Abstract

Sleep disturbance is a common problem that is often experienced by hemodialysis patients, and can have an impact on sleep quality. This needs attention so that the causative factors and effective treatment methods can be carried out. This article was written using the literature review method. The articles were taken from the online databases namely ScienceDirect, Springer, ClinicalKey, and Pubmed. The results of the study identified factors related to sleep problems of hemodialysis patients, namely low serum vitamin D levels, snoring, pain, napping during the day, respiratory problems, pruritus, good dialysis index (GDI) scores, heavy symptom distress, excess sadness fluid before dialysis, sex, age, triceps skinfold, hypoproteinemia, hyperkalemia, hyperpospatemia, and poor nutritional status, frequency of HD, restless legs syndrome (RLS). While suitable method to improve the sleep quality of HD patients include listening to music, sleep hygiene education, aromatherapy through inhalation, continuous care models (CCM), while acupressure does not have a significant impact in improving sleep quality of HD patients. Adoption or modification may be needed in order to get more effective results in improving the sleep quality of HD patients. Keywords: sleep quality; factors; hemodialysis patients ABSTRAK Gangguan tidur merupakan masalah umum yang sering dialami pasien hemodialisis, serta dapat berdampak pada kualitas tidur. Hal ini perlu mendapat perhatian agar faktor penyebab dan metode penanganan yang efektif dapat dilakukan. Artikel ini ditulis dengan metode literature review dengan pencarian melalui di online database ScienceDirect, Springer, ClinicalKey, dan Pubmed. Hasil studi mengidentfikasi faktor-faktor yang terkait dengan maslah tidur pasien hemodialisis yaitu kadar vitamin D serum yang rendah, ngorok, nyeri, napping di siang hari, masalah pernafasan, pruritus, skor good dialysis index (GDI), gejala distres yang berat, kelebihan cairan sebelum dialisis, jenis kelamin, usia, lipatan kulit trisep, hipoproteinemia, hiperkalemia, hiperpospatemia, dan status nutrisi yang buruk, frekuensi HD, restless legs syndrome (RLS). Sementara metode yang cocok untuk meningkatkan kualitas tidur pasien HD diantaranya yaitu mendengarkan musik, edukasi sleep hygiene, aromaterapi melalui inhalasi, continuous care model (CCM), sedangkan akupresur tidak berdampak signifikan dalam peningkatan kualitas tidur pasien HD. Adopsi maupun modifikasi mungkin diperlukan agar mendapatkan hasil yang lebih efektif dalm meningkatkan kualitas tidur pasien HD. Kata kunci: kualitas tidur; faktor; pasien hemodialisis

Abstract

ABSTRAK Pendahuluan: Kemampuan self-care merupakan hal penting dalam meningkatkan kualitas hidup pasien DM. Penelitian bertujuan mengidentifikasi hubungan self-care dengan kualitas hidup pasien DM. Metode: Rancangan penelitian cross sectional, melibatkan 125 anggota PERSADIA cabang Cimahi. Alat ukur self-care adalah Summary of Diabetes  Self-Care Activities (SDSCA), Diabetes Quality Of Life (DQOL) dan Beck Depression Inventory II. Hasil: Hasil penelitian menunjukkan hubungan self-care dengan kualitas hidup menjadi tidak bermakna (p value 0.164) setelah dipengaruhi oleh jenis kelamin (p value 0.006) dan  depresi (p value 0.001). Diskusi: Peningkatan satu satuan self-care, akan meningkatkan kualitas hidup sebesar 6.1% setelah dikontrol oleh jenis kelamin dan depresi. Peningkatan self-care dapat dilakukan melalui pengembangan program edukasi yang terstruktur, meningkatkan kompetensi perawat dalam memberikan asuhan keperawatan pada pasien DM terkait aktivitas self-care, dan melakukan screening depresi terhadap pasien DM. Kata kunci: Self-care, kualitas hidup, DM   ABSTRACT Introduction: The relation between self care and patient’s diabetes mellitus quality of life in Persatuan Diabetes Indonesia    (PERSADIA) in Cimahi. Self care ability is important in improving patient’s quality of life (QOL). Method: Using cross sectional method, this research is designed to identify the relationship between self care and patient’s QOL in PERSADIA Cimahi, West Java. A hundred twenty five PERSADIA members were recruited and examined using Summary of Diabetes  Self-Care Activities (SDSCA),Diabetes Quality Of Life (DQOL) and Beck Depression Inventory II. Result: The results showed no significant correlation between self care activity and QOL (p=0,164) as influenced by gender (p=0,006), depression (p=0,001). Discussion: Increase of one unit self-care was likely to increase 6,1% QOL after controlled by gender and depression. Self care improvement can be performed through developing structured education, improving nurse’s competency in diabetes care and conducting diabetes screening program for DM patients. Abstract

