Abstract

Research conducted at PT Fahifa Prima Mandiri in the production area, with the aim of identifying potential hazards and risks of work accidents, analyzing potential hazards and risks, as well as efforts to control and prevent occupational accident risks using the HIRARC (hazard identification, risk assessment, and risk assessment) method. controls). The data used in this study is to use primary data and secondary data. Data analysis and discussion using qualitative and descriptive data. The results of the analysis of the potential hazard analysis found as many as 30 potential hazards including 18 low risk, 5 moderate risk, 5 high risk, and 2 extreme risks. proposed controls to reduce the possibility of work accidents that occur are the addition of protective covers on grinding machines, adding foot switches on bandsaw, milling, and lathe machines, carrying out 5R activities, using PPE such as (safety shoes, safety gloves, safety glasses, and respirators or masks), adding fire extinguishers and first aid kits

Abstract

CV Jasa Bhakti merupakan perusahaan industri manufaktur yang memproduksi mesin pertanian dan spare part otomotif. Selama kegiatan proses produksi berlangsung mesin bubut SY-GF 2500H memiliki kendala yaitu kerusakan mesin yang mengakibatkan mesin produksi tidak dapat beroperasi dan menyebabkan terjadinya downtime yang dapat menimbulkan kerugian dan mempengaruhi tingkat efektivitas pada mesin bubut SY-GF 2500H. Metode Overall Equipment Effectiveness (OEE) digunakan untuk mengukur tingkat efektivitas pada suatu mesin dengan menggunakan tiga komponen utama yaitu availability rate, performance rate, dan quality rate untuk mengidentifikasi jenis kerugian berdasarkan six big losses, rata-rata hasil OEE sebesar 75% masih di bawah standar world class dengan faktor yang mempengaruhi rendahnya OEE yaitu performance rate sebesar 79%, sedangkan losses yang mempengaruhi nilai efektivitas performance rate yaitu idle and minor stoppage sebesar 6% dan reduce speed losses sebesar 35%. Dengan melakukan analisis akar penyebab masalah melalui fishbone diagram, dan five whys analysis, dari metode tersebut dapat disimpulkan, penyebab rendahnya nilai efektivitas pada performance rate yang dipengaruhi oleh idle and minor stoppage dan reduce speed losses yaitu faktor manusia, mesin, material, dan metode.

Abstract

CV Jasa Bhakti is a manufacturing industry company that produces agricultural machinery and automotivespare parts. During the production process, the SY-GF 2500H lathe has problems, namely engine damagewhich causes the production machine to not operate and causes downtime which can cause losses andaffect the effectiveness of the SY-GF 2500H lathe. The Overall Equipment Effectiveness (OEE) method isused to measure the level of effectiveness on a machine by using three main components, namelyavailability rate, performance rate, and quality rate to identify types of losses based on six big losses, theaverage OEE result of 75% is still below the standard world class with factors that affect the low OEE,namely the performance rate of 79%, while the losses that affect the effectiveness value of the performancerate are idle and minor stoppage of 6% and reduce speed losses of 35%. By analyzing the root causes ofthe problem through a fishbone diagram, and five whys analysis. From this method, it can be concludedthat the causes of the low effectiveness value on the performance rate which are influenced by idle andminor stoppage and reduced speed losses are human factors, machines, materials, and methods.
