Abstract

Background: Every single person including people living with HIV/AIDS (PLWHA) have five basic needs; one of which is a physiological sexual need. Unfortunately, PLWHA face many difficulties in fulfilling their sexual needs due to the stigma and fear of infecting their partner.Purpose: The purpose of this study was to explore the lived experience of PLWHA and partner’s responses toward the fulfillment of sexual needs.Methods: This study was a qualitative descriptive phenomenological research, which collected data through in-depth interviews from fourteen PLWHA in Paramacitta Spirit Foundation, Denpasar. The participants were recruited through snowball sampling, informed and gave consent for their voluntary participation. The data were analyzed using Colaizzi’s phenomenological method.Results: The study revealed five themes describing the sexual experiences of PLWHA, including: (1) the fulfillment of sexual need of PLWHA, (2) the roles of partner for PLWHA, (3) various perceived stimuli which could increase the sexual desire, (4) the perceived factors which could incapacitate the ability and quality of sexual desire, and (5) the efforts to increase sexual capability and quality.  Conclusion: Sex was an important need for PLWHA to meet. Despite having a virus, after undergoing anti-retroviral (ARV) treatment, PLWHA felt healthier, and thus they could meet their sexual need like a normal person in general.

Abstract

Background: Educational interventions about stroke have been carried out by the health workers, one of which is by the nurse. However, the majority of the interventions focus more on the stroke patients than the stroke caregivers. Meanwhile, post-stroke patient care education interventions are urgently needed by the caregivers. Therefore, the author intends to review current literature about post-stroke patient care education interventions on stroke caregivers. Objective: This literature review aims to identify the effectiveness, methods, and impacts of daily care educational interventions for post-stroke patients on stroke caregivers. Methods: The method utilized was by analyzing online databases about the related topics from reputable sources.  Seven articles that met the inclusions criteria considering Randomized Controlled Trial (RCT) and Quasi-experimental design, time of publication, and English full text were obtained, and the results were presented. Result: Educational intervention could be carried out by several methods, including direct education at the patient's bedside, lectures followed by questions and answers, and booklets, home visits or telephone follow-up. Educational interventions could improve caregiver knowledge and skills in providing daily care for post-stroke patients, reduce burdens, reduce anxiety and improve the caregiver's quality of life. Educational interventions also positively impacted post-stroke patients, including improving the quality of life, functional abilities and the ability to do daily activities (ADL), reducing cognitive impairment, anxiety, and depression.Conclusion: Daily care education interventions for post-stroke patients can reduce the burden on the caregiver, either physical, psychological or social burdens.

Abstract

AbstrakFatigue dan depresi merupakan gejala yang sering terjadi pada pasien hemodialisa dan dapat mempengaruhi buruknya kualitas hidup. Tujuan dari penelitian ini adalah untuk mengetahui tingkatan fatigue dan depresi dan hubungannya terhadap kualitas hidup pada pasien hemodialisa. Hasil penelitian ini dari sebanyak 105 pasien sebagian besar pasien (57,1%) mengalami fatigue dan sebagian pasien tidak mengalami depresi (67.6%). Penelitian ini menghubungkan dengan kualitas hidup dan didapatkan hasil adanya hubungan signifikan antara fatigue, depresi terhadap kualitas hidup (p=0.000 dan p=0.001). Depresi merupakan faktor yang paling dominan mempengaruhi kualitas hidup pasien hemodialisa (koef B=4.868).Rekomendasi dari penelitian ini adalah perlu dilakukannya deteksi awal depresi dan upaya promotif dan preventif untuk meminimalisir terjadinya depresi pada pasien hemodialisa.

