Abstract

Traffic incidents become typical cases of trauma outside of hospitals. The integrated-emergency nursing system outside of hospitals becomes an important component to minimize any worsening condition of the traumatic victims. However, this pre-hospitalized service system has not been formally established in several cities or municipalities. This research aims to explore various hopes of the trauma-victim aid process in a region that has not applied pre-hospitalized emergency service formally. This research was carried out by implementing a qualitative research method and a case study approach to identify the themes. The research was conducted in Konawe municipality, from February until March 2020. It was done by interviewing eight participants consisting of the citizens, traffic police officers, hospital party, and health agency. Four themes result from this research. Establishing the pre-hospitalized emergency service, socializing the first aid for the incident victims, training dealing with the first aid of taking care of trauma victims, and promoting socialization for all citizens about the emergency numbers of health service institutions which could be immediately contacted when an accident occurs. There is a need for an emergency service system establishment and development.

Abstract

Sindrom Koroner Akut (SKA) merupakan suatu keadaan kegawatdaruratan jantung dengan manifestasi klinis berupa keluhan perasan tidak enak atau nyeri dada yang disertai dengan gejala lain akibat dari iskemia miokard. Penanganan kasus SKA di Instalasi Gawat Darurat (IGD) membutuhkan tindakan yang cepat dan efisien untuk mencegah terjadinya perburukan kondisi pada pasien SKA. Terjadinya pemanjangan lama waktu tinggal pasien di IGD dikatakan menjadi salah satu indikator kurang efektifnya manajemen pelayanan pasien di IGD. Tujuan penelitian ini untuk mengetahui faktor-faktor yang berhubungan dengan lama waktu tinggal pasien SKA di ruang emergency jantung PJT RSUP Sanglah Denpasar.Penelitian ini merupakan penelitian prospektif dengan desain penelitian observasional analitik. Sampel dalam penelitian ini berjumlah 40 orang dengan teknik simple random sampling. Uji yang digunakan adalah Uji Bivariat Korelasi Pearson.Hasil korelasi Pearson menunjukkan bahwa tidak ada hubungan kecepatan interpretasi hasil EKG (p=0,259) dengan lama waktu tinggal pasien di ruang emergency jantung PJT RSUP Sanglah Denpasar dan ada hubungan waktu keputusan persetujuan perawatan (p<0,001) dengan lama waktu tinggal pasien di ruang emergency jantung PJT RSUP Sanglah Denpasar.Diperlukan adanya upaya dari petugas kesehatan dalam meningkatkan pengetahuan dan pemahaman pasien dan keluarga tentang penanganan pasien SKA yang membutuhkan waktu cepat serta perawatan yang intensif sehingga diharapkan dapat membantu dalam mempercepat pemberian keputusan kebersediaan perawatan pasien SKA dari pihak keluarga maupun pasien itu sendiri.

Abstract

Emergency conditions of the acute coronary syndrome (ACS) requires rapid and precise management. Prehospital delay time can result in deaths attributed to health seeking behavior and transportation mode. This research aimed to analyze the correlation of health seeking behavior and transportation mode with prehospital delay time in patients with Acute Coronary Syndrome at Emergency Department of Regional Public Hospital of dr. T.C. Hillers. The research design was observational analytic with cross sectional approach. The sample was 42 respondents obtained by using consecutive sampling. Data were collected with an interview in April-June 2017, then analyzed with univariate analysis and bivariate (Fisher test). The results showed that the majority of health seeking behavior of ACS patients was delayed to hospital (76.2%) and the mode of transportation used was public transportation (31%). Prehospital delay time most of which arrived late (> 120 min) (61.9%). Result of fisher test show there is correlation of health seeking behavior and transportation mode with prehospital delay time in patients with Acute Coronary Syndrome at Emergency Department of Regional Public Hospital of dr. T.C. Hillers. The results indicate the importance of improving patient referral systems, provide health counseling, provide village ambulances, support and organize Emergency Medical Services.

Abstract

The mortality rate concerning post-traffic accident trauma is estimated to gets higher for the next years. When a traffic accident occurs, the provision of first aid is carried out by laypeople/communities around the scene with limited knowledge and equipment available. Especially, if a traffic accident occurs in an area that has not applied an integrated pre-hospital service. This research aims to explore the first responders’ care administration in a region that has not applied pre-hospitalized emergency service formally. This is a qualitative study with a case study approach. This approach is chosen to identify emerging themes. The research was conducted in Konawe municipality, from February until March 2020. It was done by interviewing 8 participants consisting of the citizens, traffic police officers, hospital party, and health agency in the research site that had been selected based on the applied inclusion criteria. Five themes were found in this research. First responders struggle to save the traffic accident victims after the incident. They helped accident victims with limited tools. They helped the accident victims by sending them off to a health service immediately. They attempt to help the victims by sending them off to health services by using public transportation. Support Police action when finding accident victims. It could be concluded that in the area that has not applied integrated-emergency pre-hospital service would directly influence service availability in the society. The people would struggle to substitute the roles of medical workers in administering the first aid outside of a hospital with various limitations. Education and training programs concerning administering first aid, given quarterly, could be an appropriate health service program to offer for broader people as the first responders.
