Abstract

This study aims to analyse the consequences of a lack of documentation in a community-based rehabilitation (CBR) unit for children with disabilities. 278 hours of observation and two structured interviews were conducted. Observation notes were depicted in a fishbone chart and interview findings were coded manually in a series of open, axial, and selective coding. The findings, field notes, and self-reflective notes were triangulated and link thematically. The findings revealed that failures to address children’s needs and progress, inability to modify the intervention based on evidence and lack of confidence in the therapists’ professionalism are the common problems experienced by the therapists due to lack of record-keeping. While the needs of CBR units are greatly increased for people with disabilities, CBR organisations and the Indonesia physiotherapy association need to cooperate in order to develop a clear clinical pathway and standardised documentation for a CBR context.

Abstract

This study aims to measure the implementation of the hand hygiene program at the Muhammadiyah Hospital Cirebon after accreditation. The method used is a qualitative analysis method, with a case study design. The research case is the Infection Control Prevention Working Group in which there is a hand hygiene program at the time of accreditation, including those that do not meet the minimum passing score, so the hand hygiene program implementation will be measured from an effective strategy, organizational characteristics, influence of the behavior of officers and patients and their families, and budget support from post accreditation management. The research instrument used interview guides with 4 respondents. The results of this study indicate that the Muhammadiyah Hospital in Cirebon does not yet have an effective strategy in building a hand hygiene culture. The post-accreditation Infection Control Prevention Team Organization is not running, so a hand hygiene program has not been established at the Muhammadiyah Hospital in Cirebon. The behavior of officers and patients the level of compliance with handwashing is still lacking, due to the use of consumables after accreditation is still low. In terms of the budget, the hospital has a good financial condition, but because there is no work program, the proposed expenditure budget of each unit does not reflect the need for programs that include training, procurement of consumables, and education for both officers and patients. ineffective hand hygiene strategies, PPI Committee organizational structure is not going well, there has been no change in hand hygiene behavior of employees, patients and visitors, and the PPI Committee budget is still contained in the unit budget, has not become a special budget item.Keyword: Hospital accreditation; cost; hand hygiene

Abstract

This study evaluates the Non-Communicable Disease Posbindu services in the Jagasatru Public Health Center in Cirebon City during the Covid-19 pandemic era seen from the aspects of communication, resources, disposition, and bureaucratic structure. The method used is a qualitative analysis method, with a case study design. The research subjects were eight cadres of Posbindu Non-Communicable Diseases using random sampling technique; data collection was done by in-depth interview technique. This study's results indicate that the evaluation of services from the aspect of communication to the target of the analysis is correct, but has not yet reached the mark of productive age. Evaluation from the aspect of resources can be seen that in the pandemic era, every Non-Communicable Disease Posbindu requires personal protective equipment and health equipment to support services, and there is no cadre regeneration in every Posbindu. Evaluation from the aspect of disposition, the acceptance of cadres as executors is quite good because they perform well in serving participants in limitations. The evaluation of the reporting system's bureaucratic structure has been graded from cadres to health center officers to the Cirebon City Health Office.Keyword: Evaluation, Public Health Center, Posbindu PTM

Abstract

This study aims to determine the quality of the RSIA Nuraida webiste and the factors causing the dimension that has the lowest score. This study uses a mixed method with an explanatory sequential design. The quantitative phase is carried out by testing the quality of the RSIA Nuraida website using a nibbler, based on the dimensions of accessibility, experience, marketing and technology. The qualitative phase was carried out by in-depth interviews with managers and staff related to the factors causing the lowest score of the four dimensions. The results of this study indicate the accessibility dimension has a score of 8.9, the experience dimension has a score of 6.6, the marketing dimension has a score of 3.3 and the technology dimension has a score of 8.4. The factor that causes the marketing dimension to have the lowest score is the lack of involvement of the marketing team in managing the website at RSIA Nuraida.Keyword: Digital Marketing, Website, Nibbler

Abstract

Performance-based capitation payments complement capitation payments based on the fulfillment of service commitments, which in principle will make adjustments to the capitation received if it does not meet the predetermined indicators. The Primary Clinic Akbid Muhammadiyah Cirebon requires a performance-based analysis of the readiness to face capitation payments. The purpose of this study was to simulate income before and after performance-based capitation adjustments and to see the readiness of the Primary Clinic Akbid Muhammadiyah Cirebon in facing performance-based capitation payments in terms of information, authority, staff, and infrastructure. This research uses an explanatory sequential mixed-method approach. In the first phase, data from primary care in 2020 is simulated so that income is known after adjustment by looking at three indicators. The qualitative phase is carried out by in-depth interviews with clinic leaders, medical support and service coordinators, management coordinators, and medical record staff to analyze the clinic's readiness to face performance-based capitation payments. The results showed that the clinic had not been able to meet all performance-based capitation payment indicators so that the income decreased by Rp. 23,178,200. Preparation from the information side by optimizing the JKN Mobile application as a telemedicine service, in terms of authority with regulations governing non-specialized referrals, from infrastructure side with the repair of computers and internet networks, from the staff side with training in both educational materials and innovative delivery media. The study concluded that the clinic requires system improvements in terms of information, authority, infrastructure, and staff to meet indicators of contact rates, non-specialist outpatient referral ratios, and prolanis participant ratio. Keywords: Contac number, Referral, Prolanis

