Abstract

Diabetic ulcers is a chronic complications of diabetic mellitus which caused people treated in hospital about 80%. Patients with diabetic ulcers need more care and additional costs. It will be wretched if the infection caused by multidrugs resistant bacterials like Methicillin resistant Staphylococcus aureus (MRSA). This research aimed to identified and understand the resistance of MRSA bacteria from Wagner grade I and II diabetic ulcers at department of Internal Medicine of RSUD Arifin Achmad. Samples were taken with a swab technique and then the bacteria were identified and further resistance test with Kirby-bauer disk diffusion method. This research reported about 6 samples (21,42%) from 28 grade I and II swabs diabetic ulcers are Staphylococcus aureus and 5 samples 83,33% from 6 samples Staphylococcus aureus were identified as MRSA. Antibiotics trimethoprim-sulfamethoxazole, chloramphenicol and gentamycin resistance test results had sensitivity 100% to MRSA bacteria. Followed by ofloxacin 60% and ciprofloxacin 40%.Key words : Diabetes Mellitus, Diabetic ulcers, Methicillin resistant Staphylococcus aureus, Resistance

Abstract

Constipation is a condition often found in pregnant women and can lead to various complications. Based onpathophysiology, constipation can be classified into constipation due to structural abnormalities and functionalconstipation. Constipation in pregnant women is classified as the functional constipation caused by hormonal factors,changes in diet, growth of fetus, decrease in physical activities and the position of the defecation. The management ofconstipation during pregnancy take precedence on non pharmacological therapy. Pharmacological therapy (laxatives)that can be used is the bulk-forming agents and osmotic laxatives, however, they are only when absolutely necessaryand are not for long term use. The reasons are to avoid dehydration and electrolyte imbalance in pregnant women.

Abstract

Diabetic ulcers are open wound on the skin surface effect of neuropathy andvascular insufficiency that made the wound are often not felt by the patient , and candevelop into an infection caused by aerobic and anaerobic bacteria . Diabetic ulcersmedical patients are very susceptible infected with antibiotic-resistant bacteria likeESBL -producing bacteria. This descriptive study to detect Gram-negative bacillibacteria producing enzyme Extended spectrum beta - lactamase ( ESBL ) in diabeticulcers, are carried out using a double disc approximation Method ( DAM ) to thenumber of study subjects were 26 people with diabetic ulcers Wagner grade I and II .The results obtained ESBL producing bacteria as much as 16 samples ( 69.57 % )some of them were Citrobacter freundi , Enterobacter sp , Proteus sp , Klebsiella sprespectively – each of them are 3 isolates ( 18.75 % ) , and Pseudomonas sp ,Salmonella sp respectively – each of them are 2 isolates ( 12.5 %) .The results of theantibiotic sensitivity test showed that Gram-negative bacilli bacteria producing ESBLhave high sensitivity toward amikacin and meropenem as much as 16 samples ( 100% ) Antibiotics Betalaktam groups such as ampicillin, and third -generationcephalosporins such as cefotaxcime , ceftriaxone, and ceftazidime factually hadexperienced resistance 100% toward ESBL producing bacteria.Keywordrs : diabetic ulcers , Double disc aproximation Method ( DAM ) , Gram -negative bacilli bacteria , Extended spectrum beta - lactamase ( ESBL )

Abstract

Chronic renal failure is an end stage of renal disease that can be treated by dialysis, which one of them is Continuous Ambulatory Peritoneal Dialysis (CAPD). CAPD is a dialysis technique uses peritoneal membrane as dialysis membrane that splits between dyalisate in peritoneal membrane with blood plasm in peritoneal vessels. CAPD as a theraphy for chronic renal failure patients can be examined by KDQOL-SFTM questionnaire to measures its effect on patients’ quality of life. This research helds on purpose to get a representation of Continuous Ambulatory Peritoneal Dialysis (CAPD) patients’ life quality. This is a descriptive method of research with cross sectional approachment and had implemented on February 2014. About 47 chronic renal failure patients have been undergoing CAPD therapy. Research results showed most of respondents is dominated by male respondents (59,57%). Results from respondent by age category indicated that category 18-44 years old and category 45-64 years old are relatively equal ini amount. Category by ethnic showed that most of respondents are Malay ethnic (31,91%). Respondents have been educated and got undergraduated and degree: D1/D2/D3/S1 (53,19%). Most of respondents have been married (93,61%) and work as a housewife, retired and unemployed (36,17%). CAPD has been used as theraphy for about 1-5 years by most respondents (72,34%). Abstract

