Abstract

Infark Miokard merupakan salah satu penyakit terminal yang memerlukan perawatan intensif. Perawataan intensif yang diperlukan harus holistik, mencakup bio psiko sosial dan spiritual. Psikologis infark miokard harus selalu diperhatikan, karena salah satu penyebab infark miokard adalah dari psikologis atau dikenal dengan stress. Tujuan dari penelitian ini untuk mengetahui pengalaman pasien infark miokard akut yang menjalani perawatan di ruang intensif. Jenis penelitian adalah deskriptif kualitatif dengan pendekatan phenomenology yang dilakukan di salah satu rumah sakit di Bandung periode Juni-Juli 2013. Jumlah informan 10 orang pasien infrak miokard akut yang pada saat dilakukan wawancara sudah dalam perbaikan killip I dan II yang diambil secara purposive sampling, dirawat di ruang intensif dan kondisinya telah stabil. Pengumpulan data dilakukan dengan wawancara mendalam kemudian dianalisa menggunakan content analysis dari Hancoch. Hasil penelitian didapatkan 3 tema penelitan yaitu seluruh responden merasa tidak berdaya, 9 responden mengalami ketidakpastian menghadapi masa depan dan 10 responden menyatakan ketakutannya akan kematian. Seluruh pasien infrak miokard mengalami masalah psikologis, oleh karena itu hasil penelitian ini dapat menjadi rekomendasi dalam memberikan layanan kesehatan bagi pasien kondisi terminal; infark miokard akut yang sedang menjalani perawatan intensif. Penting kiranya untuk dapat mengelola dan mengintegrasikan pelayanan perawatan pada pasien infark miokard akut yang sedang dirawat di unit intensif secara holistik meliputi fisik psikologis sosial dan spiritual. ABSTRACTCommunication is a very important process in human relationship. In providing nursing care, nurses should have a good knowledge and communication skill as the beginning of a good relationship between nurses, patients, and their families. Nurses with good communication skill had an easier opportunity to make a good relationship with the patient and their families. This study aimed to identify effective communication barriers among nurses in developing communication with patients’ family according to nurses’ perspective in Intensive Care Unit Rumah Sakit Umum Al Islam Bandung. This descriptive explorative study involved 10 nurses were taken with accidental sampling. Data were gathered using interview and observation. Data analyzed with the content analysis. Result showed that there were at least five topic of effective communication barriers among nurses in developing communication with patients’ family according to nurses’ perspective in Intensive Care Unit Rumah Sakit Umum Al Islam Bandung; role conflict, family demographic factors, misunderstanding, environment and situation in the ICU, and family psychological condition. So, training related to communication between nurses and patients’ family were necessary to undertake in order to improve the ability of nurses such as foreign language skills and patience in dealing with the situation in the ICU especially in relation to the patient's family. This is because nurses are the spearhead of health care service in hospital.

Abstract

Introduction: Stroke is the second leading cause of death in the world and the most common disability-causing disease. Stroke causes the physical changes of paralysis, social changes of communication problems, cognitive impairment and psychological effects of difficulty in understanding, forgetfulness, and depression. The impact of post-stroke patients is very complex and comprehensive in terms of physical, psychological, and social change. Health workers, especially nurses need to provide nursing care in post-stroke patients comprehensively. The purpose of this study was to explore the life experiences of post-stroke patients. Method:The research method used in this study was phenomenology. The subjects in this study were post-stroke patients with the number of informants of 6 post-stroke patients obtained through purposive sampling technique. The data collection was conducted through in-depth interviews with the help of recording devices conducted in March-April 2017. Result and analysis: The data were analyzed using Collaizi.  The results of the study showed that there were five themes related to the life experiences of post-stroke patients, namely: helplessness, sadness, anger, shame of life uncertainty, and decrease of social contact and hope to be healed. In conclusion, the results showed that post-stroke patients had physical and functional disorders causing psychological and social responses that affected the life experience. Discussion: Based on the results of this study, it is necessary to provide the peer support of post-stroke patients and appropriate information by health workers, family and the environment to help patients undergo the process of treatment and recovery.

