Abstract

Hearing loss can interfere with communication, affecting social, school, and work environments. Hearing loss can be prevented by early detection and management of hearing loss. Tuban is one of the districts in East Java; the case data was obtained on the ear as a hearing organ, namely otitis externa (25.56%). acute otitis media (22.46%), cerumen obturation (16.15%), chronic suppurative otitis media (7.56%), and sensorineural deafness (5.72%). The early detection and management skills of hearing loss are mandatory for doctors working in primary health care, so a series of activities are needed to improve this knowledge and skills. Activities outside the network include lecture methods, question-and-answer discussions and training. The activity was attended by 52 participants with details of 48 doctors and four health workers. Most recent education is S1 or equivalent (96.1 most occupations are general practitioners (92.30%), and most workplace public health centers (69.23%). The pre-test score was 47, and the post-test value was 57.

Abstract

Humans are social beings who always communicate with other individuals. The ear as a hearing organ has a big role in the communication process. Disorders of the ear will cause disturbances in individual communication and socialization with other individuals in society. Efforts to maintain and improve hearing health are the responsibility of all parties, especially doctors in primary health services as the spearhead of efforts to detect early public health, so that the quality of these doctors must be maintained. Pacitan, one of the districts in the province of East Java, has a mountainous area, that accessibility is quite difficult and remote. The availability of human resources and health infrastructure in Pacitan, which is quite limited, requires the ability to carry out early detection and management of hearing loss in the community. Therefore, community service activities were carried out in the form of training on early detection and management of hearing impairments for doctors throughout Pacitan district. The activity was attended by 47 general practitioners who were delegates from health facilities. The pre test results obtained an average value of 45.23 and the post test was 68.55. There is an increase in knowledge before and after the activity.

Abstract

Allergic rhinitis and rhinosinusitis are global problems that are frequently encountered in daily clinical practice. This disease has an impact on the quality of life of sufferers who can resulting in decreased productivity, interpersonal relationships, and social activities. Socialization is needed regarding allergic rhinitis and rhinosinusitis so that medical personnel can participate in early diagnosis and management of allergic rhinitis and rhinosinusitis. Educational activities are carried out through outreach activities using lecture methods, discussions, training, as well as pretest and posttest. Based on the results of the pretest and posttest, the pretest average was 56,55 (± 27.51) and the posttest average was 89.27 (±12.95). Based on the comparison of pretest and posttest results, we found a statistically significant difference (p < 0.05). Medical personnel's knowledge of allergic rhinitis and rhinosinusitis needs to be increased so that they can participate in early diagnosis and management of allergic rhinitis and rhinosinusitis.

Abstract

Covid-19 is a new disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The increasing number of cases of Covid-19 has made it difficult for hospitals to handle it, and health workers are at a higher risk of infection by SARS-CoV-2 and urgently need Personal Protective Equipment (PPE) to protect themselves and minimize the spread of the virus. However, the usage of PPE especially face-masks often causes discomfort. Therefore, a new type of face mask has emerged, namely the medical snorkel mask. This mask is designed to cover the entire face and is made by modifying the air circulation section by adding an air filter. The filter used is PALL Ultipor 25 with a HEPA filter capable of filtering up to 0.3 microns in size and has an efficiency of 99.99%. Medical snorkel mask that is used properly are quite efficient in preventing the transmission of Covid-19, with a fitting factor of 142 from pass level 100. This mask also has passed the CO2 toxicity test. The advantages of a medical snorkel mask include protecting the entire face from infectious droplets, preventing direct hand contact with the face, being reusable, and being equipped with an effective air filter by minimizing the potential for filtrate malfunction. Some of the disadvantages of this mask include interfering with the communication process, cannot being used in conjunction with glasses, a buildup of CO2, increasing the work of breathing, and limiting vision. However, some of these weaknesses can be minimized by modifying the mask and maintaining an optimal barrier system. Given the high level of effectiveness and efficiency, medical snorkel masks can be a novel solution to overcome the discomfort of using PPE for health workers in Indonesia. Hopefully, it can reduce the morbidity and mortality of health workers due to exposure to Covid-19.

