Abstract

Sebagian besar etiologi hipertensi pada anak adalah sekunder dengan kelainan yang mendasari, berbeda dengan dewasa yang biasanya primer. Renovascular hypertension (RVH) terjadi lebih sering pada remaja daripada usia dewasa. Stenosis arteri renalis adalah istilah umum yang merujuk pada lesi vaskuler yang menyebabkan penyempitan arteri renalis sehingga mengganggu aliran darah ke ginjal. Proses untuk mengidentifikasi hipertensi sekunder dan mencari penyebabnya selalu menjadi tantangan pada remaja. Dilaporkan kasus hipertensi akibat stenosis arteri renalis pada remaja perempuan, usia 15 tahun 3 bulan yang didiagnosis berdasarkan gejala klinis, USG doppler dan CT angiografi.

Abstract

Latar belakang. Penyakit kardiovaskular (PKV) merupakan penyebab orbiditas dan mortalitas tersering pada penyakit ginjal kronik (PGK) anak. Hipertrofi ventrikel kiri (LVH) dan disfungsi diastolik paling awal terlihat.Tujuan. Mengetahui proporsi PKV pada PGK stadium 1, hubungan stadium PGK dengan LVH, dan disfungsi jantung.Metode. Penelitian potong lintang analitik komparatif 26 subjek PGK anak di RS M Djamil Padang/ RS Hasan Sadikin Bandung. Dilakukan pemeriksaan urinalisis, hematologi rutin, ureum, kreatinin (eLFG), EKG, foto toraks, ekokardiografi. LVH dengan ekokardiografi bila left ventricular mass index (LVMI) >persentil 95 (38g/h2,7). Uji stastistik bermakna bila p<0,05.Hasil. Rerata umur subjek 9,1(3,8) tahun. LVH pada 1 dari 3 subjek PGK stadium 1 dan 61,5% pada PGK seluruh stadium, terbanyak stadium 5. Tidak terdapat hubungan bermakna stadium PGK dengan LVH (p=0,055), disfungsi diastolik (p=0,937) dan disfungsi sistolik (p=0,929).Kesimpulan. Pada PGK stadium 1 ditemukan LVH dan disfungsi diastolik. Tidak terdapat hubungan antara stadium PGK dengan LVH dan disfungsi jantung.

Abstract

Jengkolic acid intoxication is one of the causes of acute kidney injury in children. This case report discusses the diagnosis and management of jenkol intoxication in children. We reported a 12-year-old boy with painful urination accompanied by low back pain and urine was initially whitish-colored sand then turned red-colored urine accompanied by reduced urine volume after consuming jengkol. The patient was diagnosed with acute kidney injury due to jenkolic acid intoxication, urinary tract infection, and bilateral hydronephrosis with initial conditions of oliguria, elevated urea, the impression of an estimated glomerular filtration rate (GFR) was stage failure, and planned emergency hemodialysis. The patient was managed with bicarbonate sodium and other symptomatic therapy. During monitoring of patients with increased urine output, decreased ureum and increased estimated GFR, hemodialysis was not performed. The patient was treated for 6 days and recommended for control at the pediatric clinic.

Abstract

Background: Several conditions in children can cause nutritional intake via the oral route to be inadequate. Providing enteral nutrition via a nasogastric tube (NGT) is an alternative option. Research on the use of NGTs in Indonesia is still limited, so this research aims to determine the effect of NGT use on the weight of pediatric patients with feeding difficulties at Dr. M. Djamil General Hospital, Padang. Methods: Experimental research with a pre and post-test-only design was conducted on 96 children aged 1 month - 18 years in the children's inpatient room at Dr. M. Djamil General Hospital, Padang. Body weight measurements were carried out before and after using NGT for 4 weeks (January 2022 – January 2023) at the pediatric nutrition and metabolic diseases polyclinic. Data were analyzed using paired t-test and ANOVA. Results: Most subjects (40.9%) were in the 1-12 month age group. The most common type of nutrition was ONS (oral nutritional supplements) (84.4%), the most common diagnosis was pulmonology (33.3%), and the majority were without complications (72.9%). There is an influence on the child's weight before and after using NGT (p-value <0.01). Significant differences were also found in changes in children's weight with diagnosis and complications (p<0.01). Conclusion: The use of NGT can increase the weight of pediatric patients who have feeding difficulties at Dr. M. Djamil General Hospital, Padang. Significant differences were found in changes in children's weight by diagnosis and complications.
