Abstract

The microbial flora of the stomach and upper small intestine was studied in 21 malnourished Indonesian children with diarrhoea and compared with that found in 21 Australian children of Caucasian extraction with a variety of alimentary disorders. In the Indonesian children there was marked microbial contamination of the upper gastrointestinal tract with a wide variety of microorganisms. In several patients there was significant overgrowth by Candida sp. and in several others large numbers of anaerobic bacteria were isolated. It is suggested that these microbiological abnormalities are related to the pathogenesis of diarrhoea in children with malnutrition.

Abstract

Forty one infants suffering from diarrhoea hospitalized in the Department of Child Health, Medical School, University of lndonesia/Dr. Tjipto Mangunkusumo General Hospital, Jakarta, from August 15, 1973 to January 15, 1974 were refed with "free lactose milk".The result is as follows :1. From the point of view of increase of body weight:27 (65.8 %) out of 41 cases were excellent.2 ( 4.9 %) out of 41 cases were good.12 (29.3 %) out of 41 cases were poor.2. From the point of view of stopping the diarrhoea :25 (60.9 %) out of 41 cases were excellent.10 (24.5 %) out of 41 was good.6 (14.6 %) out of 41 cases were poor.

Abstract

Recent advances in the Indonesian Paediatric Gastroenterology have been presented in the 4 following headings and conclusions :(1)'ROSE' system as the principle of treatment of diarrhoea, which is proven to be effective and gives an excellent result in reducing the case fatality rate of acute gastroenteritis, especially cholera.(2) MCT and low lactose containing formula in low birth weight infants gives good to excellent results in improving fat malabsoprtion, ceasing the diarrhoea and increase of body weight.(3} The preliminary study in malnourished Indonesian children has shown that the paediatric enterotest duodenal capsule can be used to study the upper intestinal microflora. It is useful in studying the human intestinal microflora in places where radiologic examination is not available and can easily be used in field studies.(4) The presesent report is the first study to document duovirus shown electron microscopically as virus particles in faecal specimens from children in Indonesia where malnutrition and diarrhoea remain important problems.

Abstract

Fifty patients hospitalized with severe gastroenteritis - dehydration with acidosis were investigated. The age average was 8 months with the range of 1-18 months. The amount of the Ringer's lactate solution given was as follows:1st hour : 30 ml/kg b.w.the following 7 hours: 70 ml/kg b.w. or 10 ml/kg b.w./hour.After 8 hours on parenteral treatment, the child was given oral solution, either glucose-electrolyte solution or milk formula in ¼ dilution. The mortality rate was 6% (3 out of 50 patients) i.e. one due to seizures suspected encephalitis, one due to potasium deficiency (K = 2.1 mEq/l) and the third due to dehydration itself because the child was admitted already in a moribund stage (subvinum vitae).

Abstract

Diarrheal diseases still constitute one of the major causes of morbidity and mortality in Indonesian children. The ertiology is multi-complex as in other developing countries. In 1974 a National Seminar on Rehydration was held in Jakarta and a rehydration program started with a standardized treatment of Ringer's Lactate and oral rehydration (ROSE SYSTEM). As a result the case fatality rate of diarrheal diseases has been dramatically reduced for for gastroenteritis and cholera.
