Abstract

Background: Stroke is a major health problem in many parts of the world and is increasing in prevalence in line with modernization. Damage to brain cells after stroke cause cognitive function disability that affect the quality of life and independence of the patient. Decline in cognitive function relates to size and location of the lesion. This research aims to find out the differences in cognitive function loss between right and left hemisphere ischemic stroke patients. Methods: This research is observational analytic study with retrospective cohort design performed in an Outpatient Neurology Clinic at RSUD Dr. Moewardi. There are 30 patients as subject consists of 14 right hemisphere and 16 left hemisphere ischemic stroke patients. Sampling techniques is exhaustive sampling. Cognitive function of patients with ischemic stroke was measured using Indonesian version of MoCA (Montreal Cognitive Assessment) questionnaires. The data collected are presented in tabular form and analyzed using the multiple linear regression analysis through SPSS 20.00 program for Windows. Results: The location of lesions influence cognitive function score significantly (p = 0,027) and there is a difference in which patients with left hemisphere lesion score lower cognitive function than patients with right hemisphere lesion with regression coefficient (β =-4.604). Age is a confounding variable on the relationship between lesion location with cognitive function. Age has negative effect on cognitive function, where the increase of age correlates with the declining cognitive function (p = 0.040; β = 0.228-). Conclusions: The decline of cognitive function in left hemisphere ischemic stroke patients is more severe than right hemisphere ischemic stroke patients. Abstract

Coronavirus Disease 2019 (COVID-19) merupakan penyakit baru yang terjadi akibat infeksi virus Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Manifestasi klinis umumnya berupa gangguan sistem respirasi. Namun, COVID-19 juga dapat bermanifestasi sebagai gejala ekstraparu, salah satunya dermatologi. Mekanisme pasti terjadinya manifestasi kulit pada COVID-19 masih belum dipahami secara penuh, namun diduga disebabkan oleh efek langsung virus terhadap sel keratinosit, respon imun berlebihan, maupun karena obat-obatan yang digunakan untuk tatalaksana COVID-19. Manifestasi kulit dari COVID-19 dapat dibagi menjadi lima kelompok lesi, yaitu lesi pseudo-chilblains, vesikuler, urtikaria, makulopapular eritematosa, dan vaskular. Lesi makulopapular eritematosa (44,18%) dan chilblains (19,72%) merupakan lesi yang paling sering ditemukan. Lesi vesikuler dan vaskular lebih jarang ditemukan. Lesi chilblains biasa ditemukan sebelum atau bersamaan dengan timbulnya gejala COVID-19 yang lain, dan seringkali muncul pada pasien kontak erat atau asimptomatik sehingga pengenalan lesi secara dini dapat digunakan untuk mendeteksi adanya COVID-19.

Abstract

Coronavirus Disease 2019 (COVID-19) is a new disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection. Common clinical manifestations are respiratory system disorders. However, COVID-19 can also manifest as extrapulmonary symptoms, one of which is dermatology. The exact mechanism of COVID-19 cutaneous manifestations is still not fully understood, but is suspected to be caused by the direct effects of the virus on keratinocyte, excessive immune response, or drugs used to treat COVID-19. Cutaneous manifestations of COVID-19 can be divided into five groups, namely pseudo-chilblains, vesicular, urticaria, erythematous maculopapular, and vascular lesions. Erythematous maculopapular (44.18%) and chilblains (19.72%) are the most common lesions. Vesicular and vascular lesions are less common. Chilblains lesions are usually found before or together with the onset of other COVID-19 symptoms, and often appear in close contact or asymptomatic patients, so early recognition of it can be used to detect the presence of COVID-19.
