Abstract

Pediatric acute appendicitis is common; however, it is different from torsion ovarian cysts. The estimated incidence of ovarian torsion in children is 5 per 100,000 girls. Herein, we report two cases of ovarian torsion mimicking pediatric acute appendicitis mimicking ovarian cyst. In the first case, a girl who was being assessed for acute appendicitis was identified to have torsion of the ovarian cyst during the surgery. In the second case, a girl experienced a ruptured ovarian cyst but then got severe appendicitis following the surgical procedure. The simultaneous occurrence of appendicitis and ovarian cysts is rare. These two patients' acute gastrointestinal complaints and symptoms exhibit similarities and may coincide. Both patients underwent salpingo-oopherectomy and appendectomy. Both patients were healthy on hospital discharge in a healthy condition. Female patients with severe abdominal pain and amenorrhea should be evaluated for ovarian torsion. Diagnosing the condition may be challenging due to non-specific clinical signs.

Abstract

Background: During pregnancy, a woman's immune cells undergo cellular and functional alterations, rendering them more vulnerable to illness. Preeclampsia and HELLP syndromes are pregnancy problems and one of Indonesia's leading causes of maternal mortality; COVID-19 infection would exacerbate the situation. The evidence for COVID-19 vertical transmission is not well established, and the report on the incidence of vertical transmission, especially in Indonesia, is still limited.Case presentation: We discuss the case of a 26-year-old, 36 weeks of gestation, with preeclampsia and HELLP syndrome, who was infected with COVID-19 and got severe pneumonia and ARDS. The infant was born by cesarean section, and there was no contact between the infant and mother following delivery. Based on the results of the PCR examination, the baby was also confirmed to have COVID-19 without any symptoms. Conclusion: Vertical transmission during pregnancy is possible in women with a severe type of COVID-19 with preeclampsia and HELLP syndrome.

Abstract

Objective : The aim of this study to give clinical characteristic of ovarian cysts in adolescent patient and comprehend the delineation of the management protocol implemented in patients diagnosed with ovarian cysts Methods : This study used a retrospective method conducted by reviewing patient medical records. This study was conducted by collecting medical record data retrospectively for 5 years at PKU Muhammadiyah Gamping Hospital and Panembahan Senopati Bantul Hospital. All data of outpatients, and inpatients diagnosed with ovarian cysts from January 2018 to July 2023 aged 11-21 years were taken to be included in the study. Results: A total of 56 adolescent patients were identified with ovarian cysts. Researchers found most patients with adolescent age group 18-21 years (n=43). Patients presented with a chief complaint of abdominal pain and the cyst was unilateral. Conclusion :. The patients exhibited a primary concern of pain, and the cyst was found to be located on one side only. Efforts to save the ovary were deemed vital owing to the relatively low probability of cyst malignancy. In this study also found that there were no correlation between clinical symtopms (abdoment pain, nausea and menstrual disorder) with size of cyst. Abstract

Background. Maternal mortality is still high. The case that we will report below is an example of cases that can be happened in the hospital and can be ended with maternal mortality. Case presentation. A 30 years old disabled primigravida with ileus post- Caesarean Section day 5 was managed by relaparotomy and partial resection of caecum and ileum due to intestinal perforation, and die on the fifth days after relaparotomy due to sepsis. A-33 years old secundigravida with history of relaparotomy on 4 days after caesarean delivery in the first pregnancy due to ileus, was performed re-caesarean section due to unprogressed labor. Twelve hours after the operation the abdomen becomes distended and can be managed successfully by conservative early diagnosis management. A 29 years old primigravida has performed caesarean section with history of myomectomy 3 years ago. Twenty four hours after Caesarean Section, the abdomen becomes distended and early conservative management was done, but at 72 hours after the operation, there was intestinal perforation and performed colostomy. Conclusion. Acute colonic pseudoobstructive after caesarean section is a rare case, and delayed diagnosis and management increase the morbidity and mortality.