Abstract

 ABSTRACTThe end-of-life (EOL) care is the progressive terminal illness, leading to death; that in the situation the nurse have significant role in decision making. In decision making, temporarily, the nurse will experiences the ethical dilemmas, including in intensive care unit (ICU) with any factors resulting in problem in medical decision making. The literature review intends to analyze the description of ethical dilemmas occurred in intensive care unit, especially to the EOL care. A method used is critical review full text of 2007-2018 periods in English langguage. The multiple databases used is PubMed, Proquest and Google Scholar with keyword “End of Life Care” and “Nursing Ethic in critical care” and “issue End-of-life critical care”, and “Dilemmas Ethic in ICU.” The articles selected gradually by using of Appraisal tool of PRISMA and obtained 21 articles. The literature study obtaining 4 themes related to description of the ethical dilemmas in ICU including (1) the ethical principle involved in EOL care at ICU, (2) resource of ethical conflict in ICU, (3) impact of ethical conflict in ICU, and (4) response of nurse in dealing with ethical dilemmas. The discussion of literature review related to the perception of nurse on EOL care; that nurse have important role in medical decision making involving ethical principle in the implementation. it is required further research on exploration of nurse experience on implementation of ethical principle in case of EOL care.ABSTRAKPerawatan end of life (EOL) merupakan suatu perawatan pada penyakit terminal yang bersifat progresif, yang akan berujung dengan kematian, pada kondisi ini perawat mempunyai peran dalam pengambilan keputusan perawatan. Terkadang dalam pengambilan keputusan ini perawat akan mengalami kondisi dilema etik, tak terkecuali di ruangan intensive care unit dimana diruangan tersebut banyak faktor yang mengakibatkan kesulitan dalam pengambilan keputusan medis. Literature reviews ini bertujuan untuk menganalisia gambaran dilema etik yang terjadi di area intensive khususnya pada kasus end of life care. Metode yang digunakan adalah critical review full text dengan rentang tahun 2007-2018 dalam Bahasa Inggris. Multiple database yang digunakan adalah PubMed, Proquest dan Google Scholar merupakan database yang digunakan, dengan kata kunci “End Of Life Care” and “Nursing Ethic in critical care” and “issue End-of-life in critical care”, and“Dilema Ethic in ICU”. Artikel diseleksi bertahap menggunakan Appraisal tool PRISMA dan didapatkan 21 artikel. Studi literatur diperoleh 4 tema terkait gambaran dilema etik di ruang ICU diantaranya (1) Prinsip etik yang terlibat dalam end of life care di ICU, (2) Sumber konflik etik di ICU, (3) Dampak konflik etik di ICU, dan (4) Respon perawat dalam menghadapi dilema etik. Pembahasan telaah literatur ini terkait persepsi perawat mengenai perawatan end of life, dimana perawat mempunyai peran penting dalam pengambilan keputusan medis yang melibatkan prinsip etik dalam pelaksanaannya. Diperlukan penelitian lebih lanjut mengenai eksplorasi pengalaman perawat terhadap implementasi prinsip etik pada kasus perawatan menjelang ajal.

Abstract

There are many cardiac arrest events in the community that can not be saved because they do not get the right and quick action due to the ignorance and the inability of the nurses of the puskesmas to provide emergency response to cardiac pulmonary resuscitation. Cardiopulmonary resuscitation (CPR) is the main action in cardiac arrest and stop breath, which can increase life expectation. Goal to be achieved in the implementation of this program is increased ability of nurse puskesmas in performing action of heart resuscitation of lung. The specific targets of the activities are the increased ability of health center nurses on the ability to perform cardiopulmonary resuscitation with the latest guidelines and the implementation of a structured training program on pulmonary cardiac resuscitation at community health center nurses on an ongoing basis. Solutions offered to overcome the problem is the training of Lung Heart Resuscitation (RJP) to nurses Puskesmas. Training activities conducted for one day include pre-test evaluation, material exposure, rjp skills training and post training evacuation. The result showed that there was significant difference between attitude value and perceived behavioral control between before and after training, whereas there was no difference of subjective norm value between before and after training. Training on updating and increasing the competence of cardiopulmonary resuscitation should be done regularly and periodically. Routine and periodic training needs to be done to improve nursing services, especially cardiovascular emergency

Abstract

Background: The Early Warning Score (EWS) system has been recommended for early identification tool of deterioration. However, its implementation has not been optimal; one of which is due to the low level of knowledge and understanding of EWS among nurses.Purpose: This study aimed to determine the effects of EWS tutorial simulation on nurses’ knowledge and clinical performance.Methods: This study employed a pretest posttest quasi-experimental design with a control group. Purposive sampling was used to recruit the samples of 42 respondents each in the intervention group and control group. The data were collected using the questionnaires to measure the knowledge and clinical performance, and analyzed using Chi square, Wilcoxon and Mann-Whitney tests.Results: The results showed that there were differences in the pre-test and post-test of knowledge and clinical performance in the intervention group and control group (p<0.001). There was also a significant difference in clinical performance between the intervention group and the control group (p<0.001). However, no significant difference in knowledge was found between.Conclusions: Tutorial simulation of EWS had an effect on increasing nurses’ clinical performance. Although there was no significant difference in knowledge between the intervention group and the control group, but the intervention group showed a better value than the control group. EWS tutorial simulation can be used as one of the training methods to increase nurses' knowledge and clinical performance in EWS.

