Abstract

East Nusa Tenggara Province is the second-highest province with the percentage of pregnant women having HBsAg reactive (5,26%) and has the lowest screening coverage (4,55%). Increasing the mother’s screening coverage is necessary, followed by increasing access to therapy and early infant detection. A positive attitude should exist to increase the mother’s behavior for assessing triple elimination testing, thus can reduce the transmission risk of Hepatitis B, Syphilis, and HIV-AIDS to the infants. This research analyzed the relationship between knowledge, family support, and information frequency with the mother’s triple elimination testing attitude. This research was an analytical study using the cross-sectional method, conducted at four primary health centres in Kupang city in 2020. The sample selection used a stratified random sampling method, with 110 pregnant women from the four PHC chosen. Data analysis used Chi-square. Results showed only 36,7% of respondents had good triple elimination knowledge, and 33,9% have insufficient knowledge. Most respondents (64%) have a neutral perception, and 27% have a positive perception of family (husband’s) support. 79% of respondents receive scarce information about triple elimination. Respondent’s attitude primarily neutral (61%). However, 23% still have a negative attitude towards triple elimination testing. There is a significant relationship between perception of family support and attitude towards triple elimination testing (p < 0.001). The mother having a positive perception of their husband’s support, also have a positive attitude toward triple elimination testing. Knowledge level and information frequency showed no significant relationship (p = 0.941 and p = 0.093) with mother’s attitude.

Abstract

Osteoartritis merupakan penyakit degeneratif kronis pada sendi yang diderita oleh 240 juta orang di seluruh dunia, termasuk di Indonesia. Penyakit ini menyebabkan nyeri dan kaku pada sendi, sehingga terjadi hambatan fisik dalam mobilitas sehari-hari. Osteoartritis lutut secara keseluruhan adalah yang paling umum dengan 83% kasus yang meningkat setiap tahun. Salah satu faktor risiko terjadinya osteoartritis adalah obesitas dan dikaitkan sebagai penyebab perkembangan osteoartritis lutut. Tujuan untuk mengetahui hubungan antara obesitas dengan kejadian osteoartritis lutut di Kota Kupang. Penelitian ini merupakan penelitian observasional analitik dengan menggunakan pendekatan cross sectional. Pengambilan sampel menggunakan teknik purposive sampling dan didapatkan 97 sampel. Data dalam penelitian ini dianalisis secara univariat dan bivariat dengan menggunakan uji Chi-Square. Terdapat hubungan yang signifikan antara obesitas dengan kejadian osteoartritis lutut di Kota Kupang dengan didapatkan nilai p = <0,001 (p < 0,05). Terdapat hubungan antara obesitas dengan kejadian osteoartritis lutut di Kota Kupang. Temuan ini memiliki implikasi penting bagi upaya pencegahan dan penatalaksanaan osteoartritis lutut, terutama melalui pengendalian berat badan dengan edukasi gizi seimbang dan aktivitas fisik teratur yang perlu diprioritaskan dalam program kesehatan masyarakat, khususnya pada kelompok usia lanjut. Tenaga kesehatan di puskesmas perlu melakukan skrining IMT secara rutin pada pasien osteoartritis untuk deteksi dini dan intervensi obesitas, sementara kebijakan kesehatan perlu mengintegrasikan pengelolaan obesitas sebagai bagian dari strategi komprehensif pencegahan osteoartritis lutut mengingat prevalensi obesitas yang terus meningkat di Kota Kupang. Penelitian selanjutnya disarankan untuk menganalisis faktor-faktor lain seperti aktivitas fisik, riwayat cedera, dan faktor genetik, serta menggunakan desain kohort untuk melihat hubungan sebab-akibat secara lebih kuat.

Abstract

Tuberculosis (TB) is a public health problem that imposes a substantial economic burden, particularly among patients with Human Immunodeficiency Virus (HIV) coinfection. This study aimed to analyze the differences in economic burden between TB patients with HIV and TB patients without HIV in Kupang City. This study employed an analytic observational design with a case–control approach. A total of 64 respondents were included, consisting of 34 TB–HIV patients as the case group and 34 TB patients without HIV as the control group, selected through purposive sampling based on inclusion and exclusion criteria. The economic burden analyzed included direct non-medical costs, indirect costs, and the occurrence of catastrophic costs. The results showed differences in the total economic burden between TB patients and TB–HIV patients in Kupang City. TB–HIV patients tended to bear a higher economic burden than TB patients without HIV. These differences were mainly driven by higher direct non-medical costs and indirect costs, particularly income loss, asset sales, and indebtedness during the treatment period. In addition, the proportion of patients experiencing catastrophic costs was higher in the TB–HIV group than in the TB group. The study demonstrated a statistically significant difference in the total economic burden between the two groups. The median direct non-medical cost among TB–HIV patients was IDR 113,500.00, which was higher than that among TB patients without HIV (IDR 17,500.00; p < 0.001). The median indirect cost among TB–HIV patients was IDR 2,450,000.00, whereas it was IDR 0.00 among TB patients without HIV (p < 0.001). A total of 23.5% of TB–HIV patients experienced catastrophic costs, while none were observed among TB patients without HIV, with a significant association between HIV status and the occurrence of catastrophic costs (p < 0.003).