Abstract

Latar Belakang: Tingkat kepatuhan dokter dalam menuliskan resep berdasarkan formularium merupakan salah satu indikator mutu pelayanan kefarmasian di rumah sakit. Jika resep tidak berdasarkan formularium maka akan mempengaruhi mutu layanan kefarmasian di rumah sakit.Tujuan: Mengetahui tingkat kepatuhan dokter dalam menuliskan resep berdasarkan formularium dan mengetahui periode tingkat kepatuhan dokter dalam menuliskan resep berdasarkan formularium pada depo rawat inap, depo rawat jalan, dan depo IGD periode Januari-Desember 2019.Metode: Penelitian observasional dengan pendekatan deksriptif kuantitatif, dengan menghitung persentase tingkat kepatuhan dokter menuliskan resep berdasarkan formularium dari resep yang telah ditulis dokter di depo rawat jalan, depo rawat inap, dan depo IGD.Hasil: Kepatuhan dokter dalam penulisan resep berdasarkan formularium di depo rawat jalan sebesar 91,73%, tertinggi di bulan Maret dan terendah di bulan Desember. Pada depo rawat inap rata-rata kepatuhannya 94,34% tertnggi di bulan Januari dan terendah di bulan Desember. Pada depo IGD rata-rata kepatuhannya 94,36% tertinggi di bulan Maret dan terendah di bulan Juni. Kesimpulan: Kepatuhan penulisan resep berdasarkan formularium di depo rawat jalan, rawat inap dan depo IGD belum ada yang memenuhi standar pelayanan minimal yang berlaku. Periode tingkat kepatuhan tertinggi pada Januari dan terendah pada Desember.

Abstract

Latar Belakang: USAID mengeluarkan Infection Control Assessment Tool for Primary Health Care Facilities (ICAT) modul 7 yang menilai pengendalian infeksi terkait penanganan persalinan dan nifas, tetapi belum banyak dilakukan pada layanan primer yang melayani pertolongan persalinan di Indonesia.   Tujuan : Membandingkan capaian penilaian  risiko infeksi pelayanan kebidanan di Klinik Pratama kota dan kabupaten di Cirebon menggunakan instrument Infection Control Assessment Tool for Primary Health Care Facilities (ICAT) modul 7.Metode: Penelitian observasional dengan pendekatan deksriptif. Perbandingan capaian penilaian  risiko infeksi pelayanan kebidanan dilakukan di dua klinik pratama kota dan kabupaten Cirebon yaitu Klinik Akbid Muhammadiyah dan Klinik Dunia Medika menggunakan instrument ICAT modul 7 yang terdiri dari 8 indikator penilaian.Hasil: Capaian dari bagian satu sampai delapan dari Klinik Akbid Muhammadiyah memiliki rerata 66, 33%, dan capaian Klinik Dunia Medika 67,70%, masuk dalam ketegori baik.Kesimpulan: Angka kematian ibu dapat ditekan apabila layanan primer dalam hal ini klinik yang melayani pertolongan persalinan memiliki capai 100% pada setiap bagian di modul tujuh ICAT.