Hemodialysis is the most preferred renal replacement therapy. Besides having a therapeutic effect, hemodialysis also has side effects, namely intradialysis complications. The intradialysis complications are related to Interdialytic Weight Gains (IDWG) because if patient have higher IDWG level, make more fluid will be excreted during hemodialysis. The purpose of this study was to determine the description of intradialysis complications based on the IDWG level. This research used a descriptive design involving 90 hemodialysis patients in one of hospitals in Malang. The data collection technique was carried out by observing the patient every hour during hemodialysis. The results of this study indicate that the higher the IDWG level, the higher the number and frequency of intradialysis complications. Nurses need to optimize their role in assessing, monitoring, planning and documenting patients undergoing hemodialysis so that they can reduce and prevent problems that may be experienced by patients during hemodialysis.

Abstract

Angka diabetes mellitus (DM) setiap tahun meningkat secara global, Edukasi merupakan pilar penting karena dapat meningkatkan literasi kesehatan yaitu peningkatan pengetahuan dan kemampuan mengelola perawatan diri/self care bagi penderita DM. Systematic review ini bertujuan untuk mengidentifikasi model edukasi yang digunakan perawat untuk meningkatkan literasi kesehatan pasien DM dalam perspektif manajemen. Systematik review ini mengikuti pedoman PRISMA. Penelusuran dilakukan secara sistematis pada 5 database berbahasa inggris: Pubmed, Proquest, Science direct, Scopus dan Sage, dengan menggunakan kata kunci education model, health education model, diabetic, diabetes mellitus, health literacy, management. Artikel dipilih yang dipublikasikan sejak tahun 2013-2023. Model edukasi dianalisis menggunakan perspektif manajemen. Perencanaan edukasi; tujuan, kebutuhan, sasaran , prosedur.  Pengorganisasian; tim edukator, pembagian tugas dan koordinasi. Kepegawaian; kompetensi dan tanggung jawab staf memberikan edukasi. Pengarahan; motivasi, kolaborasi dan koordinasi, identifikasi masalah edukasi. Pengontrolan: kendali mutu edukasi, literasi kesehatan pasien DM dan hasil klinis. Perencanaan; edukasi dirancang secara proaktif yang bersifat dinamis. Pengorganisasian; edukator dibentuk dalam tim interprofesional, tugas dan tanggung jawab tiap profesi. Kepegawaian; tiap edukator memiliki kompetensi profesional. Pengarahan; dukungan pimpinan merupakan support system bagi edukator. Pengontrolan; kendali mutu edukasi dengan mengidentifikasi hambatan edukasi karena budaya dan gaya hidup pasien.

Abstract

Background: Chronic kidney disease is one of the global public health challenges that continues to increase with a significant impact on morbidity, mortality and high health care costs. Hemodialysis, one of the renal replacement therapy methods, often causes anxiety and decreased quality of life. Mind body therapies such as guided imagery, yoga and meditation have the potential to reduce anxiety and improve the quality of life of patients undergoing hemodialysis. Purpose: To evaluate the effectiveness application of mind body therapies on anxiety and quality of life in patients undergoing hemodialysis. Method: This systematic review uses an approach that follows the PRISMA guidelines. Searches were conducted in six electronic databases, namely Scopus, Springerlink, Proquest, EBSCO, Science Direct and Pubmed with appropriate keywords. The included articles were published within the last 10 years, in English and the type of research was RCT or quasi-experimental. After the selection process, 10 articles met the criteria for further analysis. Results: Eight articles were RCT designs and two articles were quasi-experimental designs. The application of mind body therapies including guided imagery, yoga and meditation is a complementary therapy that can reduce anxiety and improve quality of life. In addition, it also affects the improvement of sleep quality, reduces depression, reduces stress levels and emotional regulation in patients undergoing hemodialysis. Conclusion: Mind body therapies have been proven effective in reducing anxiety and improving the quality of life of hemodialysis patients. This therapy can be used both during hemodialysis sessions and when done alone at home. With its various benefits, this therapy has the potential to be integrated into holistic and evidence-based patient care.
