the pre-elderly people, the lack of clarity in terms of organizational structure, posyandu did not monitor the elderly to check their Hb and blood glucose, and the lack of community support. Other problems found in puskesmas Bandarharjo include posyandu which had not been functioning for one year, the implementation of posyandu that had not been in accordance with guidelines, and the fact that not all of the cadres were able to write the report.

Abstract

Primary health care is a health service facility where organize first-rate individual health care efforts, one of which is outpatient care. Quality outpatient services so as to provide satisfaction to patients. One factors that needs to be considered is patient waiting time. There are still complaints about waiting times, speed of service, service facilities and summoning sources are less still found in the Tlogosari Wetan health center with the category of the number of visits including many but experienced a decrease in visits from the previous year. Objective: Analyze the factors related to the waiting time of outpatients in the Tlogosari Wetan Health Center in Semarang City according to the patient's perception. Method: This research was using quantitative model with with analytical survey approach and cross sectional research design. Samples totaled 100 with accidental sampling. The result data of data was number with Chi-Square test. Result:The data showed that the majority of respondents aged adult, female and high school educated. Most respondents were adults, female and medium educated. The average registration service time is 3.81 min, the main service is 7.6 min, supporting services are 2.36 min. Average registration waiting time is 15.28 min, main service is 23.83 min, supporting service is 11.38 min. Respondents' perceptions of service time, service discipline, service facilities, the source of the call and waiting time are good. Variables related to the perception of outpatients waiting time at the Tlogosari Wetan Health Center in Semarang City included perceptions of service time (p = 0.018), perception of service discipline (p = 0.01), perception of service facilities (p = 0.000). While the variables that are not related include perception of the source of calling (p = 0.280). Suggestion: Print larger and strategically placed online registration information, place reading material to look and add the latest reading material, as well as periodic checks and maintenance of facilities and sources of calling.

Abstract

Public Health Centers as public sector organizations that serve the health sector can not be separated from the community's assessment of the increasingly critical of its performance. One of the ways used by Health Office in assessing the success of Puskesmas is by conducting Puskesmas Performance Assessment. Candilama Health Center is one of the urban-type community health centers in Semarang City. Candilama Health Center performance assessment results in the last 4 years starting from 2012 shows a declining trend every year. The purpose of this research is to analyze the factors that are suspected to be the cause of the poor performance of the Candilama Health Center organization. This study is a qualitative research with in-depth interviews to program implementers in Health Center and informants triangulation Head of Puskesmas, Sub Division of Planning and Evaluation of Health Office, Head of Primary Health Service Section of Health Office.The result of the research shows the factors that cause the low performance of Candilama Public Health Center are, the availability of the number and competency of health personnel are still lacking, the supervision process from the Head of Health Center is still not good, the unmistakable leadership of the Head of Health Center in making decisions, work motivation from colleagues and leadership perceived less, and still weak organizational culture at Candilama Community Health Center. To overcome this problem, it is suggested that Candilama Head of Puskesmas can change their leadership style, all employees are expected to build good communication among employees so as to reduce misunderstanding, reward and proper recognition appropriately to employees with achievement of work achieved, and vice versa, also doing recreation together so that all employees feel close to other employees and leaders to further tighten the bonds. Health Office is expected to open the recruitment of health workers who are still lacking in Public Helath Center Semarang City in cooperation with BLUD Puskesmas and willing to provide training to all program implementers in Puskesmas both civil servants and non-civil servants.

