Abstract

The training activities are based on the staff of medical records in this Anduring Clinic, in terms of data processing and making reports still using a computer manually, namely making letters, typing and making reports using Microsoft Excel, while in terms of computerized data processing there is no or still being done manually, so the computers in this clinic are only used as far as typing and reporting. The purpose of this activity is carried out to improve medical record staff in their skills and expertise, in processing and processing patient data, doctor data, disease data and patient history data and making reports desired by the Anduring clinic, so that the data can be processed and made in a manner quickly and precisely to those who need information. This activity was carried out for one day with 9 participants, including general staff and doctors. The method used is training accompanied by lectures, demonstrations, hands-on practice in front of the computer. The results of the training showed that the medical record staff was able to use and practice the Medical Record Data Compuerization (KDRM) application program in terms of data processing and medical record reporting in the clinic. With this training, it shows that there is a significant influence in training computerized medical record data on improving performance and time efficiency in the processing of data and medical record reports at the clinic.

Abstract

According to PMK No. 24 of 2022, a medical record is a document containing data on patient identity, examination, treatment, actions, and other services that have been provided to the patient. Through this policy, health service facilities are required to run an electronic patient medical history recording system. The transition process is carried out until no later than 31 December 2023. This type of research uses quantitative methods. Quantitative research methods are used to determine the picture of hospital readiness. The method used to analyze hospital readiness is DOQ-IT with indicators of organizational culture readiness, governance and leadership readiness, human resource readiness, and infrastructure readiness. Based on the results of the research that has been carried out, information can be obtained that in the aspects of human resources, organizational work culture, leadership governance, and infrastructure as a whole can be seen from the answers of 18 ready respondents (46.06%) and very ready as many as 20 people (49.87%) then with the respondent's answer not ready as many as 1 person (2.22%) the answer is very unprepared as many as 1 person (0.35%) and for the answer hesitant as many as 1 person (1.44%), it is confirmed from the informant that the preparation for the implementation of electronic medical records is ready to be implemented.

Abstract

Data in health services is stored in the form of patient health records, which is a collection of various data elements in health records that are used to compare, combine, and present patient conditions by doctors and for health services. From the results of data processing, the most grouping of diagnoses based on ICD-10 is Gastroenteritis and colitic of unspecified with code A09.9 as many as 33 patients. Then the grouping based on the ICD-10 chapter is mostly in Chapter I (Certain Infectious and Paracitic Disease) with a percentage of 22.71% or 129 patients. The most grouping based on procedure was Tonsillectomy with ICD-9CM code 28.2 as many as 14 patients. The most grouping based on the DTD code and its definition was DTD 270.9 (Abnormal Clinical And Laboratory Symptoms, Signs and Findings, YTK elsewhere) as many as 35 patients. The grouping based on Table Format RL4a was the most in the 45-64 year age group as many as 143 patients with 71 female patients and 72 male patients. Grouping by address, the most sub- districts in Solok District/City are Kubung District with 111 patients. There were 554 patients alive with disposition and 14 patients died. After processing the data, statistical processing is carried out with the following results: the results of the BOR obtained are 60.55%. BOR has reached efficiency figures because the efficiency level of BOR is 60-85%. Then the BTR result is 9 times. BTR is not efficient because the calculation of BTR is done once a year with an efficiency level of 40- 50 times. For the results of TI is 2.71 days. TI efficiency is 1-3 days.

The Hospital Management Information System (SIMRS) has become a key requirement in improving the efficiency of administration and medical services, one of which is implemented at the HB. Saanin Mental Hospital through the Sijiwa SIMRS to overcome the constraints of manual medical record management. This study aims to evaluate the extent to which the implementation of the Sijiwa SIMRS improves the efficiency of medical record management. The methods used are quantitative and qualitative with a comparative approach through document analysis, surveys, and interviews, with data analysis in the form of descriptive statistics, Paired Sample T-Test, and thematic analysis. The results of the study indicate an increase in efficiency, including a reduction in patient registration time from 6 minutes 35 seconds to 5 minutes 2 seconds, file search time from 1 minute 18 seconds to 29 seconds, the number of staff reduced from 4 to 2 people while still being able to serve hundreds of patients, and file availability increased from 85% to 100%. Data input errors decreased from 10 to 1 document, although diagnostic coding errors were still found in 3–4 documents. Respondents expressed satisfaction because the system speeds up work and minimizes errors, but challenges such as network disruptions and feature limitations remain. This study recommends improving infrastructure and human resource competencies to optimize and enhance the benefits of SIMRS.
