Abstract

Abstrak             Pengobatan yang diperoleh pasien asma tidak selamanya dapat memulihkan kondisi asma yang dialami. Biaya pengobatan yang dapat dikatakan tidak murah menyebabkan tidak semua pasien asma dapat menjangkau biaya pengobatan, sehingga pasien mengalami keterbatasan dalam beraktivitas serta meningkatnya frekuensi kekambuhan yang akan mempengaruhi kualitas hidup pasien asma. Penelitian ini bertujuan untuk mengetahui gambaran kualitas hidup pasien asma dan pengaruh terapi yang digunakan terhadap kualitas hidup pasien di BP4 unit Minggiran Yogyakarta. Sampel penelitian ini adalah pasien dewasa dengan diagnosa asma tanpa penyakit penyerta. Metode penelitian ini menggunakan metode cross sectional yang bersifat observatif, dari hasil wawancara serta pengisian Asthma Quality of Life Quessioner (AQLQ). Analisis data meliputi data kualitas hidup, terapi obat yang digunakan dan faktor-faktor yang mempengaruhi kualitas hidup pasien asma. Hasil analisis kualitas hidup diolah dalam bentuk persentase dan disajikan dalam bentuk tabel. Hasil penelitian pada pasien dewasa di BP4 unit Minggiran Yogyakarta menunjukkan, sekitar 82,2% pasien memiliki kualitas hidup baik dan 17,8% pasien dengan kualitas hidup kurang baik. Berdasarkan hasil uji chi-square terapi antiasma yang digunakan berpengaruh terhadap kualitas hidup pasien asma. Kata kunci: asma, kualitas hidup,  chi-square, BP4 unit Minggiran Yogyakarta Abstract Obtained treatment of asthma patients are not always able to restore the condition of asthma is experienced. The cost of treatment that can be said is not cheap cause not all asthma patients can reach medical expenses, so that patients have limitations in activity and an increase in the frequency of recurrence that will affect the quality of life of asthma patients. This study aims to describe the quality of life of asthma patients and the effect of therapy that is used for quality of life patients asthma in BP4 unit Minggiran Yogyakarta. Samples were adult patients with a diagnosis of asthma without concomitant diseases. This research method using cross sectional observational, from interviews as well as charging the Asthma Quality of Life Quessioner (AQLQ). Data analysis included the data quality of life, therapeutic drugs used and the factors that affect the quality of life of asthma patients. The results of the analysis of the quality of life in the form of a percentage processed and presented in tabular form. The results of the study in adult, BP4 unit Minggiran Yogyakarta shows, approximately 82.2% of patients have a good quality of life and 17.8% of patients with a poor quality of life. Based on the results of chi-square test antiasma therapy is used to influence the quality of life of patients asthma.  Key words: asthma, quality of life, BP4 Minggiran unit Yogyakarta, logistic regression.

