Abstract

Type 2 diabetes mellitus (T2DM) is group with high cardiovascular disease (CVD) risk. Inflammation which is reflected by increasing leukocyte number, CRP, and lipid profile. Despite that, infection as one of diabetic complication commonly occurs in patient with T2DM. The dyslipidemia status reflected by lipid profile composition. So here, we aim to evaluate the relationship between leukocyte count with lipid profile to know the association between traditional and non-traditional risk factor of CVD in T2DM patient. The type of this research is cross sectional analytic research with 94 samples was enrolled into this study. The data is obtained initially by collecting from the quistionare and from laboratorium examination. Mean of samples’ age is 55 years and large number of patients had leukocytosis as reflected by high value of its mean (15.02±8.78 x 103/?L). We found that 56.4% of the subjects fall into worst category of glycemic control (HbA1C). Only 24.5% of the subjects had good glycemic control. Further evaluation showed that there were significant strong negative correlations between leukocyte count with HDL (r= -0.555; p=0.000). Inflammation which mainly related to infectious disease is common among T2DM patients which increase CVD risk by lowering HDL and worsening glycemic control.Keywords: Type 2 Diabetes Mellitus, lipid profile, leukocyte count

Abstract

BACKGROUND: Diagnosing iron deficiency anemia (IDA) is easy, but also can be complicated in condition with inflammation. A new modality for diagnostic which isn’t influenced with inflammation is needed. The aim of this study is to find the cut-off point and evaluate the accuracy of reticulocyte hemoglobin equivalent (Ret-He) to diagnose IDA using ferritin as the gold standard.METHODS: This study was an observational study with cross-sectional analytical design continued with the diagnostic test conducted in anemic individuals with age 18 years old or above.RESULTS: Eighty-seven patients (41 men and 46 women) were included in this study with mean of hemoglobin 7.42 g/dL, serum iron 42.71 mg/dL, total iron-binding capacity (TIBC) 242.82 mg/dL, ferritin 799 ug/L and Ret-He 23.63 pg. Ret-He with cut-off value 25 pg showed a sensitivity 97.2% (95% CI 83.79-99.85%), specificity 66.67% (95% CI 51.97-78.85%), positive predictive value 67.30% (95% CI 52.77-79.28%) and negative predictive value 97.14% (95% CI 83.38-99.85%).CONCLUSION: Ret-He showed the best sensitivity for detection of IDA and was suggested as the screening test for IDA.KEYWORDS: IDA, Ret-He, diagnostic test

Abstract

Introduction: Anemia is a clinical condition associated with many causes. Anemia is not a disease, it is a syndrome and generally described as a decreased of red blood cell mass and hemoglobin below the normal level. Based on WHO's criteria, someone can be diagnosed with anemia if their hemoglobin level falls below 130 g/L for men and below 120 g/L for women. Based on a survey made by WHO, about one from every four people in the world suffered from anemia.Aim: The study aims to evaluate the patient arrival pattern to the Hematological Policlinic in RSUP Sanglah Denpasar. Method: This is analytic observational research of the anemic patient who came to the Hematological Policlinic in Sanglah General Hospital from April 2015-April 2017.Result: The highest proportion of anemic patient who came to Hematologic Policlinic in Sanglah General Hospital based on age classification are the age group of 15-59 while based on sexes the percentage of the anemic male patient is 44.2%, and the anemic female is 55.8% while 2.9% of that female is currently pregnant.Conclusion: From the causative point of view, the anemic patient in Hematologic Policlinic in Sanglah General Hospital from April 2015 to April 2017 mostly diagnosed with unspecified type while 58.7% of the anemic patient come without another health issues and the other 41.3% have had underlying or accompanying diseases.Introduction: Anemia merupakan suatu kondisi klinis yang diasosiasikan dengan banyak penyebab. Anemia bukan merupakan suatu penyakit melainkan suatu syndrome dan pada umumnya didefinisikan sebagai penurunan sel darah merah dan hemoglobin di bawah nilai normal. Berdasarkan kriteria WHO, seseorang dapat didiagnosis Anemia jika hemoglobinnya berada di bawah 130 g/L untuk laki-laki dan di bawah 120 g/L untuk perempuan. Berdasarkan survey yang dibuat oleh WHO, anemia diderita oleh satu dari empat orang di dunia.Tujuan: Penelitian ini bertujuan untuk mengetahui pola kunjungan pasien ke Poliklinik Hematologi RSUP Sangah Denpasar.Metode: Penelitian ini merupakan suatu penelitian observasional pada pasien anemia yang datang ke poliklinik hematologi RSUP Sanglah dari April 2015-April 2017.Hasil: Berdasarkan penelitian ini dapat dikonklusikan bahwa proporsi tertinggi pasien anemia yang datang ke Poliklinik Hematologi RSUP Sanglah paling banyak adalah dari kelompok umur 15-19 tahun. Sementara itu, berdasarkan kelompok jenis kelamin ditemukan bahwa presentasi pasien anemia perempuan adalah 55,8% dengan 2,9% dari pasien tersebut sedang dalam keadaan hamil sementara pasien anemia laki-laki adalah 44,2%.Simpulan: Dilihat dari penyebabnya pasien anemia yang datang ke Poliklinik Hematologi RSUP Sanglah dari April 2015-April 2017 paling banyak didiagnosis dengan tipe tidak terspesifikasi dengan 58,7% pasien anemia datang tanpa disertai penyakit lain dan 41,3% datang dengan penyakit penyerta.

