Abstract

This study aims to analyse the consequences of a lack of documentation in a community-based rehabilitation (CBR) unit for children with disabilities. 278 hours of observation and two structured interviews were conducted. Observation notes were depicted in a fishbone chart and interview findings were coded manually in a series of open, axial, and selective coding. The findings, field notes, and self-reflective notes were triangulated and link thematically. The findings revealed that failures to address children’s needs and progress, inability to modify the intervention based on evidence and lack of confidence in the therapists’ professionalism are the common problems experienced by the therapists due to lack of record-keeping. While the needs of CBR units are greatly increased for people with disabilities, CBR organisations and the Indonesia physiotherapy association need to cooperate in order to develop a clear clinical pathway and standardised documentation for a CBR context.

Abstract

This study aims to measure the implementation of the hand hygiene program at the Muhammadiyah Hospital Cirebon after accreditation. The method used is a qualitative analysis method, with a case study design. The research case is the Infection Control Prevention Working Group in which there is a hand hygiene program at the time of accreditation, including those that do not meet the minimum passing score, so the hand hygiene program implementation will be measured from an effective strategy, organizational characteristics, influence of the behavior of officers and patients and their families, and budget support from post accreditation management. The research instrument used interview guides with 4 respondents. The results of this study indicate that the Muhammadiyah Hospital in Cirebon does not yet have an effective strategy in building a hand hygiene culture. The post-accreditation Infection Control Prevention Team Organization is not running, so a hand hygiene program has not been established at the Muhammadiyah Hospital in Cirebon. The behavior of officers and patients the level of compliance with handwashing is still lacking, due to the use of consumables after accreditation is still low. In terms of the budget, the hospital has a good financial condition, but because there is no work program, the proposed expenditure budget of each unit does not reflect the need for programs that include training, procurement of consumables, and education for both officers and patients. ineffective hand hygiene strategies, PPI Committee organizational structure is not going well, there has been no change in hand hygiene behavior of employees, patients and visitors, and the PPI Committee budget is still contained in the unit budget, has not become a special budget item.Keyword: Hospital accreditation; cost; hand hygiene

Abstract

Community-Based Rehabilitation (CBR) units are growing in developing countries, including Indonesia. However, patient safety measures in CBR units have not yet been conducted, especially regarding flexible, unscheduled appointment system. This study aims to evaluate the unscheduled appointment system as a cause of errors in the care delivery of a CBR unit for children with disabilities in Makassar city. This case study conducted 278-hours observations and three structured interviews. The observation findings, encoded interview findings, field notes, and self-reflective notes were triangulated and link thematically. Four key findings were illustrated in an explorative model. The CBR unit implements a day-based scheduling system without time slots. Working in this system, the therapists experienced random visits, long waiting times, and excessive workload as a result of this flexible scheduling system. Significant risks of human errors and patient safety were revealed due to these consequences. As flexible scheduling systems produce more negative effects to the therapists and children, it is suggested that a CBR unit or a health care unit implement a scheduled appointment system with timeslots to achieve prudent healthcare.

Abstract

Background: Treatment variations have been considered as a sign of an inappropriate healthcare service whether due to the underuse or the overuse of resources. To reduce these variations, establishing and implementing well-developed standardized operating procedures (SOPs) and evidence-based standardized clinical guidelines is required. A rehabilitation unit for children with disabilities in Makassar did not implement any SOPs or standardized clinical guidelines in their service and treatment. This conduct may have an adverse effect on physiotherapists’ professionalism.Aims: The study aimed to explore the possible impact of community-based rehabilitation (CBR) practice with no SOPs and evidence-based standardized clinical guidelines on the physiotherapists. Method: This study was a qualitative case study. The CBR unit was observed for 278-hours and the study conducted three semi-structured interviews. Three physiotherapists voluntarily participated in the study. Open, axial, and selective coding were conducted to encode the interview findings. The findings from observation encoded interview, field, and self-reflective notes were triangulated, analyzed thematically, and illustrated by the explorative model.Results: Working with no SOPs and guidelines was associated with variations in procedures by physiotherapists, irregularity in their treatment choices, liability feeling toward errors making, and Low self-confidence in their professionalism.Conclusion: Working without SOPs and guidelines was associated with psychological exertion of the physiotherapists. They showed uncertainty in their professional capabilities as a physiotherapist. It is recommended for YPAC Makassar city to develop SOPs and evidence-based standardized clinical guidelines for their organization.Keywords: children, community-based rehabilitation, disability, guidelines, standard-operating procedure