Abstract

Pasien gagal jantung merupakan salah satu kelompok pasien yang memiliki risiko tinggi mengalami hiponatremia. Selain masalah diagnosis, permasalahan terapi juga banyak dijumpai dalam penanganan hiponatremia. Di tengah minimnya hasil uji klinis terapi konvensional untuk penanganan hiponatremia, hasil uji klinis obat-obat golongan antagonis reseptor arginine vasopressin atau yang dikenal dengan golongan vaptan membawa harapan baru untuk penanganan hiponatremia pada pasien gagal jantung. Tulisan ini merupakan hasil telaah pustaka terhadap artikel-artikel hasil penelitian dan pengkajian yang relevan yang ditelusuri dengan menggunakan kata kunci heart failure, hyponatremia, argininevasopressin, arginine-vasopressin receptor antagonist dan vaptan. Hasil uji klinis golongan vaptan pada pasien gagal jantung menunjukkan bahwa obat-obat tersebut secara efektif meningkatkan kadar natrium dalam darah dengan efek samping ringan yang umumnya dapat ditoleransi. Berdasarkan hasil uji klinis, conivaptan dan tolvaptan disetujui oleh Food and Drug Administration di Amerika untuk digunakan dalam penanganan hiponatremia pada pasien gagal jantung. Namun, banyak pakar mengajukan pertanyaan kritis terhadap hasil uji klinis obat-obat golongan vaptan ini dan masih meragukan penggunaannya untuk penanganan hiponatremia. Tulisan ini mengulas secara ringkas tentang hiponatremia pada pasien gagal jantung dan peran hormon arginine vasopressin (AVP), aspek farmakologi dan hasil uji klinis beberapa obat golongan vaptan, serta beberapa pertanyaan yang masih tersisa dari hasil uji klinis obat-obat tersebut.Kata kunci: Antagonis arginine vasopressin, arginine vasopressin, gagal jantung, hiponatremia, vaptanArginine-Vasopressin Receptor Antagonists: A New Hope for Treatment of Hyponatremia in Patients with Heart FailureHyponatremia is an important problem in patients with heart failure. It has also been known as a predictor both of short and long-term clinical outcome of heart failure patients. While evidences of clinical efficacy and safety of conventional options for treatment of hyponatremia resulted from clinical trials are very limited, several clinical trials have been conducted to evaluate the efficacy and safety of arginine vasopressin antagonists or also known as vaptan group. This paper reviews hyponatremia in patients with heart failure and the important roles of arginine vasopressin in its development, pharmacological aspects and clinical trials outcomes of some vaptans and several questions emerged from those clinical trials. Relevant research papers, reviews, and website information have been searched and included in this review using keyword of heart failure, hyponatremia, arginine-vasopressin, arginine-vasopressin receptor antagonist and vaptan. Generally, clinical trials have highlighted the efficacy and safety of vaptan groups in the management of hypervolemic hyponatremia in patients with heart failure. Those drugs can effectively increase serum sodium level with tolerable adverse reactions. Based on those clinical data two drugs of vaptan group, conivaptan and tolvaptan, have been approved by Food and Drug Administration in The United States to be used for the treatment of euvolemic and hypervolemic hyponatremia. However, many experts are still hesitant to recommend vaptans as treatment of choice for hyponatremia due to the lack of patients-focused outcomes measurements.Keywords: Arginine vasopressin antagonist, heart failure, hyponatremia, vaptan

Abstract

ABSTRACTUrinary tract infections (UTIs) are the most common bacterial infection and account forsignificant morbidity and health care cost. It was well accepted that data of antibiotic utilization fortreatment of infectious diseases are needed in developing antibiotic use policy. Antibiotic utilizationstudy can provide useful information for improvement of the appropriate and effective antibiotic use.This research aims to compare antibiotic utilization for treatment of hospitalized patients withurinary tract infections at two hospitals in Yogyakarta based on drug utilization 90% (DU90%)profile. Data of antibiotic prescription were obtained from patient’s medical record for patientswhich were hospitalized during 2005 at hospital A (650 beds) and B (250 beds). Quantity ofantibiotics utilization was calculated as the number of defined daily dose (DDD) per 100 bed days(BD) based on ATC/DDD system released by WHO Collaborating Centre for Drug StatisticsMethodology. The antibiotics were ranked by percentage of DDD/100BD and antibiotics thataccounted for 90% (DU90%) were determined. Total of antibiotics use for hospitalized patients withUTIs at hospital A amounts to 65.77 DDD/100 BD included 14 classes, consisted of 24 agents ofantibiotic. Total antibiotics use at hospital B amounts to 58.22 DDD/100 BD included 9 classes,consisted of 13 agents of antibiotic. Ceftriaxone was the greatest antibiotic used at both hospitals.There were 9 items of antibiotics within DU90% at hospital A and 7 item of antibiotics at hospital B.There was a difference in the utilization of antibiotics for hospitalized patients with UTIs at twohospitals in Yogyakarta based on total number of antibiotics and number of antibiotics withinDU90% segment.Keywords : Antibiotic, ATC/DDD, DU90%, Urinary Tract Infections

Abstract

ABSTRACTA tissue culture and plant generation for the ceplukan, Physalis angulata, L has beendeveloped. Callus were efficiently induced on Murashige and Skoog (MS) medium supplementedwith 0,1 ppm indole-3-acetic acid (IAA) and variation of 1, 2, 3, 4, and 5 ppm benzyladenine (BA).The regenerable callus differentiated into shoots. Phytochemical containing of the shoots weredetected and compared with the origin plant using Thin Layer Chromatography (TLC). Flavonoidwas showed by bright blue spot under UV 366 nm and became yellow after smoked by NH3 withHRf 43 and 69. Saponin was showed by yellow spot with HRf 18 and purple spot with HRf 45 aftersprayed by anisaldehyde-sulfuric acid. HRf of the shoots from culture and origin plant were thesame, showed that shoots contains the same phytochemical as the origin plant.Keyword: shoots, tissue culture, restoration, flavonoid, saponin