Abstract

Heart failure is a cardiovascular disorder with a high prevalence, morbidity and mortality. The prevalence of heart failure is predicted to increase to 46% in 2030 which is estimated to reach more than 8 million cases in people aged more than 18 years. The quality of life of patients with heart failure is indirectly affected by the condition of the damage to the anatomy and physiology of the heart. More over, it can also result in changes in the psychological condition due to the worsening of the disease. The quality of life is reflected in the limitation of mental and physical activity and decreased social role. The symptoms of the psychological disorder such as anxiety, anger, depression and hostility. Those both will influence each other so that the healing and recovery process of heart failure will be hampered, it can be worse, which in turn will increase re-hospitalization and death rates. The purpose of this literature review was to examine the impact of psychological aspect, especially depression and anxiety in patients with heart failure. The articles were searched through google search, such were BMJ, BMC Psychology, Heart Journal, JAMA and school or university journal. Those articles were published between 2000 – 2019, and selected through the keyword depression, heart failure, self care, then critically examined using CASP guidelines. Those articles should discuss (1) the relationship between psychological factor such as depression or anxiety on heart failure (2) tke mechanism of depression that affect heart faiure and (3) the influence of psychological factors on self care of heart failure patient. This study revealed that the prevalence of depression in heart failure is quite high, around 19-63%. The emergence of depression, anxiety and other psychological disorders are influenced by several factor such as age, gender, level of education, economic status and education about the disease. Patient’s perceptions about the disease are related to depression and anxiety cases of heart failure patients. The interaction between the physical response to heart failure and the neuron connections will cause psychological disorders in heart failure patients. Psychological factors are often forgotten by health workers so that patients feel more “alone” in dealing with heart failure. Depression, anxiety and other psychological disorder often occur in patients with heart failure. The prevalence is quite high around 19-63%. There are no theories and studies that explain precisely how this disorder can occur, but from the literature review it is suspected that this disorder occurs due to neuro-hormonal responses in the central and peripheral neural networks that are disturbed due to heart failure. Inverselly, psychological disorder can aggravate existing heart failure condition. These two things were interrelated and will affect the recovery process of heart failure. Although often forgotten, depression and other psychological disorders are strong predictors of self-care behavior and quality of life and re-hospitalization of heart failure patients. Keywords: depression; heart failure; self-care ABSTRAK Gagal jantung merupakan salah satu ganguan kardiovaskuler dengan prevalensi yang tinggi dan angka morbiditas dan mortalitas yang tinggi pula. Prevalensi gagal jantung diprediksi semakin meningkat hingga 46% pada tahun 2030, yang diperkirakan akan mencapai lebih dari 8 juta kasus pada penduduk berumur 18 tahun keatas. Penyakit ini dapat menurunkan produktifitas penderitanya, menurunkan kualitas hidup dan sering mengalami perawatan ulangan. Kualitas hidup pasien gagal jantung secara tidak langsung dipengaruhi oleh kondisi kerusakan anatomi dan fisiologi jantung. Selain itu juga dapat mengakibatkan perubahan kondisi psikologis akibat semaikn memburuknya kondisi penyakitnya. Kualitas hidup ini tercermin dengan adanya keterbatasan aktifitas fisik, mental dan penurunan peran sosial. Adapun gejala gangguan psikologis seperti cemas, marah, depresi dan rasa permusuhan. Kedua hal tersebut akan saling mempengaruhi sehingga akan mengakibatkan proses penyembuhan dan pemulihan gagal jantung menjadi terhambat, bahkan dapat semakin memburuk yang pada akhirnya akan meningkatkan angka rawat inap ulang dan angka kematian. Tujuan dari review literatur ini adalah untuk menelaah pengaruh aspek psikologis khususnya depresi dan ansietas pada pasien gagal jantung. Artikel dicari melalui google search, seperti BMJ, BMC Psychology, Heart Journal, JAMA dan jurnal sekolah atau universitas Kemudian dipilih artikel yang terbit antara tahun 2000 – 2019, melalui kata kunci gagal jantung, depresi, self care, kemudian dilakukan telaah kritis dengan menggunakan pedoman dari CASP. Artikel yang dimaksud harus membahas mengenai (1) hubungan antara faktor psikologis seperti depresi atau kecemasan terhadap gagal jantung, (2) mekanisme terjadinya depresi yang mempengaruhi gagal jantung dan (3) pengaruh faktor psikologis terhadap self care pasien gagal jantung. Dari penelitian ini terungkap bahwa prevalensi depresi pada gagal jantung cukup tinggi, sekitar 19 – 63%. Munculnya depresi, ansietas dan gangguan psikologis lainnya dipengaruhi oleh beberapa faktor seperti usia, gender, tingkat pendidikan, status ekonomi dan edukasi tentang penyakit. Interaksi antara respon fisik akibat gagal jantung dan koneksi neuron akan menyebabkan munculnya gangguan psikologis pada pasien gagal jantung. Persepsi pasien tentang penyakitnya juga berhubungan dengan kejadian depresi dan kecemasan pasien gagal jantung, Faktor psikologis sering terlupakan untuk diperhatikan oleh petugas kesehatan sehingga pasien semakin merasa “sendiri” dalam menghadapi penyakit gagal jantungnya. Kejadian depresi, ansietas dan gangguan psikologis lain sering terjadi pada pasien dengan gagal jantung. Prevalensinya cukup tinggi sekitar 19-63%. Gangguan ini bisa terjadi karena adanya respon neurohormonal pada jaringan saraf pusat dan tepi yang terganggu akibat gagal jantung. Sebaliknya, gangguan psikologis yang terjadi dapat memperberat kondisi gagal jantung yang telah ada. Kedua hal ini saling berkaitan dan akan mempengaruhi proses pemulihan gagal jantung. Meskipun sering terlupakan, depresi dan gangguan psikologis lain ini merupakan prediktor kuat terhadap perilaku self care, rawat inap ulang serta kualitas hidup pasien dengan gagal jantung. Abstract

