Abstract

Background: The measurement of Hospital National Quality Indicators (NQI) is mandatory for improving healthcare services in hospitals according to Minister of Health Regulation Number 30 of 2022. However, the achievement of NQI has not been optimal and is not in line with the hospital's accreditation status. Purpose: To determine the implementation of the NQI measurement policy on hospital accreditation status achievement using the RE-AIM framework. Method: Evaluative research was conducted at 6 selected hospitals in Indonesia representing the western, central, and eastern regions. Primary data collection was done through in-depth interviews, while secondary data were obtained from document searches. Data analysis used content analysis techniques according to the RE-AIM framework. Results: The total number of accredited hospitals up to December 2023 was 2,884 hospitals (95.6%), yet the average compliance rate for reporting National Quality Indicators in 2023 was only 69%. Utilizing the RE-AIM approach on six hospitals in Indonesia, only one hospital consistently reported NQIs, while the rest could only achieve targets of 5-8 NQIs. The NQI measurement policy proves effective in achieving accreditation goals and enhancing service quality; however, NQI measurement in these six hospitals remains suboptimal due to challenges such as leadership commitment. Nonetheless, NQI measurement shows promise with the establishment of Hospital Quality Committees/Teams and NQI reporting through the SIMAR application. The sustainability of NQI measurement in hospitals is pursued through socialization, training, and enhancing monitoring and evaluation processes. Conclusion: The implementation of NQI measurement policies in six hospitals is good but needs to be improved for routine reporting compliance and NQI target achievement. The NQI measurement policy has a positive impact on achieving accreditation goals. Suggestion: The government, through the Ministry of Health, needs to optimize advocacy for NQI measurement, implement rewards and punishments, and develop NQI reporting applications to be more effective and efficient.

Abstract

Accidents are unexpected, undesirable events and can cause material and life losses. Accident prevention must be carried out based on the control hierarchy and the root causes of the accident. Purpose: The aim of this research is to know the determinants of work accidents among fabrication workers in the steel structure workshop area at PT X in 2024. The research method uses a cross sectional design. The sample was 58 respondents from a population of 112 workers at PT Data were collected using a questionnaire. The data was tested using the chi square test. The research results showed that of 58 respondents, 26 (44.8%) had experienced work accidents. Variables are age (P=0.126), education level (P=1.000), length of service (P= 0.126), length of work (p=0.029; OR 11.143), OHS knowledge (P=0.290), use of PPE (P=0.003; OR=5.971). There is a significant relationship between length of work and use of PPE and the incidence of work accidents among fabrication workers in the steel structure workshop area. The conclusion is that there is a relationship between compliance with the use of PPE and length of work with work accidents, while the factors age, education, length of service, OHS knowledge have no relationship with the incidence of work accidents. Suggestions are to regulate employee working hours so that there is no overtime in work progress and to increase compliance with the use of PPE for fabrication workers in the steel structure workshop area of PT.

Abstract

Musculoskeletal disorders (MSDs) are a major contributor to disability worldwide, being the leading cause of disability in 160 countries. A review of 9,482 workers in 12 regions found that MSDs were the leading impact on workers in 16%. To analyze the most important factors on the risk of MSDs in construction workers of PT. Aluphi Hijau Lumina at the Dhoho Kediri International Airport Project in 2024. Analytical observational research with a cross-sectional study approach. aims to see the relationship between independent variables and dependent variables, namely individual characteristics (age, length of service, BMI/body mass index, smoking), ergonomic factors such as work posture (Total Back Exposure Static, Total Back Exposure Moving, Shpulder Arm Exposure, Wrist & Hand Exposure, Neck Exposure, Driving Exposure, Vibration Exposure, Work Pace Exposure, & Stress Exposure), workload, and duration of work time on complaints of musculoskeletal disorders (MSDs). The results of the study showed no relationship between individual characteristics (Age, Length of Service, BMI and Smoking Status) with MSDs complaints in Construction workers with P Value of age = 0.110; length of service = 0.597; BMI = 0.228; smoking status = 0.451. There was no relationship between ergonomic factors of body posture (Total Back Exposure Static, Total Back Exposure Moving, Total Wrist Hand Exposure, Total Driving Exposure, Total Vibration Exposure, Total Work Pace Exposure, Total Stress Exposure) & duration of work of construction workers with MSDs complaints in Construction workers of PT. Lumina Green Aluphi with p value Total Back Exposure Static = 0.960; Total Back Exposure Moving = 0.286, Total Wrist Hand Exposure = 0.344; Total Driving Exposure = 0.512; Total Vibration Exposure= 1,000, Total Work Pace Exposure= 1,000, Total Stress Exposure= 0.316. There is a significant relationship between Total Shoulder Arm, Total Neck Exposure, & Workload of construction workers with MSDs complaints in construction workers of PT. Aluphi Hijau Lumina with Total Shoulder Arm p value= 0.005, Total Neck Exposure p value= 0.040, & Workload p value= 0.005. The most influential variable on MSDs complaints is neck exposure with a P value of 0.026, OR = 6.9. The risk of experiencing pain complaints in construction workers' activities with neck positions is 7 times more at risk of experiencing MSDs complaints. There is a need to be modified of work equipment with a working position with a bent neck condition, which can be done with engineering control on changes to work stations or work methods or setting the time for work exposure with an awkward position that is too long.