Tujuan: untuk mengetahui prevalensi dan karakteristik responden breakthrough infection pasca vaksinasi dosis 2 Sinovac. Metode: penelitian deskriptif observasional dengan pendekatan cross sectional, pengambilan data dilakukan menggunakan metode consecutive sampling dengan besar sampel adalah 120 orang. Hasil: menunjukkan bahwa sebanyak 11 orang (9,2%) tenaga kesehatan di RSUD Arifin Achmad Provinsi Riau terkonfirmasi mengalami breakthrough infection. Mayoritas subjek penelitian yang mengalami breakthrough infection adalah laki-laki (63,6%), pada kelompok usia 40-49 tahun (36,4%), berprofesi sebagai dokter (40%), kelompok obesitas (81,8%), tanpa penyakit komorbid (54,5%), mempunyai riwayat kontak erat (63,6%), dan belum pernah terinfeksi COVID-19 sebelum vaksinasi (90,9%). Sebagian besar kasus merupakan kasus asimtomatis/tanpa gejala sebanyak 6 orang (54,5%), dan terdapat 5 orang (45,5%) yang memiliki gejala yaitu pada 3 orang (27,3%) dengan gejala sedang, dan pada 2 orang (18,2%) dengan gejala ringan. Kesimpulan: kejadian breakthrough infection pasca vaksinasi Sinovac dosis dua pada tenaga kesehatan di RSUD Arifin Achmad Provinsi Riau tidak ditemukan adanya kasus dengan gejala berat.

Abstract

Tujuan: untuk mengetahui gambaran KIPI vaksin booster mRNA heterolog pada tenaga kesehatan di RSUD Arifin Achmad Provinsi Riau. Metode: penelitian deskriptif observasional, pengambilan data dilakukan menggunakan metode consecutive sampling dengan besar sampel adalah 117 orang. Hasil: menunjukkan bahwa mayoritas subjek penelitian berjenis kelamin perempuan (70,9%), pada kelompok usia 30-45 tahun (66,7%), dengan IMT overweight (46,2%), tanpa komorbid (71,8%), bukan penyintas (86,3%), dan memiliki riwayat kontak erat (54,7%).  Keluhan yang muncul terbanyak pada setiap kategori terdiri dari, keluhan umum berupa demam/meriang (82,9%), keluhan lokal/area penyuntikan berupa bengkak di daerah penyuntikan (55,6%), keluhan muskuloskeletal/otot berupa nyeri otot (88,9%), keluhan pencernaan berupa penurunan/peningkatan nafsu makan (24,8%), keluhan psikis berupa sering ngantuk/tidur lebih lama (23.9%), keluhan saraf berupa kebas pada tangan dan/atau kaki (11,1%), keluhan pada kepala/telinga/hidung/tenggorokan berupa hidung tersumbat (14,5%), dan keluhan pada jantung dan pembuluh darah dan pernapasan berupa batuk (8,5%). Kesimpulan: tenaga Kesehatan di RSUD Arifin Achmad Provinsi Riau mengalami Kejadian Ikutan Pasca Imunisasi (KIPI) yang terbagi kedalam beberapa kelompok.