Sleep disturbance is a common problem that is often experienced by hemodialysis patients, and can have an impact on sleep quality. This needs attention so that the causative factors and effective treatment methods can be carried out. This article was written using the literature review method. The articles were taken from the online databases namely ScienceDirect, Springer, ClinicalKey, and Pubmed. The results of the study identified factors related to sleep problems of hemodialysis patients, namely low serum vitamin D levels, snoring, pain, napping during the day, respiratory problems, pruritus, good dialysis index (GDI) scores, heavy symptom distress, excess sadness fluid before dialysis, sex, age, triceps skinfold, hypoproteinemia, hyperkalemia, hyperpospatemia, and poor nutritional status, frequency of HD, restless legs syndrome (RLS). While suitable method to improve the sleep quality of HD patients include listening to music, sleep hygiene education, aromatherapy through inhalation, continuous care models (CCM), while acupressure does not have a significant impact in improving sleep quality of HD patients. Adoption or modification may be needed in order to get more effective results in improving the sleep quality of HD patients. Keywords: sleep quality; factors; hemodialysis patients ABSTRAK Gangguan tidur merupakan masalah umum yang sering dialami pasien hemodialisis, serta dapat berdampak pada kualitas tidur. Hal ini perlu mendapat perhatian agar faktor penyebab dan metode penanganan yang efektif dapat dilakukan. Artikel ini ditulis dengan metode literature review dengan pencarian melalui di online database ScienceDirect, Springer, ClinicalKey, dan Pubmed. Hasil studi mengidentfikasi faktor-faktor yang terkait dengan maslah tidur pasien hemodialisis yaitu kadar vitamin D serum yang rendah, ngorok, nyeri, napping di siang hari, masalah pernafasan, pruritus, skor good dialysis index (GDI), gejala distres yang berat, kelebihan cairan sebelum dialisis, jenis kelamin, usia, lipatan kulit trisep, hipoproteinemia, hiperkalemia, hiperpospatemia, dan status nutrisi yang buruk, frekuensi HD, restless legs syndrome (RLS). Sementara metode yang cocok untuk meningkatkan kualitas tidur pasien HD diantaranya yaitu mendengarkan musik, edukasi sleep hygiene, aromaterapi melalui inhalasi, continuous care model (CCM), sedangkan akupresur tidak berdampak signifikan dalam peningkatan kualitas tidur pasien HD. Adopsi maupun modifikasi mungkin diperlukan agar mendapatkan hasil yang lebih efektif dalm meningkatkan kualitas tidur pasien HD. Kata kunci: kualitas tidur; faktor; pasien hemodialisis

Abstract

Perawat yang bekerja di ruang rawat klien kritikal mempunyai peran yang penting dalam menurunkan angka kesakitan dan kematian klien dengan trauma kepala melalui pengkajian dan intervensi dini. Menit-menit pertama merupakan waktu kritikal yang sangat menentukan apakah klien akan mengalami penurunan fungsi otak yang progresif atau terjadi perbaikan. Oleh karenanya, deteksi dini dan intervensi yang tepat merupakan kunci keberhasilan. Untuk mendapatkan hasil yang optimal, perawat yang bekerja di ruang rawat kritikal harus meningkatkan cara berfikir kritis dan keterampilannya secara teratur. Koordinasi kerja dengan tenaga kesehatan lain juga penting dalam menentukan keberhasilan pelayanan. Nurse has an important role in reducing morbidity and mortality of patients with head injuries through meticulous assessment. First minutes after cerebral trauma are a critical time that determine – patient experience severe deteriorated cerebral function or experience improvement of cerebral function. Therefore, early detection and accurate nursing intervention will be the critical activities in a achieving successful results. To get a goo result, critical care nurse need to improve their critical thinking and skills continuously. Good coordination among health care team may help the best possible outcome in managing patients with severe head injuries.

