Abstract

Disorders resulting from body iron overload (hemosiderosis in reticgoendothelial cells or accumulation within parenchymal cells of the liver, heart and other organs) may octal- in patients who have undergone frequent blood transfusions or under other excessive iron therapy to treat the anemia. Therefore, every iron preparation especially by repeated parenteral route or blood transfusions must be accompanied by monitoring of body iron status. In the present study the most useful indirect measurement of ,body iron status, included serum ferritin (SF), serum iron (SI), total iron binding capacity (TIBC), transferin saturation (TS) and the erythrocyte, were studied in 34 women anemic patients at the Dr. Sardjito General Hospital. The results of the study are as follows: among 30 subjects with anemia caused by other than iron deficiency, included chronic renal diseases, inflammation or infection, hematologic and other organ malignancy, idiopathic thrombocytopenic purpura, the mean of SF level increased to 210,85ng/ml; but SI level (66,941g/d1) and TIBC (271, 07/µg/dl) showed no useful information about the state of iron stores, because its large biologic and analytic variability. In overall there was normal percentage of TS (25,58%), but among them 7 cases showed evidence of blood loss with TS less than normal, and erythrocyte indices within normal range. In 4 patients with uncomplicated iron deficiency anemia the mean of SF and 1S were low (4,62ng/ml and 11,85% respectively), although SI only slightly decreased (38,7/tig/d1), and the TIBC was normal (340,3ffig/d1). The blood smears showed variation of size in the erythrocyte, with many smaller than normal and pale RBCs.It is concluded that ferritin serum measurement was the most sensitive and non invasive indicator for early stage of iron deficiency assessment. However, in fact no single indicator or, combination of indicators is ideal for the evaluation of body iron status in all clinical circumstances, because each indicator may be by other conditions such as infections, inflammations, liver, chronic, renal disease and malignancy.Key Words: anemia - serum ferritin - serum iron - total iron binding capacity - transferin saturation

Abstract

Background: Reticulocyte production index (IPR) is used to determine the classification of functional anemia. More specific laboratory parameters are used in managing patient. There were some inappropriate of IPR correlation with the diagnosis of anemia in clinical practice. Objectives: The study was designed to know normal hematocrit in Sardjito General Hospital, to determine IPR in various anemia and to compare with standard IPR and final diagnosis in anemia. Methods: Observational study was done in patients with anemia who whole blood examination in uderwent Clinical Pathology Laboratory on January 2001 until May 2002. Reticulocyte and hematocrit examination were performed before treatment. Patients were followed until. The diagnosis of anemia was established based on the clinical manifestations and laboratory results. Normal mean calculated hematocrit was based on age and sex. IPR was calculated based on standard hematocrit (45%) and mean normal hema-tocrit.Results: The normal mean hematocrit in male, female, children and infant were 43%, 39%, 40% and 48%, respectively. There was significant difference between the 2 formulas. The concordance of IPR determination in hipoproliferative anemia and maturation defects were 100%, in hemolytic/hemorrhagic anemia and maturation defect was 23,5%. The IPR of hypoproliferative anemia and maturation defect anemia differed significantly with haemolytic/ hemorrhagic anemia. IPR determination using factual hematpcrit show a better correlation with classification of functional than using standard hematocrit of 45%.Conclusion: IPR determination based on normal calculated hematocrit mean gave more benefit in increasing the correlation with classification of functional anemia. IPR value showed inappropriate result in hemolytic/hemorhagic anemia probably due to comorbidities.Key words: IPR - functional clasification anemial - anemia - normal calculated hematocrit

Abstract

Reticulocyte Production Index (RPI) is a shift correction for the presence of reticulocyte that can assesses the real erythropoietic response of the bone marrow. RPI can be used to classify anemia as result from functional defect, i.e. RPI<2 for hypoproliferative and maturation defect anemia, and RPI>3 for hemolytic anemia. Measurement of Red Blood Cell Distribution Width (RDW) is the mathematical representation of variance within the size distribution of the red cell population. The objective of this study was to identify the variation of the red cell size within the population measured (anisocytosis) enable to determine the cell population as homogeneous (normal RDW) or heterogenous (increased or high RDW). The role of RDW to differentiate functional defect anemia with RPI<2 has not been studied. This study was done at Hematology Department Dr. Sardjito Hospital. In such a cross sectional study, the subjects were considered as anemic patients based on automated laboratory analyzer, peripheral blood smear, and reticulocyte count result. The result showed that seventy patients met the inclusion criteria. Sixty seven patients (95,71%) had RPI<2 classified as hypoproliferative and maturation defect anemia, 2 patients (2,85%) had RPI = 2-3 classified as borderline group and 1 patient (1,42%) had RPI>3 classified as hemolytic anemia. Means for reticulocyte count, corrected reticulocyte count, RPI, and RDW for hypoproliferative and maturation defect anemia were 2,8% and 2,3%; 1,1% and 1,1%; 0,4 and 0,5; 21,8% and 17,8% respectively, with p value by the Wilcoxon Signed Ranks Test are = 0,322; 0,52; 0,273; and 0,27 (CI = 95%). The proportion of anisocytosis and normocytosis for hypoproliferative anemia were 76,08% and 23,9% and for maturation defect anemia were 76,19% and 23,8% with Chi-Square test 0,000 and p = 0,993. No correlation between was observed RDW and RPI<2 with Spearman correlation (r = 0,0297 and p = 0,993). It is concluded that the use of RDW and RPI is not always useful to differentiate hypoproliferative and maturation defect anemia on the group of anemia categorized as RPI<2 without support from another laboratory test especially in combined anemia.

