Abstract

Kepala ruang merupakan manajer tingkat bawah/lini yang memegang peranan cukup penting dalam menentukan keberhasilan pelayanan keperawatan melalui fungsi manajemen pelayanan. Fungsi pengarahan merupakan merupakan fungsi yang paling fundamental dalam manajemen keperawatan karena berdampak dalam meningkatkan atau menurunkan kepuasan kerja perawat pelaksana yang pada akhirnya akan sangat berpengaruh pada mutu pelayanan di rumah sakit. Penelitian ini menggunakan metode kualitatif dengan pendekatan fenomenologi untuk mengekplorasi berbagai pengalaman kepala ruangan dalam menjalani fungsi pengarahan di Rumah Sakit .Pengumpulan data yang dilakukan terhadap 7 orang partisipan dengan wawancara mendalam di tiga rumah sakit berbeda di Banjarmasin. Wawancara direkam kemudian dibuat dalam bentuk transkrip verbatim. Hasil penelitian ini menunjukkan bahwa pelaksanaan fungsi pengarahan yang dilakukan oleh kepala ruangan terdiri atas pendelegasian dalam fungsi pengarahan yang dilakukan secara berjenjang dengan memperhatikan aspek legal; pemberian motivasi dengan menyeimbangkan pemberian reward dan punishment serta tidak melupakan motivasi relligi; supervisi dengan mempertimbangkan mekanisme, tujuan, sasaran dan model supervisi; Mengatasi konflik dengan belajar dari pengalaman sebelumnya; melaksanakan komunikasi dan koordinasi dalam melakukan pengarahan. Hasil studi ini diharapkan perawat manager dapat berkolaborasi baik dengan institusi pendidikan maupun rumah sakit dalam hal pendidikan kepala ruangan baik secara formal dan informal.

Abstract

One of the effects of diabetes brought on by poor venous and arterial blood circulation is diabetic foot injury (LKD). Buerger Allen exercise is one modality of therapy that uses active postural movements to enhance blood flow in the lower extremities venous and arterial blood vessels. The brachial index ankle (ABI) technique was used to evaluate improved foot circulation. This study sought to assess the impact of Buerger Allen exercise on LKD patients' lower circulatory extremities. A quasi-experiment with a pretest and post-test without a control strategy was used as the research design. 43 respondents made up the 43 samples for this study, which were collected using successive sampling methods. Using bivariate data analysis, Wilcoxon tests were utilized to compare values before and after the Buerger Allen exercise. The findings demonstrated a statistically significant relationship between the average ABI in LKD patients before and after 0.85 (P-value = 0.000). The Buerger Allen exercise intervention is performed twice daily for three weeks. Positional changes and gravitational force variations can assist in emptying and filling blood vessels. At the same time, contraction of the gastrocnemius muscle and the plantar muscles as a muscle pump activates the ship and prevents backflow. Regular exercise can widen collateral circulation channels, boosting the supply of oxygen to peripheral tissues and the distribution of nutrients. Exercise helps increase circulation, according to studies by Buerger Allen. Therefore it can be suggested in the future that nurses teach patients with lower extremity circulatory disorders in LKD patients.

Abstract

According to Banjarmasin City Health Office data, hypertension is still ranked among the top 10 non-communicable diseases. The increase in hypertension can be associated with unhealthy lifestyle changes. One way to make it easier for patients to control themselves, especially blood pressure control and adherence to self-management, is through health promotion programs. Therefore, researchers are interested in conducting heat promotion efforts with the "DIARII" program for the prevention and control of hypertension patients in Banjarmasin. The purpose of this study was to determine the effect of implementing the "DIARII" program in increasing the ability of patients and families to control hypertension independently. The research design used in this study was Quasi-Experimental Design with a Control Group Pre-Post Test Design. The population and samples used were 60 people with hypertension aged 26-65 in Banjarmasin. The research was conducted in several places in the city of Banjarmasin. This research has been deemed ethical by the Research Ethics Commission of the University of Muhammadiyah Banjarmasin, with the number 283/UMB/KE/IX/2022. The results showed that the value of the Wilcoxon test for family independence in the intervention group showed the importance of Sig. p Value 0.317 > 0.05 means no significant difference after the intervention. So it can be concluded that implementing the DIARII program affects the patient's ability to manage hypertension. Health promotion education interventions were found to improve the quality of life and the ability of sufferers to understand any health information they received.

Abstract

Mistakes made when administering intravenous drugs to patients can jeopardize their safety. According to the PMKP report, there were three cases of medication administration errors in one private hospital in Banjarmasin City in 2018, four cases in 2019, two cases in 2020, and one in 2021. This study aimed to see how nurses' characteristics and career paths affect the application of the six correct principles of intravenous drug administration. This study was conducted using a cross-sectional research model. Thirty-five samples were taken through simple and cluster random sampling techniques. Observation sheets were used to collect data. For multivariate data analysis, frequency distribution and multiple linear regression were used. The results showed that nurses applied the six correct principles of intravenous drug administration in the excellent category as many as 23 respondents (65.7%), and the results of multiple linear regression tests obtained the results of a significant influence between career level variables on the application of the six correct principles of intravenous drug administration with a p-value of 0.033 (>0.05). This p-value indicates that the relationship between career level and applying the six correct principles of intravenous drug administration is statistically significant. At the same time, there was no significant influence between nurse characteristics, including age variables, gender, and education level, where the p-value successively on the age variable was 9.437, 0.605, 0.742, where the value was >0.05. The modeling results on each variable, namely career level and nurse characteristics, were simultaneously influenced by 0.481 (48%) in applying the six correct principles of intravenous drug administration. This means that career level and nurse characteristics explain 48% of the variation in applying these principles. Nurses have a graded career path from PK I to PK V, with higher levels of experience and knowledge that contribute to the application of nursing care. Nurses are also expected to have various characteristics that enable them to carry out and cooperate in the implementation of the six correct principles of intravenous drug administration, especially double-checking, asking for allergies to drugs, patient complaints before and after drug administration, checking drug expiration dates, patient identity, recording patient complaints, and patient reasons for refusing drugs.
