Abstract

Gangguan tidur di ICU disebabkan oleh banyak faktor, diantaranya lingkungan, kebisingan, pencahayaan, kegiatan perawat, penyakit yang diderita, tindakan keperawatan, terapi obat, dan ventilasi mekanik. Efek yang ditimbulkan akan memengaruhi fungsi kekebalan tubuh, sistem metabolisme, regulasi sistem saraf pusat, dan kondisi psikologis. Tujuan penelitian ini menerapkan dan membuktikan efektifitas penggunaan Earplug dan Eye Mask dalam meningkatkan kualitas tidur pada pasien di ICU. Desain yang digunakan randomized controlled trial (RCT) crossover design. Peneliti membagi Group A dan Group B dengan simple random sampling. Jumlah sampel 24 responden. Instrumen kualitas tidur menggunakan Richard Campbell Sleep Questionnaire (RCSQ). Data dianalisis dengan uji Independent Sample T-Test. Hasil penelitian didapatkan p-value < 0,05, berarti pada alpha 5% terdapat perbedaan yang signifikan kualitas tidur antara malam pertama dan kedua pada masing-masing group sehingga disarankan dijadikan evidence based di rumah sakit sebagai salah satu terapi komplementer yang dapat dijadikan intervensi mandiri keperawatan untuk membantu mengatasi gangguan tidur. Kata Kunci: earplug, eye mask, ICU, kualitas tidur Abstract Evidence-Based Nursing Application Effect of Earplugs and Eye Mask on Quality Sleep in Patients in ICU. Sleep disorders in ICU are caused by many factors, including environment, noise, lighting, nursing activities, illness, nursing, medication therapy, and mechanical ventilation. The effects will affect the immune function, metabolic system, central nervous system regulation, and psychological conditions. The purpose of this study to apply and prove the effectiveness of the use of Earplug and Eye Mask in improving sleep quality in patients in ICU. The design used randomized controlled trial (RCT) crossover design. The researchers divide Group A and Group B by simple random sampling. The Total sample of 24 respondents. Sleep quality instrument using Richard Campbell Sleep Questionnaire (RCSQ). Data were analyzed by the Independent Sample T-Test. The result of this research is p-value <0,05, mean at alpha 5% there is a significant difference of sleep quality between first and second night in each group so it is suggested to be an evidence-based in a hospital as one of complementary therapy which can be made independent intervention nursing to help overcome sleep disorders. Keywords: earplug, eye mask, ICU, sleep quality

Abstract

The new coronavirus disease 2019 (Covid-19) has been declared a pandemic by WHO. The transmission of Covid-19 is robust and the infection rate is fast. This systematic review aims to analyze and review the currently published literature related to factors associated with functional status and mental health in post-Covid-19 patients. The method used is by searched the relevant databases and assessed research articles that met the inclusion and exclusion criteria. After an initial screening of articles identified by the title and abstract, 167 were selected. Furthermore, 52 full-text articles were assessed and reviewed against our inclusion criteria and thus 16 articles were deemed eligible and analyzed in this analysis. All studies included an adult population between 18 and 95 years. The median length of hospitalization for Covid-19 patients ranged from 12 days to 21 days.  The most common long-term symptoms in Covid-19 patients include: shortness of breath, fatigue, decreased physical function, insomnia, impaired cognitive function, depression, and anxiety. Factors that influence it besides demographics also include the length of hospitalization, The severity of illness, and the presence of comorbidities. Therefore, having the right support and treatment with the proper management strategy can help restore and improve their health after being discharged from the hospital

Abstract

Self Monitoring Blood Glucose (SMBG) has been accepted as an important part of diabetes mellitus management. The purpose of this study was to identify factors that affect type 2 diabetes mellitus patients to perform SMBG. This is a cross sectional study, with 80 respondents. Statistical analysis was using a chi square correlation test and a multiple logistic regression. The result shows that there a significant correlation between belief on diabetes mellitus management and SMBG (p = 0,000). The most influential factor on SMBG is the belief variable on diabetes mellitus management with OR = 14,069 after controlled with the knowledge variable (95% CI OR 0,100 - 1983, 518). It is needed to improve the belief on diabetes mellitus managements especially in type 2 diabetes mellitus patient

