Abstract

Aim to assess the differences between Atherogenic Index of Plasma (AIP), ratio of oxidized-Low Density Lipoprotein (Ox-LDL)/High Density Lipoprotein (HDL) and ratio of Lipoprotein-associated Phospholipase A2 (Lp-PLA2)/HDL in predicting the risk of coronary heart disease (CHD) in patients with controlled and uncontrolled type 2 Diabetes Mellitus (T2DM).Methods The study was done observationally with cross sectional design. A total of 80 patients, consisted of 40 controlled and 40 uncontrolled T2DM. The serum triglyceride (TG), HDL-C, Ox-LDL, Lp-PLA2 were examined in their relationship with T2DM risk. AIP is a ratio calculated as log (TG/HDL-C).Results AIP and ratio of Ox-LDL/HDL were significantly higher in uncontrolled than controlled T2DM (0.72 + 0.13 vs 0.47 ± 0.22 , p < 0.001) and (1738.8 ± 625.5 vs 1418 ± 535.3, p = 0.02), but no significant difference was found in ratio of Lp-PLA2/HDL (5.09 ± 2.17 vs 5.95 ± 3.11, p = 0.16).Conclusion AIP and ratio of Ox-LDL/HDL value were significantly higher in uncontrolled than in controlled T2DM. Abstract

Aim Circulating endothelial progenitor cells (EPCs) are reduced in number and function in patients at risk for cardiovascular diseases. On the other hand, hypertension is related with excess angiotensin II which would lead to oxidative stress. In this study, we investigated the correlation between F2-Isoprostane (as marker of oxidative stress) with Stromal Cell-Derived Factor-1 (SDF-1) and CD34 viable in non hypertension and hypertension subjects.Methods This was a cross sectional study conducted on 54 non hypertension and 64 hypertension subjects visiting Prodia laboratory, Jakarta. F2-Isoprostane (as marker of oxidative stress) and SDF-1 (a strmal cell growth factor) were measured by ELISA method, and CD34 viable (marker of progenitor cell) was measured by flow cytometry.Results F2-Isoprostane concentration was higher in hypertensive subjects compared to non hypertensive subjects, although statistically non significant (mean + SD: 0.13 ± 0.120 vs 0.10 ± 0.16; ρg/mL; p = 0.091). SDF-1 concentration was significantly higher in hypertensive subjects compare to non hypertensive subjects (2821.63 ± 281.94 vs 2623.04 ± 356.28 ρg/mL; P < 0.05). CD34 viable level was significantly lower in hypertensive subjects compare to non hypertensive subjects (1.9 ± 0.9 /μL vs 2.7 ± 1.7; P < 0.05). F2-Isoprostane had negative correlation with CD34 viable in circulation (r = 0.022, p < 0.05) but no correlation with SDF-1 (p > 0.05).Conclusions F2-Isoprostane was higher, but CD34 was lower, in hypertensive subjects compared to non hypertensive. It seems that high F2-Isoprostane impaired the CD34 viable level as shown by negative correlation between F2- Isoprostane and CD34. Patients with hypertension have been shown to express reduced number and functional capacity of endothelial progenitor cells (EPCs). Recent study reveals that EPCs contain an important capability to maintain endothelial integrity and vascular homeostasis. Therefore, enhancement of EPCs could be benefit for individuals with cardiovascular diseases. We investigated the correlation between low grade inflammation marker (hsCRP) with EPC surface marker (CD34 total) in hypertension. This was an observational study with cross sectional design conducted in 51 nonhypertenive and 60 hypertensive subjects. hsCRP (as marker of inflammation) was measured by immunochemiluminometric method. CD34 total (as marker of EPC) was measured by flow cytometry method. hsCRP concentration was higher in hypertensive subjects compare to nonhypertensive subjects (mean±SD, 2.709±2.50 vs 2.476±2.438 mg/L; P>0.05). CD34 total level was lower in hypertensive subjects compare to nonhypertensive subjects (mean±SD, 1.957±0.858 vs 3.244±3.463/μL; P<0.05). hsCRP had positive correlation with CD34 total in hypertensive subjects (P<0.05). Penelitian ini menginvestigasi korelasi antara marker inflamasi tingkat rendah (hsCRP) dengan marker EPC surface (total CD34) pada hipertensi. Penelitian ini merupakan studi observasional dengan desain cross sectional pada 51 orang nonhipertensi dan 60 orang pasien hipertensi. hsCRP (marker inflamasi) diukur menggunakan metode immunochemiluminometric. Nilai total CD34 (marker EPC) diukur dengan menggunakan metode flow cytometry. Konsentrasi hsCRP lebih tinggi pada pasien hipertensi dibandingkan pada pasien nonhipertensi (mean±SD, 2,709±2,0 vs 2,476±2,438 mg/L; P>0,05). Nilai total CD34 lebih rendah pada pasien hipertensi dibandingkan dengan pasien nonhipertensi (mean±SD, 1,957±0,858 vs 3,244±3,463/μL; P<0,05). hsCRP memiliki korelasi positif dengan total CD34 pada pasien hipertensi (P<0,05). Peningkatan konsentrasi hsCRP akan menstimulasi level total CD34 pada pasien hipertensi seperti yang ditunjukkan dengan korelasi positif antara hsCRP dengan total CD34.Kata kunci: hsCRP, total CD34, endothelial progenitor cell, hipertensi

Abstract

Cerebrovascular disease are the third most common cause of death in Western countries. The most frequent manifestation of disease is a sudden episode of neurological deficit termed stroke which is the result of cerebral haemorrhage or cerebral infaction in the mayority of cases. Stroke secondary to atherosclerosis is most common in people over 50 years old.The incidence of stroke rises dramatically with ages, with the risk doubling with each decade after 35 years old. About 5 % of people over 65 years old have at least one stroke. Atherosclerosis is condition where fatty acid deposits occur in the inner lining of arteries and the forming of atherosclerotic plaque,a mass consisting of fatty deposit, and blood platelets.The plaque may obstruct or my trigger clot,a trombus,at that location causing cerebral trombosis.It is called ischemmic stroke. The basis of the response to injury hypothesis introduced by Russel Ross is that the earliest cellular events that occur during atherosclerosis is a specialized type of chronic inflammatory response to cell injury. What may begin as a protective inflammatory reponse can become excessive and deleterious to the cell of the artery wall. The adhesion of leucocytes on endothelial cells and their trans –endothelial migration into intima are mediated by adhesion molecules on the endothelial cell membrane that mainly belong to two protein families:the selectin and the addhesion molecules.