Abstract

ABSTRACTThis research was aimed at knowing potential drug interactions on drug prescribing fortreatment of hospitalized pediatric patients with respiratory tract infection. Research was carriedout at a private hospital in Surakarta and data were collected from medical records of patients werehospitalized during January – December 2005. Potential drug interactions were determinedtheoretically using some literatures on drug interaction. From total 186 patients included in thisresearch, 67.2% were 0-4 years old with common cold was the most diagnosed. There were 52.7%patients receiving 4-6 item of drugs and 76.9% patients were hospitalized during 1-3 days.Potential drug interactions were identified in 30.7% patients with 1.8% and 15.8% of them at level 1and 2 clinical significance, respectively. Statistically, there was a relationship between potentialdrug interactions and length of hospitalization (p = 0.001). Some further investigations(prospectively) are needed to ensure results from this research.Key words : drug interaction, respiratory tract infection, pediatric

Abstract

Obat antiinflamasi non-steroid (AINS) banyak digunakan dalam pengobatan berbagai penyakit yang melibatkan proses inflamasi dan merupakan grup terbesar dari agen farmasetik yang digunakan secara luas di seluruh dunia. Beberapa studi telah melaporkan bahwa komplikasi pada saluran pencernaan merupakan reaksi obat yang tidak dikehendaki (ROTD) yang paling sering terjadi terkait penggunaan AINS. Penelitian ini dilakukan dengan tujuan untuk mengetahui kuantitas dan kualitas peresepan obat golongan AINS untuk pasien rawat jalan di salah satu rumah sakit. Penelitian dilakukan dengan menggunakan sampel data peresepan obat selama satu tahun, yaitu selama periode 1 Juli 2006 sampai 30 Juni 2007. Data penggunaan obat golongan AINSdikumpulkan dari catatan penggunaan obat di instalasi farmasi rumah sakit meliputi jenis obat, bentuk sediaan, kekuatan, serta jumlah penggunaan. Pengukuran kuantitas penggunaan obat AINS dilakukan dengan menggunakan satuan unit DDD (Defined Daily Dose) yang dinyatakan dalam DDD/1000 kunjungan pasien rawat jalan (KPRJ). Kualitas peresepan obat AINS diukurberdasarkan kriteria keamanan relatif obat AINS terhadap saluran pencernaan yang dianalisis berdasarkan data kuantitas penggunaan obat AINS menggunakan metode Drug Utilization 90% (DU90%). Hasil penelitian menunjukkan bahwa keseluruhan AINS yang digunakan meliputi 7 jenis dengan 22 nama dagang. Kuantitas AINS yang digunakan adalah sebesar 182,5 DDD/1000kunjungan pasien, yang artinya adalah setiap pasien mendapatkan AINS sebesar 1,8 DDD. Jenis AINS yang termasuk dalam segmen DU90% adalah asam mefenamat (62,8%), ketorolak (15,4%), ketoprofen (8,6%), dan diklofenak (8,5%). Berdasarkan tingkat keamanan obat golongan AINS terhadap saluran pencernaan, kualitas peresepan obat golongan AINS untuk pasien rawat jalan di rumah sakit ini sudah cukup baik, karena AINSs yang digunakan sebagian besar merupakan AINS dengan risiko moderat dan rendah terhadap gangguan saluran pencernaan, yakni asam mefenamat dan diklofenak dengan persentase penggunaannya sebesar 71,3%.

