Abstract

Post-stroke patients experience in various disturbances including physical, psychological and spiritual aspect. Post-stroke patients in the rehabilitation phase focus more on physical needs while the unfulfilled spiritual needs can reduce the patient's health condition. Therefore, it takes action to meet the spiritual needs of patients. This study was conducted to identify the spiritual needs among post-stroke patients.The descriptive quantitative was used among post-stroke patients in Neurological Polyclinic and stroke center in one of the Hospital in Bandung in 2017. The sampling technique used was consecutive sampling with 83 samples included post-stroke patients who have full awareness and didnot experience of aphasia. The instrument of this study used Spiritual Needs Questionnaire (SpNQ) which consists of 4 dimensions including religious, peace, self-existence, and dimension of the giving needs. SpNQ had a validity coefficient of 0.73 and r 0.75. Data were analyzed using frequency distribution. The results showed that the most of respondents feel the spiritual need in all dimensions. In the religious dimension, the majority of respondents desired to pray with others, to move closer to God and participate in various religious activities (98.8%). In the dimension of peace showed the most of respondents desired to be more cherished by others (96.4%). The most perceived item of respondents were forgiving someone from the past in the dimension of self-existence (98.8%) and a solace for others in the dimension of the giving needs (98.8%). The conclusion from this study was most respondents feel the spiritual needs in all dimensions. Based on these results, nurses are expected to provide advice to people around the patients to invite them in various religious activities. Nurses supposed more expressing affection to them and involving the patients in some family activities.

Abstract

Cardiac arrest is a sudden cessation of heart activity that requires immediate treatment in the form of Basic Life Support (BLS). Students who are members of the Student Activity Unit (UKM) in the humanitarian field at Unpad are potential first aiders to provide basic life support in the event of cardiac arrest. This study aims to determine the level of knowledge of basic life support in students who are members of UKM in the humanitarian field at Universitas Padjadjaran. The method used in this research is quantitative descriptive with a cross sectional approach. The population in this study amounted to 158 active members of the humanitarian student activity unit and the sample was taken using convenience sampling type (n = 120). Data were collected using a questionnaire containing 30 questions that have been declared valid and reliable with a Cronbach's alpha value of 0.797 and data were analysed using computer software with univariate analysis techniques. The results showed that the level of knowledge of humanitarian UKM members at Unpad regarding basic life support was classified into the good category with a total of 108 respondents (90%). The conclusion in this study is that the level of knowledge of basic life support in members of humanitarian UKM at Universitas Padjadjaran is in the good category, but it is necessary to increase knowledge, especially on procedural topics related to basic life support.

Abstract

Cardiovascular disorders have been associated with coronavirus disease 2019 (COVID-19). One of the emerging complications is acute pericarditis, characterized by inflammation of the pericardium. Pericarditis is a complication that arises after COVID-19 infection because the virus can cause widespread inflammation, including in the pericardium, the outer layer of the heart. Risk factors that may exacerbate pericarditis in COVID-19 patients include the severity of COVID-19 disease, an immunocompromised state, or an excessive immune system response to infection. The aim of the study is to describe a case of acute pericarditis that occurred after Severe Acute Respiratory Syndrom - Coronavirus 2 (SARS-CoV-2) infection. This study used a case study design. This study was conducted on one of the patients who experienced pericarditis after Covid-19. Researchers conducted nursing assessments and analyzed nursing data. Data analysis was carried out descriptively to describe the results of nursing assessment. A few days after experiencing SARS-CoV-2 pneumonia, 51-year-old Mr. S. developed chest pain that became more painful during exertion, and received a diagnosis of acute pericarditis. As a result, medical treatment was escalated with clinical improvement continuing to improve. Electrocardiography showed elevation in ST segment and depression in PR interval. Echocardiography showed grade I diastolic phase disruption. Acute pericarditis is a rare complication following viral infection. This report shows the assessment and management of acute pericarditis so as to prevent complications that can occur.