Abstract

Physical activity has been evaluated to decrease mortality rates in people with diabetes mellitus. After the COVID-19 pandemic, healthcare delivery was leaned towards telemedicine or telehealth to conduct social distancing. The primary aim of this review is to evaluate formats and strategies used in telemedicine to improve physical activity in adults with diabetes mellitus. ProQuest, Medline, and CINAHL were searched for studies involving adults with diabetes mellitus (more than 18 years old); telemedicine; and physical activity outcomes. The quality of the studies was appraised using the JBI Critical Appraisal Checklists for risk of bias, study design, and quality of evidence. Data syntheses were conducted following a simplified approach by Popay et al. Themes were developed based on the findings. Five randomized controlled trials met the inclusion criteria. The risk of bias was determined low in most of the studies. Text messages were the most common method. Pedometer, accelerometer, and International Physical Activity Questionnaires were commonly used to quantify the level of physical activity. Frequent reminders using text messages to the participants were the common strategy to ensure their adherence. The telehealth intervention has not yet shown significant positive effects on physical activity in people with diabetes mellitus. Further studies conducting synchronous and hybrid telehealth to improve physical activity in adults with diabetes mellitus were needed. Abstrak: Aktivitas fisik telah dievaluasi untuk menurunkan angka kematian pada penderita diabetes mellitus. Setelah pandemi COVID-19, pemberian layanan kesehatan condong ke telemedicine atau telehealth untuk melakukan social distancing. Tujuan utama dari tinjauan ini adalah untuk mengevaluasi format dan strategi yang digunakan dalam telemedicine untuk meningkatkan aktivitas fisik pada orang dewasa dengan diabetes mellitus. ProQuest, Medline, dan CINAHL dicari untuk penelitian yang melibatkan orang dewasa dengan diabetes mellitus (lebih dari 18 tahun); telemedis; dan hasil aktivitas fisik. Kualitas studi dinilai menggunakan JBI Critical Appraisal Checklists untukrisiko bias, desain studi, dan kualitas bukti. Sintesis data dilakukan mengikuti pendekatan yang disederhanakan oleh Popay et al. Tema dikembangkan berdasarkan temuan. Lima uji coba terkontrol secara acak memenuhi criteria inklusi. Risiko bias ditentukan rendah di sebagian besar penelitian. Pesan teks adalah metode yang paling umum. Pedometer, akselerometer, dan Kuesioner Aktivitas Fisik Internasional biasanya digunakan untuk mengukur tingkat aktivitas fisik. Pengingat yang sering menggunakan pesan teks kepada para peserta adalah strategi umum untuk memastikan kepatuhan mereka. Intervensi telehealth belum menunjukkan efek positif yang signifikan terhadap aktivitas fisik pada penderita diabetes mellitus. Diperlukan penelitian lebih lanjut yang melakukan telehealth sinkron dan hibrida untuk meningkatkan aktivitas fisik pada orang dewasa dengan diabetes mellitus.

Abstract

Waiting time for prescription services is one indicator of minimum pharmacy service standards. This study aims to determine the average time, coverage of compliance, and to find out the obstacles related to prescription services. This research uses mixed methods. The quantitative approach was carried out using the prescription service worksheet instrument. The qualitative approach was carried out by observing the process during the prescription service. Data were taken from March to April 2019 at a private type B hospital in West Java Province. The number of samples was determined based on the Slovin formula with an error tolerance value of 5%, obtained a sample of 60 non-concoction prescriptions and 60 concoction prescriptions. This study applied descriptive statistics with univariate analysis. The results showed the average service time for non-concoction prescriptions was 26 minutes 58 seconds and 40 minutes 14 seconds for concoction prescriptions. The coverage of the suitability of service time based on the Decree of the Minister of Health of the Republic of Indonesia was 68% (non- concoction prescriptions) and 92% (concoction prescriptions), while according to the hospital's internal quality standards it was 3% (non-mixed prescriptions) and 15% (concoction prescriptions). Several obstacles occurred during the process of receiving prescriptions, working on compound prescriptions, submitting prescriptions, confirming doctors, and peak days for patients. The establishment of hospital internal quality standards related to the timing of prescription services needs to be reviewed and the application of the lean hospital concept needs to be considered.

Abstract

Generator set (genset) merupakan utilitas rumah sakit dan termasuk dalam pengawasan instalasi sarana prasarana rumah sakit. Keandalan waktu genset untuk memberikan suplai otomatis harus tercatat dan dievaluasi secara berkala, karena hal ini memengaruhi suplai listrik untuk peralatan medis yang berhubungan langsung dengan keselamatan pasien. Salah satu kegiatan yang harus disiplin dalam pelaksanaannya adalah pengisian checklist pemeliharaan. Penelitian ini dilakukan untuk mengetahui gambaran dalam pemeliharaan generator set secara berkala. Penelitian ini menggunakan metode kuantitatif. Data yang digunakan merupakan data sekunder yang meliputi data pemeliharaan, data operasional, dan data kerusakan genset Tahun 2021. Data yang dianalisis adalah perhitungan Specified Operating Time (SOT), Actual Operating Time (AOT), Mean Time Between Failure (MTBF), keandalan dan ketersediaan. Data diolah menggunakan program spreadsheet dengan analisis univariat. Hasil penelitian menunjukkan bahwa SOT genset tahun 2021 sebesar 8.760 jam, AOT genset 1 sebesar 8.720 jam dan AOT genset 2 sebesar 8.724 jam, MTBF pada tahun 2021 menggambarkan rendahnya waktu kegagalan yang terjadi, terdapat penurunan persentase keandalan pada genset 1 dan ketersediaan sebesar 99,5%.