Abstract

ABSTRACT Cardiovascular system nursing care and Acutely Decompensated Heart Failure (ADHF) cases increase every year accompanied by an increase in life expectancy. Cardiovascular system disorders nursing care with energy conservation theory approach developed by Myra Levine. Patients with cardiovascular disorders on average experience shortness of breath during activities and chest pain. A balance of energy input and output is required to avoid excessive fatigue. The main focus of care is acute cardiac care. The purpose of this case study is to provide nursing care for ADHF patients with Myra Levine's energy conservation theory approach. This research design uses a descriptive case study approach. The sample in this study was a patient diagnosed with Acutely Decompensated Heart Failure (ADHF). The results of the application of nursing care in ADHF patients with Myra Levine's energy conservation theory approach obtained conservation priority Tropichognosis: Activity intolerance; and priority structural integrity: decreased cardiac output. The results of the priority hypothesis of nursing care in ADHF patients with Myra Levine energy conservation theory approach obtained an effective heat pump. The priority intervention applied in nursing care for ADHF patients with Myra Levine’s energy conservation approach is acute cardiac care. The final result of nursing care for ADHF patients with the energy conservation theory approach of Myra Levine, it was found that patients were able to conserve the problem of decreasing cardiac output. Keywords: Acutely Decompensated Heart Failure (ADHF); Myra Levine; energy conservation theory ABSTRAK Asuhan keperawatan sistem kardiovaskuler serta kasus ADHF meningkat setiap tahunnya diiringi dengan peningkatan usia harapan hidup. Asuhan keperawatan gangguan system kardiovaskuler dengan pendekatan teori konservasi energi yang dikembangkan Myra Levine. Pasien yang mengalami gangguan kardiovaskuler rata-rata mengalami keluhan sesak saat beraktivitas dan nyeri dada. Keseimbangan input dan output energi diperlukan untuk menghindari kelelahan berlebihan. Fokus perawatan utama adalah perawatan jantung akut. Tujuan studi kasus ini adalah melakukan asuhan keperawatan pada pasien ADHF dengan pendekatan teori konservasi energi Myra Levine. Desain penelitian ini menggunakan desain deskriptif pendekatan studi kasus. Sampel dalam penelitian ini adalah 1 pasien terdiagnosis Acutely Decompensated Heart Failure (ADHF). Hasil penerapan asuhan keperawatan pada pasien ADHF dengan pendekatan teori konservasi energy Myra Levine didapatkan Tropichognosis prioritas konservasi: Intoleransi aktivitas; dan prioritas integritas structural: penurunan curah jantung. Hasil prioritas hipotesis asuhan keperawatan pada pasien ADHF dengan pendekatan teori konservasi energy myar Levine didapatkan pompa jantung efektif. Intervensi prioritas yang diterapkan dalam asuhan keperawatan pada pasien ADHF dengan pendekatan konservasi energy myra Levine adalah perawatan jantung akut. Hasil akhir asuhan keperawatan pada pasien ADHF dengan pendekatan teori konservasi energy myra Levine didapatkan pasien mampu melakukan konservasi terhadap masalah penurunan curah jantung. Abstract

Aspiration pneumonia in patients with stroke can occur as a result of the gram-positive and gram-negative in the oral cavity that spreads to the respiratory tract of patients due to decreased ability to fight bacteria mouth. Selection of materials for oral care need to consider in terms of the material effectiveness, safety, convenience and cost efficiency. This quantitative study using a quasi-experimental design with pre and post test control group design. This research aims to study the effectiveness of oral care using honey to the risk of aspiration pneumonia in stroke patients who suffered loss of consciousness and dysphagia. In this design 46 respondents 23 respondents were divided into intervention group with oral treatments using honey and 23 respondents into a control group who received oral treatment with 0.2% chlorhexidine. Performed oral care performed by a nurse twice a day in the morning and evening for 30 minutes within 3 days sequentially. Results: There are significant relationships in oral care using honey to the risk of aspiration pneumonia in stroke patients who suffered loss of consciousness and dysphagia (p.0,000: α.0,05). The risk of aspiration pneumonia was lower by 2,522 with oral treatments using honey instead of using Clorhexidine 0.2%. Conclusion: Oral care using honey to effectively prevent the increased risk of aspiration pneumonia in pasein stroke decreased consciousness and dysphagia. Suggestion: Oral care with honey can be used as material for oral care, especially in the choice of antiseptic agent as proven in this study honey can maintain and even reduce the risk of aspiration pneumonia in stroke patients with loss of consciousness and dysphagia. Abstract

This study aims to identify the effectiveness of music therapy in reducing anxiety and pain levels in patients with Coronary Artery Bypass Graft (CABG) surgery. The method used in this research is a literature review by searching online databases: Ebscohost, Wiley Online, Proquest, Pubmed, & ScienceDirect. The results showed that music therapy was effective in reducing anxiety and pain. The type of music provided is mostly natural music, the duration is 20-30 minutes and the level is 25-60 dB. In conclusion, music therapy effectively reduces anxiety and pain in patients with CABG surgery. Keywords: Anxiety, Pain, CABG Surgery, Music Therapy