Abstract

Background: Cardiac chest pain is a typical complaint experienced by patients with Acute Coronary Syndrome (ACS) in Emergency Departments (EDs). Pharmacological therapy is one major intervention used to reduce cardiac chest pain due to ACS. However, this therapy does not optimally and completely reduce cardiac chest pain; therefore, additional therapy is greatly required.Purpose: This study aimed to examine the effect of Dhikr therapy as one of the additional therapies for the cardiac chest pain experienced by patients with ACS in EDs.Methods: This quasi-experimental research was conducted using a pretest-posttest control group design. As many as 52 patients with ACS were recruited using a consecutive sampling technique and then equally divided to the intervention and control group. The intervention group received both pharmacological and Dhikr therapy approximately for 17 minutes, while the control group only received the pharmacological therapy based on the hospital’s protocol. The Numeric Pain Rating Scale (NPRS) was used to measure the intensity of cardiac chest pain, and both paired and independent t-tests were utilized to analyze the data.Results: The results showed that there was a significant difference in pain reduction in both groups (p=0.000), although the decrease in the intervention group was higher than that in the control group. Furthermore, the pain reduction was significantly different between groups (p=0.021)Conclusion: Dhikr combined with the pharmacological therapy reduced the intensity of cardiac chest pain in ACS patients better than the use of pharmacological therapy alone. Therefore, this study recommends the combination of pharmacological and Dhikr therapy for patients with ACS.

Abstract

The Emergency Department (ED) is a hospital service unit that provides the first service for patients with disease conditions that threaten their lives or can cause disability for 24 hours. Implementation of patient safety in the ED should be applied to minimize the risk of error handling for the patient. ED staff perceptions related to the implementation of patient safety is a factor that directly-related to his behavior in applying the implementation of patient safety. This study aimed to analyze the relationship between perceptions of staff ED and patient safety by implementing patient safety at the Regional Hospital Emergency Department Cirebon. This study was a correlational study with the cross-sectional approach of 99 emergency staff with total sampling at Cirebon. Collecting data used questionnaires of patient safety. Based on the results of the univariate analysis showed that the majority (80%) of respondents either category on the implementation of the sub-variables of patient safety team collaboration and communication, only a small proportion of respondents less category (20%) on the implementation of the sub-variables of patient safety team collaboration and communication. In addition, less than half (49.5%) category lacking in implementing patient safety, only half (50.5%) categories, both in the implementation of patient safety. Based on the results of the bivariate analysis showed that the relationship implementation of patient safety with all the variables, namely teamwork (p-value = 0.000), communications (p-value = 0.005), the concept of patient safety (p-value = 0.005), and perception (p-value = 0.005). Based on the results of the study, the researchers concluded that the relationship between staff perceptions of the emergency department (ED) on patient safety by implementing patient safety at the Regional Hospital emergency department (RSD) Cirebon. IGD support staff perceptions of patient safety, but still found lacking in the category of health workers implementation of patient safety, so the need for patient safety education and training with simulation methods to illustrate the approach in the implementation of patient safety.

Abstract

End stage renal disease (ESRD) is a serious chronic disease that resulted from complications of chronic renal failure and a final diagnosis that requires treatment modalities such as dialysis or continuous ambulatory peritoneal dialysis (CAPD). However, CAPD is rarely selected as renal replacement therapy by ESRD patients in Indonesia. Therefore, the phenomenon associated with patients undergoing CAPD is unclear, especially in Indonesia. The purpose of this study was to explore the lived experiences of ESRD patients undergoing CAPD therapy. The research method employed qualitative descriptive phenomenology. The participants in this study were 10 ESRD patients with CAPD. Data collection utilized semi-structured interviewed and data was analyzed using the Colaizzi approach. The result of the study was explained through 6 themes. These are: 1) Condition improved with CAPD, 2) Freedom of activity, 3) Between feeling better and bored 4) Health is increased, 5) It is sustained life, 6) Resignation to accept illness. In conclusion, the patients with CAPD felt significant positive changes in their life and CAPD is considered life-sustaining for the participants, as the majority of the symptoms was decreased significantly. Therefore, the partisipants felt their conditions improved better. The unpleasant sides of CAPD are feeling bored. The support of the family is important as it produced strength and passion for patients in undergoing CAPD

Abstract

The family presence during resuscitation (FPDR) has received wide support to be implemented in the ICU.The implementation of FPDR has positive impacts on family satisfaction. However, some of the nurses’ opinion still not agree on implementation of FPDR in ICU. This research therefore utilized phenomenological approach to gain descriptive explanation regarding this phenomenon. The purpose of this study is to explore the nurse’s experiences in the implementation of FPDR in the ICU. This research was conducted on the year 2020 and qualitative phenomenological approach was utilized for this research. Ten (10) ICU nurses have participated in this study and in-depth interview was employed for data collection. Data analysis is conducted using Colaizzi. Result of this research areexplained in 6 themes which describe the phenomenon of nurses’ experience onfamily presence during recusitation, as follows: (1) Decisions on family; (2) From burdened to selfconfident; (3) Families enhancing teamwork; (4) The bereaved family disrupts the CPR process; (5) Feels of ignoring patiets’ family and (6) Expectingthe family be accompanied by staff nurse.From a nurse’s perspective, the implementation of FDPR has a positive impact on patients, families and on the ICU nurse team. However, this also has an unpleasant impact on ICU nurses, especially if the nurse is not ready for the practice. Therefore, policy support and training for nurses are needed in its implementation.
