Abstract

Background: Mutations within the hepatitis B virus (HBV) reverse transcriptase (RT) gene have been associated with drug resistance against nucleos(t)ide analogs (NAs). Objective: This study aimed to identify mutations in the RT gene among patients with chronic hepatitis B (CHB) before receiving antiviral therapy and its relationship with the HBV genotypes. Methods: A total of 26 HBV DNA was extracted from the blood plasma of CHB patients. HBV RT gene was amplified and sequenced using the Sanger dideoxy sequencing method. The HBV genotype was determined through phylogenetic analysis using the Maximum Likelihood method. Results: The study subjects comprised 14 CHB patients without complications and 12 CHB patients with cirrhosis/hepatoma. CHB patients with cirrhosis/hepatoma were older than those without complications. The HBV genotypes comprised 15 (57.7%) genotype C and 11 (42.3%) genotype B. All treatment-naïve CHB patients did not demonstrate any classical NA resistance mutations within the RT gene. However, several putative and pretreatment resistance mutations, including F221Y, N238H, and V224I, were high frequency in more than 40% of study subjects. In addition, F221Y and N238H/Q mutations were frequently observed in genotype B, while V224 I was only found in patients infected with genotype C (p=0.000). Conclusions: There was no evidence of classical RT gene mutations associated with NA resistance in treatment-naïve patients with CHB. However, several putative and pretreatment mutations were identified as genotype-specific mutations and may contribute to antiviral resistance against NAs.

Abstract

Hirschsprung disease (HD) is a congenital anomaly due to intestinal aganglionosis starting from the internal anal sphincter and spreading to the proximal area with varies extension of the segment. Definitive treatment for this abnormality is to perform surgery to remove the aganglionic bowel and can be carried out in one-step and multi-step procedure. In the process of wound healing after surgery, it involves inflammatory mediators as a marker of whether there is a stress response from the endocrine system such as neutrophil-lymphocyte ratio (NLR). This study aims to compare NLR as an inflammatory biomarker in one-step and multi-step procedure of hirschsprung disease patients at Arifin Achmad Hospital. The design of this study was cross-sectional using medical records and using total sampling for number of samples. Age, gender, and method of surgery were evaluated by univariate and bivariate to determine whether there were statistical differences. Seven HD patient taken from Mei-October at Arifin Achmad Hospital taken for evaluated. The results showed that the most samples were in < 1-month-old (42.9%) and male (71,4%) for gender. The average number of NLR was in 7.33 after one-step procedure and 2.77 for multi-step procedure. This number of NLR both not statistically different neither for the post-operative examination nor the comparison between procedure.

Abstract

Stigma towards People with Mental Disorders (ODGJ) is a serious problem in Indonesia. One of the efforts to address stigma against ODGJ involves promotional and preventive activities as part of a psychosocial approach, as outlined in Law No. 18 of 2014 on Mental Health. One of its goals is to eliminate stigma, discrimination, and human rights violations against people with mental disorders (ODGJ). In the digital era, android smartphones serve as communication tools and easily accessible sources of information due to their time, effort, and cost efficiency. The purpose of this research is to understand community attitudes towards the development of Android-Based Mental Health Intervention Media in addressing community stigma towards People with Mental Disorders (ODGJ) and to assess the stigma present in society towards ODGJ. This research is a descriptive quantitative study using purposive sampling on 100 community respondents. The study was conducted by distributing questionnaires on community attitudes towards the development of intervention media and questionnaires to assess community stigma towards ODGJ. The data was analysed descriptively and analytically. The results showed that the majority of the community stigma towards ODGJ remains low, at 59% (59 respondents), and community attitudes towards the development of intervention media applications are positive, at 62% (62 respondents). It is recommended that, with the positive attitude of the community towards the development of educational media to combat stigma against ODGJ, it can further reduce societal stigma towards ODGJ.