Abstract

Burns are serious injuries that can have significant physical and psychological impacts on individuals. Burns due to electric shock are relatively rare but carry a high risk of serious complications, including respiratory problems and fluid loss that can be life-threatening. Purpose: The purpose of this study is to present the case study of burn electrical burns injury with a history of heart disease. Methods: This study used a case report approach on one patient. Data were collected by conducting interviews and assessments to one patient with electrical burn injury with a history of heart disease. Data analysis was carried out by comparing the results of the assessment with the theory of electrical burn injury. Results: The results of this study indicate that Mr. P, aged 35 years, showed burns covering 65% of the total body surface, with grade III severity. The patient also experienced respiratory problems with SpO2 85%. In addition, other nursing diagnoses were found such as impaired airway clearance due to edema in the larynx, hypovolemia due to active fluid loss, and acute pain due to physical injurious agents. This is the importance of proper and rapid assessment to reduce the risk of more serious complications. Conclusions: Holistic therapy and a multidisciplinary approach are key in providing optimal care and minimizing the risk of complications in patients with complex conditions such as this.

Abstract

Methylmalonic acidemia (MMA) is a rare metabolic disorder caused by the body's inability to break down certain proteins and fats, leading to a buildup of toxic methylmalonic acid. MMA symptoms vary and can cause serious complications if not treated appropriately. Purpose : to explanation of electrolyte management and monitoring to prevent complications on patient with methylmalonic acidemia. Method : this study used case study. Collecting data used nursing assessment. An 8 year old patient with MMA, cough accompanied by vomiting, decreased appetite, and dehydration came to the emergency room in critical condition. The patient showed signs of dehydration, metabolic acidosis, and electrolyte disturbances that required immediate treatment. Results: Management of MMA involves adequate hydration, management of hypoglycemia, correction of metabolic acidosis, and careful monitoring of electrolytes. Additional therapies such as L-Carnitine supplementation and intensive clinical monitoring are also necessary to prevent life-threatening complications. Conclusion: Rapid and appropriate treatment of the metabolic crisis in MMA is essential to prevent serious complications and improve the patient's prognosis. Regular monitoring of electrolytes and clinical parameters as well as comprehensive therapy can help improve the quality of life of individuals with MMA.

Abstract

Caring behavior in nursing has an important role in improving patient satisfaction. Islamic caring behavior is not only limited to technical and professional aspects, but also includes spiritual and emotional dimensions in accordance with Islamic values. Purpose: The aim of this study is to explore how Islamic caring behavior is applied in nursing practice and its impact on patient satisfaction. Method: This study used a scoping review method by analyzing and synthesizing data from six relevant journal articles. The articles were selected based on specific inclusion criteria and analyzed to identify key themes related to Islamic caring behaviors in nursing. The databases used were PubMed, CINAHL, Scopus, and google scholar search engine. Results: The authors found 7 articles that discussed Islamic caring on patient satisfaction. Islamic caring behavior includes aspects of professionalism, empathy, patience, and spirituality. Factors that influence the implementation of Islamic caring behavior include nurse motivation, institutional support, and work environment. Specific activities such as prayer, recitation, and dhikr were also found to be integral to the practice of Islamic caring. In terms of impact, Islamic caring contributes to increased patient satisfaction through a more holistic approach to care. Conclusion: Integration of Islamic values in caring behavior can increase patient satisfaction in nursing services. The implication is that there needs to be greater recognition and support for the application of Islamic caring in nursing practice, both in terms of nurse education and health institutional policies.

Abstract

Guillain-Barre Syndrome (GBS) and Thrombotic Thrombocytopenic Purpura (TTP) are two medical conditions that are rare but can cause serious complications if they appear simultaneously. Both conditions involve autoimmune mechanisms and can result in organ failure, including respiratory failure. Although there are several case reports of each condition, research exploring the interaction between GBS and TTP in the context of respiratory failure is limited. Purpose: The aim of this study is to describe a case of cross-reactivity of Guillian-Barre Syndrome and Thrombotic Thrombocytopenic Purpura with complications respiratory failure. Methods: This research uses a case study design involving an adult patient who was treated at a hospital in Bandung. Data was collected through a comprehensive nursing assessment. Patients provided informed consent before data were collected. Data analysis was carried out descriptively to describe clinical findings and patient management results. Results: The patient is a young woman 27 year old admitted to hospital with a diagnosis of GBS. Eleven days before entering the hospital the patient experience dascending paralysis, fever, nausea and vomiting.TTP cross-reactivity and peripheral nervous system inflammation in GBS.Complications of respiratory failure requiring invasive mechanical ventilation (IMV) in the ICU. Laboratory and radiological examinations supported the diagnosis, and medical intervention was performed according to protocol for both conditions. This shows the importance of close monitoring and prompt treatment to prevent further complications. These findings emphasize the need for a multidisciplinary approach in managing patients with complex medical conditions. Conclusion: An in-depth understanding of the interaction between GBS and TTP is essential for effective clinical management, especially in preventing and managing complications of respiratory failure.