Abstract

Evaluation of antibiotics use plays an important role in antimicrobial stewardship programs to support the rational use of antibiotics at all healthcare facilities. This study aimed to describe the profile of drug utilization 90% (DU90%) of antibiotics use for hospitalized patients at a private teaching hospital in Yogyakarta over period of 2013 – 2017. Aggregate data of all systemic antibiotics having an anatomic therapeutic chemical (ATC) code and used for hospitalized patients during 2013 – 2017 were included in this study. Quantity of antibiotics use was expressed in defined daily dose (DDD) and final quantity was expressed in DDD/100 bed days. DU90% profile was obtained by calculating the cumulative use of antibiotics from the highest to the lowest percentage. There were 44 antibiotic agents used each year over the period of 2013 – 2017. However, there were only 10 – 12 agents within the DU90% segment. Cephalosporins group was identified as the most used antibiotics consistently over the period and the top three antibiotics that were consistently prescribed were: ceftriaxone, levofloxacin, and cefixime. It can be concluded that only about a quarter of the total antibiotic agents used in hospitals are within the DU90% segment and those antibiotic agents are relatively consistent, especially for the top three of the highest used antibiotics.

Abstract

Perilaku pencarian pertolongan kesehatan yang utama atau paling banyak dilakukan jika penduduk sakit adalah melakukan pengobatan mandiri / swa medikasi (self medication) dengan membeli obat di warung, toko swalayan dan apotek. Masyarakat hanya mengandalkan iklan, referensi keluarga, teman, tetangga yang tidak memiliki dasar pendidikan kefarmasian sebagai panduan dalam menggunakan obat. Hal tersebut potensial menimbulkan permasalahan di bidang kesehatan. Metode kegiatan didasarkan pada model promosi kesehatan, melalui pemberian edukasi dan pelatihan kader terpilih tentang penggunaan obat secara rasional. Kegitan ini melibatkan ibu-ibu PKK dusun Candi 3, Desa Sardonoharjo, Sleman. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan ibu-ibu PKK di dusun Candi 3 dan terbentuknya kader bola salju yang akan membantu masyarakat dalam menggunakan obat secara tepat dan rasional.

Abstract

Mahkota dewa (Phaleria macrocarpa Boerl.) was used empirically for treatment of any kind of diseases, including cancer. The aim of this research is to explore the carcinogenesis inhibition effect of mahkota dewa epicarp extract. The effect was examined using 7,12-dimethylbenz(a)anthracene (DMBA) induced new born mice which Were divided into five groups. All groups received 0.25 mg single intraperitoneal injection of DMBA, except group of normal control. The extract was tested onto three different groups of the mice at the age of 28 days which Was separated from their parental with the doses of 6.25 mg, 12.5 mg, and 25 mg for 16 Weeks and one group Was used as negative control. Anticarcinogenic effect of the extract was evaluated after 16 Weeks by observing the histopathology and total number of all types of neoplasm in hepar, kidney, lung, gastric, intestine, and lymphatic tissue. The result showed that the extract at 6.25 mg, 12.5 mg, and 25 mg decreases carcinogenesis event by 50%, 83%, and 100% , respectively, compared with negative control. In conclusion, the dose of 25 mg extract improved several organs carcinogenesis than other dose, and thereby can be developed as a potential anticancer agent.

Abstract

Increasing age can cause a variety of degenerative disease, one of which is hypertension. One issue is the treatment of geriatric polypharmacy the drug interactions may pose clinical problems that require a careful monitoring and action, for example the use of antihypertensives. This study aims to determine the picture of drug use and the relationship between the number of medication and comorbidities with potential drug interactions. This study is an observational analytic study using cross sectional study design. Data is retrospectively collected by using affordable population that meet the criteria. The determination of the relationship between the amount of active drug substance and the number of comorbidities, with the potential for drug interactions are analyzed using a linear regression. The results show that the most widely prescribed antihypertension research is in the singular (35.47%) of amlodipine (46.37%), about 51.27%) of the recipes contain a potential for drug interaction, with the potential drug inter actions at the level of the most significant (45.78%). The results of linear regression analysis showing the relationship between the amount of active drug sub stances and the number of potential drug interactions can be denoted by the equation y = 0.386 x- 0.385, but the relationship is very weak because of the pearson correlation (r) 0.491. The relationship between the number of comorbiditie and potential drug interactions can be denoted by the equation y = 0.416 x + 0.806, but the relationship is very weak because of the pearson correlation (r) 0.06.Key words: drug interaction, hypertension, geriatric, outpatient, PKU Muhammadiyah
