Abstract

Karsinoma sel skuamosa (SCC) merupakan tumor ganas langka yang dapat timbul dari osteomielitis kronis dengan berbagai gambaran klinikopatologi lesi. Kami melaporkan SCC yang disebabkan oleh osteomielitis kronis. Laki-laki 68 tahun datang dengan keluhan kaki kemerahan, edema, gangrene yang membuat kesulitan berjalan. Benjolan pertama kali diketahui setelah pelepasan open reduction internal fixation (ORIF) pada tahun 2022 pasca trauma yang dialaminya 18 tahun lalu. Pasien memiliki riwayat diabetes mellitus. Pada pemeriksaan foto rontgen cruris dextra AP lateral lesi litik geographic type dan lesi litik moth eaten type pada tibia yang mengarah pada gambaran osteomielitis. Selain itu, ditemukan lesi dengan ukuran 9,3x6,9x16cm pada distal femur dan proksimal tibia kanan pada MRI tanpa kontras lesi mendestruksi epifisis hingga cartilago pada distal femur dan proksimal tibia kanan yang menunjang pada malignant solid mass pada MRI kontras serta jaringan terfragmentasi. Sel tersebut tapak tersusun solid nest dan trabecular dengan kesan infiltratif ke jaringan sekitar. Tampak area nekrosis dengan sel PMN disekitarnya yang mengarah pada well differentiated squamous cell carcinoma (SCC) pada biopsi. SCC yang disebabkan oleh osteomielitis pasca pelepasan ORIF merupakan kasus yang jarang. Mekanisme yang diakibatkan oleh infeksi dan inflamasi diperparah oleh DM.

Abstract

Indonesia as an agricultural country relies heavily on its corps as the main sector of nation’s development. On daily basis, many farmers use pesticide to increase corps productivity. However, pesticides were known to induce malignancy, one of them being gastric cancer; the fourth most mortal cancer in the world, with less than 12 month life expectancy on advanced stage, thereby making it a global health problem. This research used systematic review and meta-analysis methods to examine pesticide exposure variables on gastric cancer cases. Three journals were found, and meta-analysis results showed that there was a significant increase in the risk of gastric cancer in respondents who experienced pesticide exposure, especially with prolonged exposure with an odds ratio of 1.47 (more than two years). However, this review cannot explain the toxicity dose of pesticides and the nutritional status of respondents which can increase the incidence of gastric cancer due to the lack of research on related variables.

Abstract

Dengue fever (DF) is mostly common in certain geographical areas, such as Indonesia, where it continues to be public health problem due to its nature as a viral infection spread through mosquitos. The focus of this study is to assess the factors affecting length of hospital stay (LOS) among DF patients in Buleleng Regency. Between July and December 2024, a cross-sectional investigation was carried out at many hospitals, retrieving the medical records of 940 DF patients. The Spearman test was used in statistical analysis to examine the association between LOS and clinical indicators. The majority of patients (60.96%) were youngsters, 95.5% were admitted to the paediatric ward, and their recovery rate (99.89%) and mortality rate (0.11%) were also high. LOS was significantly associated with leukocyte levels, fever duration, and severity of the disease (p<0.05). However, age, gender, hematocrit, and platelet levels did not show significant association with LOS (p>0.05). This reveals that leukocyte count, duration of fever, and severity of DF contribute in determining hospital stay duration while discharge criteria as such degree of anemia and thrombocytopenia do not appear to be reliable predictors. It is suggested that there is a gap in literature that needs to be looked into regarding the genetic clinical determinants of LOS patients with DF needs further evaluation.
