Abstract

Increasing age can cause a variety of degenerative disease, one of which is hypertension. One issue is the treatment of geriatric polypharmacy the drug interactions may pose clinical problems that require a careful monitoring and action, for example the use of antihypertensives. This study aims to determine the picture of drug use and the relationship between the number of medication and comorbidities with potential drug interactions. This study is an observational analytic study using cross sectional study design. Data is retrospectively collected by using affordable population that meet the criteria. The determination of the relationship between the amount of active drug substance and the number of comorbidities, with the potential for drug interactions are analyzed using a linear regression. The results show that the most widely prescribed antihypertension research is in the singular (35.47%) of amlodipine (46.37%), about 51.27%) of the recipes contain a potential for drug interaction, with the potential drug inter actions at the level of the most significant (45.78%). The results of linear regression analysis showing the relationship between the amount of active drug sub stances and the number of potential drug interactions can be denoted by the equation y = 0.386 x- 0.385, but the relationship is very weak because of the pearson correlation (r) 0.491. The relationship between the number of comorbiditie and potential drug interactions can be denoted by the equation y = 0.416 x + 0.806, but the relationship is very weak because of the pearson correlation (r) 0.06.Key words: drug interaction, hypertension, geriatric, outpatient, PKU Muhammadiyah

Abstract

Sindrom Koroner Akut mengacu pada sekelompok kondisi yang mencakup ST Segment Elevation Myocardial Infarction (STEMI), Non ST Segment Elevation Myocardial Infarction (NSTEMI) dan Unstable Angina Pectoris (UAP). NSTEMI didefinisikan sebagai peningkatan biomarker jantung dengan tidak adanya segmen elevasi ST yang persisten. NSTEMI dan UAP memiliki gejala serupa dimana perbedaan NSTEMI dengan UAP adalah pada NSTEMI terjadi peningkatan troponin. Dengan diketahuinya data karakteristik pasien NSTEMI dan UAP, pencegahan terjadinya komplikasi bisa diminimalkan, mengurangi faktor risiko yang bisa memperburuk prognosis pasien dan meningkatkan kualitas hidup. Penelitian ini bertujuan untuk mengetahui karakteristik pasien NSTEMI dan UAP yang dirawat inap di Rumah Sakit PKU Muhammadiyah Gamping Periode 1 Januari 2018 -31 Desember 2020. Metode penelitian yang digunakan adalah cross sectional menggunakan data retrospektif dengan jenis penelitian observasional analitik. Pengambilan sampel dilakukan dengan menggunakan purposive sampling dan didapatkan sampel sebanyak 224 pasien yang menjalani rawat inap. Jumlah tersebut sudah memenuhi kriteria menurut OpenEpi dimana jumlah minimal adalah 164 pasien. Data yang diperoleh dianalisis dengan IBM SPSS versi 20. Analisis chi square digunakan untuk melihat perbandingan antara variabel bebas antara lain riwayat komorbid diabetes mellitus, riwayat komorbid hipertensi, riwayat komorbid dislipidemia dengan variabel terikat yaitu lama rawat inap pada pasien NSTEMI dan UAP. Penderita NSTEMI dan UAP terbanyak terdiri dari pasien yang berusia > 45 tahun. Riwayat komorbid terbanyak adalah hipertensi. Pemberian terapi terbanyak di UGD mencakup oksigen 81.3 %, infus NaCl 0.9 % 62.1 %, clopidogrel 75 mg 57.6 %, aspilet 80 mg 57.1 %, inviclot 49.6 % dan ISDN 5 mg 25.0 %. Pemberian terapi terbanyak di rawat inap adalah clopidogrel 75 mg 90.2 %, aspilet 80 mg 87.9 %, atorvastatin 40 mg 74.6 %, alprazolam 0.5 mg 70.1 %, bisoprolol 2.5 mg 45.1 %, dan furosemide injeksi 17.9 %. Pasien NSTEMI dan UAP yang mempunyai komorbiditas diabetes mellitus berpeluang 0.41 kali untuk menjalani perawatan kurang / sama dengan 5 hari jika dibandingkan dengan yang tidak punya komorbiditas diabetes mellitus, dan secara statistik bermakna (P = 0.02).

Abstract

Bullying in the school environment is an emerging issue that needs to be taken care of by various parties. It will cause a negative impact on the future of both the perpetrator and the victim. Children may grow up with unstable emotions and have low empathy for others. The purpose of this activity is to introduce basic emotions and anti-bullying knowledge as an effort to increase empathetic awareness in the community in preventing bullying. Prospective comparative study with an observation and interview was carried out as the methodology, teachers, and parents. The research sample was selected using a purposive sampling method. Based on observations, it was found that there are behaviors that indicate bullying in the school environment. In addition, students were also found to have poor empathy and emotional management. The activity of introducing basic emotions and anti-bullying behavior utilized posters, mini games, and attaching stickers. This activity is targeted for grade 5 elementary school students because they are already able to recognize their emotions, and their experience will be an example for other students in another grade. Students, teachers, and parents who have been interviewed stated that this emotional recognition and anti-bullying activity was excellent and had a positive impact on children's emotional development. Students increasingly understand the meaning of basic emotions and anti-bullying, recognize self-emotions, know how to express emotions, increase empathy, and create a more comfortable school environment.