Abstract

Background: Remdesivir is one of the pharmacological therapies for moderate COVID-19 patients. Remdesivir is an antiviral that can increase the risk of nephrotoxicity. Objective: The study aims to evaluate the incidence of acute kidney injury (AKI) in COVID-19 patients receiving remdesivir. Methods: This is a cross-sectional analytic study of 238 COVID-19 patients receiving remdesivir therapy at Awal Bros Panam Hospital for the period 2020–2022. AKI was diagnosed using the KDIGO criteria. The chi-square test was used to determine the correlation of AKI incidence with age, gender, and comorbidities, with a significance level of p < 0.05. Results: AKI was found in 32 patients (13.4%). The majority of AKI patients (75%) were aged 60 years or older, with 65.6% being male. Additionally, 84.4% of these patients had comorbidities, and 53.1% of them died. Around half of the patients were diagnosed with stage 1 AKI, and the majority, specifically 84.4%, did not undergo renal replacement therapy (RRT). The association between the incidence of AKI and age and comorbidities was shown to be statistically significant (p=0.000; RR 5.11; 95% CI 2.44-10.8 and p=0.009; RR 3.05; 95% CI 1.22-7.64, respectively). Conclusion: The main risk factors for AKI are primarily observed in the older population and individuals with several medical conditions. Greater emphasis should be placed on administering remdesivir to COVID-19 patients who are elderly and have comorbidities, as they are at a higher risk of developing AKI.

Abstract

Elderly people are one of the susceptible communities that should not be neglected because of their vulnerability to suffering diseases due to the aging process. It needs contributions from family cooperation, cadres and health workers’ care, and local government support. This study aimed to elaborate on determinant factors affecting older people’s quality of life among suburban elderly in Riau Province, Indonesia. There were 42 elderly being interviewed using OPQoL and other questionnaires. Data was examined using SPSS and Smart PLS. Independent variables were age, gender, level of education, marital status, and type of housemate. level of income, smoking habit, exercise activity, body mass index, abdominal circumference, knowledge score, level of health perception, health security, and each score consisting of activity of daily living (Barthel index), depression status (Geriatric Depression Scale), cognitive status (Abbreviated Mental Test and Mini Mental State Examination), fall risk (Hendrich and Morse Fall Scale), mini nutritional assessment, family support, quality of life using OPQoL and SF-36 questionnaires. Only four variables acting as determinant factors to older people's quality of life i.e health security (p 0.004), health perception (0.006), AMT score (0.031) and social function score (p 0.016) proved by a formula using linear regression (p 0.007). It is concluded that health security, health perception and AMT score will in-crease OPQoL score, however, the social function score will decrease it.

Abstract

Glomerulonephritis (GN) is a heterogeneous disorder characterized by glomerular compartment damage of the nephron mediated by autoimmunity. Secondary GN can occur for the patients with systemic autoimmune disease and has higher prevalence of peripheral artery disease. Symptoms and signs of GN are characterized by periorbital oedema accompanied by leg oedema, massive proteinuria, decreased renal function, hypertension, hematuria and erythrocyte casts. We reported a 40 years old female patient found with anasarca oedema, pain in the right foot, and proteinuria who had a history of systemic lupus erythematosus for 7 years. On physical examination, we found hypertensive urgency, anasarca oedem and blackish ulcus on the dorsal pedis extremity dextra. Laboratory data showed hypoalbuminemia, hypernatremia, creatinine clearance 19.38 ml/min, and albumin creatinine ratio 18.949 mg albumin/gr cr, ANA profile arthritis + 4.2. Urynalysis showed massive proteinuria, erythrocytes and casts were present. CT angiography showed the impression of peripheral artery disease.

Abstract

The aging process in the elderly is characterized by a decline in physical and psychological abilities as well as an increased risk of disease that can lead to death. The high proportion of morbidity and disability in the elderly group can result in an increase in the need for health services. One form of health service for the elderly is the elderly posyandu..The implementation of posyandu activities for the elderly is carried out by health workers and cadres. Cadres often encounter obstacles in implementing elderly posyandu activities, one of which is the lack of access to information regarding guidelines for properly serving 5 elderly posyandu tables. This service aims to provide guidance and training on how to properly serve 5 posyandu tables for the elderly according to national guidelines. The community service activities carried out have shown results that meet the success criteria. After the training, cadres showed increased knowledge about elderly health and had a positive attitude about the need for elderly posyandu. Cadres are also more skilled in carrying on examining the elderly’s health condition