Abstract

Highlights: Nasal congestion was the most common symptom presented in CRS patients. Nasal symptoms of CRS were positively associated with nasoendoscopic findings (Polyp, Edema, Secretions).   Abstract Introduction: Chronic rhinosinusitis (CRS) is often encountered in Indonesia. Nasal endoscopy, or nasendoscopy, is considered the gold standard for assessing the severity of CRS. However, as nasendoscopy is unavailable in Indonesia, only symptom assessments can be conducted at primary healthcare facilities. This study aimed to elucidate the relationship between nasoendoscopic findings based on the Lund-Kennedy score (LKS) and symptoms in CRS patients based on the visual analogue scale (VAS) score. Methods: This retrospective analytic study evaluated CRS cases identified in the Ear, Nose, and Throat (ENT) Outpatient Clinic of Dr. Soetomo General Academic Hospital, Surabaya, from July 2021 to July 2022.  Statistical analysis was conducted using Spearman’s rank correlation test to identify the correlation (r) between LKS and the VAS score. Statistical significance was achieved if p<0.05. All statistical tests were conducted using the International Business Machines Corporation (IBM) Statistical Package for the Social Sciences (SPSS) version 26.0 for Windows. Results: A total of 57 CRS patients were included in this study, mostly females (64.9%) and those aged 50–59 years old (26.9%). Nasal congestion was the dominant nasal symptom (80.7%), with mostly moderate symptom severity (VAS 4-7) reported. Across both nasal cavities, the most frequent nasoendoscopic finding among CRS patients was nasal edema (82.4% and 93%). A positive and weak correlation existed between the total VAS score of all symptoms and the total LKS of both nasal cavities (r=0.387, p=0.003). Conclusion: The nasal symptoms of CRS patients were significantly associated with its nasoendoscopic findings (Polyp, Edema, Secretions).

Abstract

Highlights: Most rhinosinusitis patients with orbital complications were diagnosed as chronic rhinosinusitis. Orbital cellulitis (Chandler II) was the most common orbital complication degree. Rhinosinusitis patients with orbital complications commonly experience eye pain, swollen eyes, and persistent runny noses.   Abstract Introduction: Rhinosinusitis is an inflammation of the mucosa of the nasal cavity and/or paranasal sinuses. The most common complication is orbital. The limited data available does not account for the high number of cases of rhinosinusitis. This study examined the profile of rhinosinusitis patients with orbital complications. Methods: This was an observational descriptive study with a retrospective design. It used secondary data from medical records of patients with rhinosinusitis and orbital complications at the Ear, Nose, and Throat (ENT) Outpatient Unit, Dr. Soetomo General Academic Hospital, Surabaya, from January 2015 to April 2022. Results: There were 40 samples that met the inclusion and exclusion criteria (n=40). As many as 80% of cases were chronic rhinosinusitis, and 20% were acute. The average age of chronic rhinosinusitis was in 6th decade, while acute rhinosinusitis (ARS) was in 3rd decade. The majority of patients were males and reside outside Surabaya. Signs and symptoms that ARS patients often experienced were eye pain and swelling, nasal congestion, persistent runny nose, and decreased vision. Meanwhile, chronic rhinosinusitis included eye swelling and pain, persistent runny nose, and facial pain. The most common Chandler’s degree was orbital cellulitis (Chandler II). The most common treatment for ARS was a combination of medicamentosa and non-medicamentosa or medicamentosa and surgery, while chronic rhinosinusitis was a combination of medicamentosa, non-medicamentosa, and surgery. Conclusion: There were outcome differences between patients with acute and chronic rhinosinusitis regarding age, manifestations, and treatments, but there were similarities in sex, domicile, and Chandler’s degree predominance.

Abstract

Gangguan pendengaran merupakan masalah kesehatan global yang berdampak luas pada kualitas hidup individu, mulai dari hambatan perkembangan bicara pada anak, penurunan prestasi akademik pada remaja, hingga isolasi sosial dan peningkatan risiko jatuh pada lansia. Deteksi dini melalui layanan primer sangat penting, namun pengetahuan dan keterampilan tenaga kesehatan di lini pertama masih terbatas. Penelitian ini bertujuan untuk mengevaluasi efektivitas edukasi dan pelatihan skrining gangguan pendengaran bagi tenaga kesehatan di Puskesmas Kawedanan, Kabupaten Magetan. Metode yang digunakan adalah pelatihan dengan pendekatan kombinasi seminar daring dan workshop luring. Sebanyak 61 tenaga kesehatan dari berbagai profesi mengikuti kegiatan ini. Pengetahuan peserta dievaluasi menggunakan pretest dan posttest, dengan analisis statistik untuk mengukur peningkatan pemahaman. Hasil penelitian menunjukkan rata-rata skor pretest sebesar 68,03 dan meningkat menjadi 97,05 pada posttest. Uji statistik menunjukkan adanya peningkatan signifikan (p < 0,01), yang menandakan kegiatan edukasi dan pelatihan efektif dalam meningkatkan pemahaman peserta mengenai skrining gangguan pendengaran. Edukasi dan pelatihan berbasis kombinasi metode dapat meningkatkan pengetahuan tenaga kesehatan tentang deteksi dini gangguan pendengaran. Kegiatan ini perlu dilaksanakan secara berkelanjutan agar tenaga kesehatan di layanan primer mampu berperan aktif dalam deteksi dini dan penatalaksanaan awal gangguan pendengaran, sehingga kualitas pelayanan kesehatan masyarakat dapat ditingkatkan.