Abstract

Hipertensi atau tekanan darah tinggi merupakan penyakit kelainan jantung dan pembuluh darah yang ditandai dengan peningkatan tekanan darah. Hipertensi merupakan salah satu indikator SPM Bidang Kesehatan Kota Palangka Raya yang belum mencapai target standar pelayanan minimal yaitu hanya 57,27% dari target 100%.Tujuan penelitian ini untuk menganalisis implementasi kebijakan SPM penderita Hipertensi di puskesmas kota di Palangka Raya. Jenis penelitian adalah metode kualitatif deskriftif dengan indepth interview dengan informan penelitian Koordinator pemegang program Hipertensi, kepala puskesmas dan pemegang program PTM bagian Hipertensi DKK Palangka Raya yang dilakukan pada bulan November-Januari 2022. Hasil penelitian diketahui bahwa dalam pelaksanaan layanan sesuai SPM penderita Hipertensi masih belum optimal, minimnya koordinasi dari pihak dinas dan puskesmas mengenai kebijakan SPM,dan masih sebagian sasaran yang tidak melakukan pengobatan hipertensi secara rutin ke Puskemas, tenaga Kesehatan di puskesmas yang memiliki tugas ganda diluar kegiatan khusus Hipertensi sebagai petugas vaksin selama masa pandemi Covid-19. Peneliti memberikan saran agar puskesmas mengadakan sosialisasi kepada petugas Kesehatan mengenai pedoman Standar Pelayanan Minimal bidang Kesehatan sehingga seluruh petugas Kesehatan tidak hanya mengetahui namun juga memahami masing-masing indikator beserta langkah-langkah pelayanannya terkhususnya bagi pemegang program dan melakukan edukasi dengan lebih optimal lagi kepada pasien Hipertensi agar pasien lebih paham dengan penyakit yang dideritanya. Dinas Kesehatan diharapkan dapat membangun komunikasi yang lebih baik lagi dengan pihak puskesmas agar tercapainya standar pelayanan minimal penderita Hipertensi dan Dinkes rutin memonitoring dan evaluasi kepada seluruh puskesmas di Kota Palangka Raya mengenai capaian Standar Pelayanan Minimial (SPM) yang harus dicapai oleh Puskesmas, untuk menghindari pemahaman data yang berbeda antara puskesmas dan dinas kesehatan

Abstract

Background: The high number of positive cases of Covid-19 in Indonesia has made the Government of Indonesia stipulate it as a national non-natural disaster. Various obstacles related to hospital services emerged due to a significant increase in patients at the same time. Given these constraints, there needs to be a hospital management response to maintain health services at the hospital to continue to run well. Objective: The purpose of this study is to get an overview of the response of hospital management in dealing with the Covid-19 pandemic in 2021 through the rapid hospital readiness for Covid-19 instrument recommended by WHO. Methods: This research is a qualitative research with a phenomenological approach. Data collection was carried out through interviews with five main informants and 17 triangulation informants, as well as through observation. Results: showed that from the assessment of the 12 components of rapid hospital readiness for covid-19, it was found that components could not be implemented optimally, namely leadership and incident management systems (86%), administrative, financial and business continuity (87%), patient management (88%) ), proper identification and diagnosis (83%), PPI (94%) and the lowest is occupational health, mental health and psychosocial support (40%). Conclution: The management response of Kendal Hospital in dealing with the 2021 Covid-19 pandemic has been good, the lack of implementation of the rapid hospital readiness instrument for Covid-19 is technical.

Abstract

Background: HIV remains a global health issue, with 1.65 million adolescents affected worldwide. In Indonesia, HIV cases continue to rise, especially among youth. In Semarang, adolescent knowledge about HIV is still low. Given their high use of digital media, developing youth-friendly social media content is essential to improve HIV prevention and education. Methods: This study applied a mixed-methods strategy with an explanatory sequential design. The quantitative phase involved collecting data through an online questionnaire using Google Forms, targeting 211 adolescents aged 16 to 18 years in Sub-district X, Semarang City. For the qualitative phase, in-depth interviews were conducted with one adolescent engaging in risky sexual behavior, a counselor from the Central Java Provincial National Anti-Narcotics Agency, and a counselor from a drug rehabilitation center in Semarang City. All data were examined using content analysis. Results: The study involved 211 adolescents aged 16–18 in Semarang. Only six reported having had sexual intercourse, most without consistent condom use. None reported injecting drug use. While most adolescents had received HIV information from teachers or health workers, they preferred online platforms like Instagram. Many had misconceptions about HIV transmission, such as believing it can spread through kissing or mosquito bites. Adolescents showed more concern about pregnancy than HIV and considered sex a taboo topic. Most preferred short, creative videos, especially podcasts and humorous content, with maximum duration of 45 seconds. Preferred topics included HIV basics, mental health, prevention, healthy relationships, and stories of people living with HIV. These findings support the development of youth-friendly social media content for HIV education.. Conclusion: Adolescents prefer learning about HIV through engaging social media content, especially short videos and podcasts. Despite some basic knowledge, misconceptions remain, and sexual health is still a sensitive topic. These findings highlight the need for youth-friendly digital content to improve HIV prevention.