Abstract

Introduction: Multiple myeloma is the second most common hematologic cancer. It is characterized by the clonal proliferation of malignant plasma cells and associated organ dysfunction. Ophthalmic manifestations include hyperviscosity retinopathy and retinal detachment. The aim of this study is to report retinal manifestation in Multiple Myeloma. Methods: A man 45 years old, complained loss of vision on both eyes especially left eye over 1 month. He also feels weakness and 3 times spontaneous nose bleeding. His visual acquity is 6/12 on the right and 2/60 left eye with Retinal Bleeding and Retinal Detachment. His Laboratory examination result increase in WBC 65.91 x 103/µL (Neutrophil, Monocyte, Basophil and Lymphocyte) decrease RBC 3 x 106/µL, HGB 6.85 g/dL, normal PLT 193.60 103/µL, Normal Bleeding and Clotting Time and increase ESR 140 mm/hour. Multiple myeloma was diagnosed by Internal Department with bone marrow biopsy and increase of Gamma Globulin. Results: The patient was treated with melphalan 3 tablet 2 times a day and prednisone 4 mg 3 times a day for 5 days every 28 days. After 8 month the VA 6/6 on the right and 6/10 on the left with complete resolution of on the right eye and there are resolution on the left eye with fibrosis, the laboratory examination normal WBC 3.8 x 103/µL with Neutrophil 59.3%, Monocyte 6.1 %, Eosinophil 6.3 %, Basophil 0.3 % and Lymphocyte 28.0 %, normal RBC 4.6 x 106/µL, slightly decrease HGB 12.8 g/dL, normal PLT 177.0 x 103/µL and decrease Gamma Globulin. Discussion: This case of bilateral Retinal Bleeding and Exudative Retinal Detachment are remarkable for both its presentation and response to melphalan therapy. Treatment with melphalan and prednison alone was sufficient to clear the Retinal Bleeding and restoration of Exudative Retinal Detachment. Conclusion: Complication of Multiple Myeloma can caused organ disfunction such as retinal bleeding and exudative retinal detachment. Early Diagnosis is very crucial. In our case combination cemotheraphy with Melphalan and Prednison showed clinically significant resolution. Workship Internal Department and Ophthalmology Department are very important for management and monitoring ophthalmology manifestation in multiple myeloma.

Abstract

Hyponatremia is one of the electrolyte disturbances that can be found in patients with central nervous system disorders, the most cases of hyponatremia are the Syndrome of Inappropriate Antidiuretic Hormone/SIADH compared to hyponatremia in Cerebral Salt Wasting Syndrome/CSWS and it is not uncommon to also occur in patients undergoing chemotherapy treatment without accompanying head abnormalities are referred to as Renal Salt Wasting Syndrome (RSWS). There is difficulty differentiating these two conditions due to almost the same clinical symptoms. Evaluation of volume status and monitoring of urate excretion fraction can help differentiate SIADH from RSWS and CSWS. Management requiring prompt diagnosis is essential to prevent severe hyponatremia from substantially causing cerebral edema and thereby reducing the risk of seizures. Fluid resuscitation therapy and sodium restoration are important. This case series report injured 2 CSWS patients after head trauma with different treatments and hyponatremia due to cisplatin induced RSWS.