Abstract

Stroke can affect balance. Balance is the ability to maintain the body in a balanced position both in static and dynamic conditions. However, the assessment of the existing balance is only done in a static state in stroke patients. The balance assessment describes the balance of stroke patients so that they can be written on asessment form and the transfer record and can determine the need for balance training as one of the rehabilitation of stroke patients. This EBN application aims to find out the use of Berg Balance Scale to assess the balance of both static and dynamic conditions in stroke patients. The method used is conducting PICO analysis, searching database, critical appraisal, and determining relevant journals. EBN implementation was carried out on 10 people who had a stroke in zone A on the 5th floor of Building A RSUPN Dr. Cipto Mangunkusumo. This study uses the Berg Balance Scale scale in Indonesian. The analysis showed that stroke patients who experienced balance disorders were 7 people and did not experience a balance of 3 people BBS can assess the balance of stroke patients in accordance with existing evidence. BBS can be used as a balance assessment in stroke patients.

Abstract

Background: Stroke is a neurological change that occurs quickly caused by disruption of blood supply to parts of the brain. Stroke has a complex impact which includes bio-psycho-social and spiritual. Various complexities of problems encountered in stroke patients so that self-management is needed as a form of adaptation to new conditions after stroke. Self-management interventions are useful for improving self-management skills and behavior in stroke patients. Purpose this study is to analize effectiveness of self-management interventions in stroke patients. Methods : Narrative literature review, research analyzes published in the online databases of ProQuest, Google scholar and Scopus. The study was selected using the criteria and keywords for the 2015-2019 period. Seven studies were analyzed using the literature review process. Results: To improve self-management stroke patients self-management interventions are needed. Self-management interventions can be applied in hospitals when post acute patients are hospitalized, preparation for discharge, return and after discharge home and can be combined with rehabilitation programs in the community. Self-management interventions can improve self-efficacy, self-management behavior, activity daily living (ADL) ability, decreased re-hospitalisation and patient readiness to return to the community. Conclusion: Self-management interventions can be applied in Indonesia, with a note that among others prepared human resources, costs, supporting facilities and policies from the manager.

Abstract

Diabetes Melitus Tipe 2 (DM Tipe 2) adalah kelainan metabolik yang ditandai dengan hiperglikemia yang diakibatkan kurangnya sekresi insulin, resistensi insulin, atau keduanya. Kondisi hiperglikemia kronis dapat menyebabkan berbagai komplikasi yang salah satunya adalah kaki diabetik yang menjadi penyebab utama dilakukannya amputasi pada klien DM tipe 2. Penelitian ini bertujuan untuk mengetahui pengalaman klien DM tipe 2 pasca amputasi mayor ekstremitas bawah. Penelitian ini menggunakan metode kualitatif dengan pendekatan fenomenologi. Hasil analisis data menghasilkan enam tema, yaitu: perubahan dalam kehidupan setelah amputasi, respon atau perasaan terkait amputasi, mekanisme koping, dukungan sosial yang diterima, makna hidup, dan pelayanan kesehatan yang diterima. Hasil penelitian ini diharapkan dapat menjadi masukan dalam meningkatkan kualitas asuhan keperawatan melalui peningkatan dukungan rehabilitasi secara fisik, psikososial, dan spiritual pada klien DM tipe 2 pasca amputasi mayor ekstremitas bawah.

Abstract

Introduction: Patient with musculoskeletal disorder has been moving limitation and caused decreasing of gut peristaltic. The aimed of this study to find out the influence of drinking plain water 500 ml on the morning to constipation Incident of Immobilize Patient with musculoskeletal system disorder.Methods: The methode of this study were quasi-eksperiment with post test only non equivalent control group design. The number of sample was 24 respondents as intervention group, and 24 respondent as control group. The instrument that be used were Bowel Score that have been observed everyday for three days.Result: The result shows significant influences of drinking plain water on the morning to constipation incident of immobilize patient musculoskeletal system dysorder (p value=0.002; α=0.05.Conclusion: consuming water can avoid constipation on immobilize patient with musculoskeletal disorder. For the next research hopefully measures and analize psychosocial faktor of the patient defecation such as comfortable and privacy.