Abstract

Spondylolisthesis merupakan suatu pergeseran ke depan satu korpus vertebra (vertebral body) jika dibandingkan dengan vertebra yang terletak dibawahnya (inferior). Hal ini menyebabkan dampak mekanis, radikuler, atau nyeri. Tujuan: Studi kasus ini bertujuan untuk mengevaluasi pengaruh intervensi fisioterapi core stability exercise pada pasien wanita berusia 63 tahun dengan diagnosis spondylolisthesis dalam mengurangi nyeri dan meningkatkan aktivitas fungsional pada sendi lumbal. Metode: Core stability exercise yang diberikan pada penelitian ini berupa posterior pelvic tilt exercise, bridging exercise, bird-dog exercise, dan wall squat exercise dilakukan 9 kali treatment selama 3 minggu. Nyeri dievaluasi dengan menggunakan visual analog scale (VAS) dan aktivitas fungsional dengan oswestry disability index (ODI). Hasil Temuan: Setelah dilakukan 4 kali intervensi, terjadi penurunan nyeri pada gerak fleksi neck (2 poin), gerak fleksi trunk (1 poin), gerak fleksi hip bilateral(2 poin), nyeri tekan m. piriformis bilateral (1 poin), nyeri tekan m. hamstring bilateral (2 poin), nyeri tekan m. gastrocnemius bilateral (2 poin), dan nyeri tekan celah processus transversus vertebra L5(1 poin). Penurunan skor ODI menunjukkan penurunan disabilitas fungsional pasien sebanyak 9 poin. Kesimpulan: Berdasarkan hasil tersebut, dapat disimpulkan bahwa intervensi core stability exercise cukup efektif untuk mengurangi nyeri dan meningkatkan aktivitas fungsional pada pasien spondylolisthesis pada vertebra lumbal.

Abstract

Physical activity has been evaluated to decrease mortality rates in people with diabetes mellitus. After the COVID-19 pandemic, healthcare delivery was leaned towards telemedicine or telehealth to conduct social distancing. The primary aim of this review is to evaluate formats and strategies used in telemedicine to improve physical activity in adults with diabetes mellitus. ProQuest, Medline, and CINAHL were searched for studies involving adults with diabetes mellitus (more than 18 years old); telemedicine; and physical activity outcomes. The quality of the studies was appraised using the JBI Critical Appraisal Checklists for risk of bias, study design, and quality of evidence. Data syntheses were conducted following a simplified approach by Popay et al. Themes were developed based on the findings. Five randomized controlled trials met the inclusion criteria. The risk of bias was determined low in most of the studies. Text messages were the most common method. Pedometer, accelerometer, and International Physical Activity Questionnaires were commonly used to quantify the level of physical activity. Frequent reminders using text messages to the participants were the common strategy to ensure their adherence. The telehealth intervention has not yet shown significant positive effects on physical activity in people with diabetes mellitus. Further studies conducting synchronous and hybrid telehealth to improve physical activity in adults with diabetes mellitus were needed. Abstrak: Aktivitas fisik telah dievaluasi untuk menurunkan angka kematian pada penderita diabetes mellitus. Setelah pandemi COVID-19, pemberian layanan kesehatan condong ke telemedicine atau telehealth untuk melakukan social distancing. Tujuan utama dari tinjauan ini adalah untuk mengevaluasi format dan strategi yang digunakan dalam telemedicine untuk meningkatkan aktivitas fisik pada orang dewasa dengan diabetes mellitus. ProQuest, Medline, dan CINAHL dicari untuk penelitian yang melibatkan orang dewasa dengan diabetes mellitus (lebih dari 18 tahun); telemedis; dan hasil aktivitas fisik. Kualitas studi dinilai menggunakan JBI Critical Appraisal Checklists untukrisiko bias, desain studi, dan kualitas bukti. Sintesis data dilakukan mengikuti pendekatan yang disederhanakan oleh Popay et al. Tema dikembangkan berdasarkan temuan. Lima uji coba terkontrol secara acak memenuhi criteria inklusi. Risiko bias ditentukan rendah di sebagian besar penelitian. Pesan teks adalah metode yang paling umum. Pedometer, akselerometer, dan Kuesioner Aktivitas Fisik Internasional biasanya digunakan untuk mengukur tingkat aktivitas fisik. Pengingat yang sering menggunakan pesan teks kepada para peserta adalah strategi umum untuk memastikan kepatuhan mereka. Intervensi telehealth belum menunjukkan efek positif yang signifikan terhadap aktivitas fisik pada penderita diabetes mellitus. Diperlukan penelitian lebih lanjut yang melakukan telehealth sinkron dan hibrida untuk meningkatkan aktivitas fisik pada orang dewasa dengan diabetes mellitus.

Abstract

Neck pain attacks 16.6% of the general adult population in Indonesia. This condition causes disruption of function and motion in the neck, so it requires physiotherapy intervention. Sustained Natural Apophyseal Glides (SNAGs) is a mobilisation with movement techniques that can decrease pain during exercise without causing any secondary pain. Materials and Methods. From the search results with various databases found 385 journals that match these keywords, screened titles and abstracts were found 30 articles were then filtered and 20 articles were selected. However, from the 20 articles, there will be further screening. The final result after selection is 5 articles. Results. Of the five articles reviewed on Sustained Natural Apophyseal Glides (SNAGs) using a dose of 3 sets of 6 repetitions per training session with the method of Gliding can reduce pain in the cases of neck pain. Conclusion. The Mulligan Concept Exercise with the Sustained Natural Apophyseal Glides (SNAGs) technique using a dose of 3 sets of 6 repetitions per training session with the method Gliding according to the direction of movement experiencing pain can produce a positive effect on reducing pain in cases of neck pain so that it can restore motion function on the neck.