Abstract

Sepsis shock, characterized by sepsis-induced circulatory or metabolic disturbances, remains the leading cause of death in ICUs globally. Infection by Acinetobacter baumannii, which is frequently found in medical environments, can trigger life-threatening sepsis shock. This condition occurs when the bacteria or its bacterial products trigger adverse reactions from the immune system. ARDS, as a serious complication, results from endothelial damage that causes fluid to enter the alveoli, disrupting gas exchange and leading to respiratory failure. Purpose to describe a case in a patient diagnosed with epilepticus, with a history of autistic disorder, and non-specific pneumonia aged 30 years. This case report reviews a 30-year-old man admitted with a diagnosis of status epilepticus, with a history of autistic disorder, and non-specific pneumonia. The patient showed progressive improvement with initial therapy, including antiepileptic therapy and empirical antibiotics for pneumonia. However, the patient's condition deteriorated later, resulting in Acute Respiratory Distress Syndrome (ARDS) due to septic shock by Acinetobacter baumannii. The patient required mechanical ventilation and intensive management, including a change in antibiotic therapy and tracheostomy. With appropriate intervention, the patient showed improvement and was successfully discharged from mechanical ventilation. This patient with severe ARDS caused by sepsis shock due to Acinetobacter baumannii infection, demonstrates the importance of prompt and appropriate management of the complications of sepsis to prevent the patient's death. Acinetobacter baumannii infection causing ARDS and sepsis shock is a serious and rarely reported case, where aggressive treatment of sepsis with appropriate antibiotics and careful management of mechanical ventilation can improve the progression of ARDS.

Abstract

Cryptococcal meningitis is an infection that attacks the tissue lining the brain and spinal cord, caused by the fungus Cryptococcus neoformans. This infection mainly occurs in individuals with a weakened immune system, such as people with HIV/AIDS. The spread of this fungus into the central nervous system can result in serious conditions, even fatal if not treated quickly and appropriately. Objective: to present a rare and complex case report of a patient with concurrent cryptococcal meningitis and multiresistant tuberculous meningitis, as well as evaluate the management strategies used in the patient's care. Method: Case study was conducted through a comprehensive approach including health history assessment, physical examinations, psychosocial analysis, customized nursing assessment forms, electronic medical record reviews, laboratory diagnostics, imaging studies, and multidisciplinary consultations to ensure accurate diagnosis and effective interventions Mrs. R, a 33 year old woman with a final diagnosis including Pneumonia with aspiration mechanism, Cryptococcal Meningitis, Grade II MDR TB Meningitis complicated by arteritis, as well as several other conditions including Hyponatremia, hypochloremia, and hypocalcemia due to insufficient intake, Chronic Suppurative Otitis Media (CSOM) and Typhoid Fever. Results: The patient was diagnosed with aspiration pneumonia, cryptococcal meningitis, multiresistant tuberculous meningitis, as well as other conditions such as hypernatremia, hypokalemia, hypocalcemia, chronic suppurative otitis media, and typhoid fever. Despite treatment, the patient's condition worsened, leading to respiratory failure and death from aspiration pneumonia. Conclusions: This case report highlights the diagnostic and therapeutic challenges in managing concurrent cryptococcal meningitis and multiresistant tuberculous meningitis. This emphasizes the importance of a comprehensive approach in diagnosis and management in patients with complex concurrent infections, even without HIV. This case also emphasizes the need for further research and improved strategies for managing rare and complex infections such as this.