Abstract

Timber supply is diminishing in addition to the trend to lower quality but higher cost of wood production manufacturing. However, because the demand of this material continues, an alternative or substitution on the use of timber, such as bamboo laminated, is desirable. Having susceptible character to powder by beetles attack, bamboo must be passed through preservation process before it is applied as laminating materials, but chemical preservative materials used in industrial process bring negative impacts to the environment. One of human and environmental friendly preservatives is the tobacco extract. Nevertheless, the most effective concentration of tobacco extract and its influence to the adhesive materials have not been well recognized. This research objective was to investigate the influence of tobacco extract to the natural and mechanic behaviour of laminated bamboo. Dried tobacco flake was mixed with water in variable concentration of 100, 125, 150 and 175 gram/liter. Bamboo split were given treatment in boiling water with tobacco extract solutions. As benchmarking, bamboo split also preserved with boiling water (without preservatives materials) and 5% concentration of borax. Non preservative bamboo was used as the control. The result of experiment showed that tobacco extract concentration of 150 gram/liter caused 61,33% insect mortality and 1,87% decreased weigh, which is effective to be used as preservative in compare to other three concentrations. Water content and density observation showed that preservation using 150 gram/liter extract tobacco comparing to non preservative boiling preservation, produced smaller value of water content and larger value of density than borax preservative. Preservation using 150 gram/liter tobacco extract produced highest value of compression parallel to grain and modulus of the bamboo elasticity Petung mechanic properties were 277,95 MPa and 170,34 MPa, respectively. Mechanical properties of laminated bamboo that were significantly influenced by preservative variation showed that highest values for compression parallel to grain and modulus of elasticity were 66,09 MPa and 127,12 MPa, respectively and reached by preservation using 100 gram/liter tobacco extract.

Abstract

Indonesia as a developing country is in the urgent need to improve its arbitration law and practice. One of the reasons is because Indonesia may gain many advantages by such improvement, such as: increase of its international reputation as a safe place to invest or conduct trading since there is an assurance that future disputes may be promptly solved through arbitration. One way to improve Indonesia arbitration practice is by creating the jurisdiction as a friendly place to arbitrate, which firstly shall be analysed by reviewing its drawbacks as a preferable seat of arbitration, specifically on the procedure to enforce arbitral awards and the respective judicial supports to conduct arbitration within the jurisdiction.

Abstract

The chronic infections such as periodontitis that enters the body will activate various inflammatory mediators like TNFα IL5 and IL6. This will cause changes in the metabolism of fatty acids in the liver as well, as the expected increase in total cholesterol, triglycerides and LDL and lower HDL levels. These changes in lipid metabolism and atherosclerosis lesions so will increase the risk of coronary heart disease risk. The purpose of this study was to prove the effects of long exposure to lipopolysaccharide of P. gingivalis as induction periodontitis on cholesterol levels in Wistar rats.32 rats will be divided into three groups. Group 1 mice induced a LPS for 28 days while the group 2 induced a LPS for 60 days and the negative groups is a group of mice that were not treated. We are giving induction 3 times a week. Giving induction performed insivius intrasulkular on the teeth of the lower jaw then the sample will be identified on day 28 and 60. We will taking blood from the heart to measure total cholesterol levels. Total cholesterol measurement was conducted by elektrospektrofotometerik. Data were analyzed using ANOVA oneway and will be followed by post hoc multiple comparasion.Based on the statistical result showed that total cholesterol levels increased significantly in Wistar rats that had been induced by LPS P.Gingivalis, α < 0,05.Long exposure to LPS P.Gingivalis can increase total cholesterol levels inmale Wistar rats, but still need to do further research to see the effect of LPS from other bacteria on cholesterol levels of mice with the different experience.
