Abstract

Pendidikan profesionalisme kedokteran merupakan  salah satu komponen kurikulum yang sangat penting dalam kurikulum pendidikan dokter. Pengembangan dan perbaikan modul Blok Profesionalisme Kedokteran merupakan jembatan untuk menghasilkan lulusan kedokteran yang mampu bersikap dan berperilaku professional. Pengembangan modul dapat dilakukan dengan berbagai cara salah satunya yaitu dengan menerapkan metode role playing. Penerapan metode role playing dilakukan pada Blok Profesionalisme Kedokteran 2012 dan dibandingkan dengan Blok Profesionalisme kedokteran 2011 dimana pada saat itu metode role playing belum diterapkan. Penelitian ini bertujuan untuk mengetahui apakah terjadi peningkatan nilai ujian akhir mahasiswa, jumlah lulusan dan motivasi mahasiswa untuk belajar mandiri. Didapatkan kelulusan yang lebih tinggi pada Blok Profesionalisme Kedokteran 2012 dibandingkan dengan Blok Profesionalisme Kedokteran 2011 yaitu 83.7%: 32.2%. Terdapat hubungan yang bermakna antara penggunaan metode role playing dengan nilai ujian akhir mahasiswa (p<0.05) dan dengan kelulusan (p<0.05). Sebagian besar mahasiswa merasa lebih termotivasi untuk belajar profesionalisme kedokteran (94.55%). Penggunaan metode role playing dapat meningkatkan nilai ujian akhir, jumlah lulusan dan motivasi mahasiswa untuk belajar mandiri Blok Profesionalisme Kedokteran 2012. Development and improvement of the module of the Block Medical Professionalism are the bridge to yielding a good, capable and professional medical doctor. The development of the module can be conducted in various ways, one of which is by applying the method of role playing. Role playing method applied at Block Medical Professionalism of 2012 was compared to Block Medical Professionalism of 201in which role playing was not applied This research aimed to assess if there was  increase of the result score of the students’ final test, total  number of passing students, and students’ self motivation to study  individually. The study results showed a higher number of passing students   in medical professionalism block of 2012  as compared to medical professionalism block of 2011 (83.7% vs 32.2%). There was a significant correlation between role playing and the mean score of the students’ final test (p<0.05) and with the number of passing students (p<0.05). Most student felt more motivated to learn medical professionalism (94.55%). Application of role playing method can improve the final test score, total number of passing students, and students’ self motivation to study medical professionalism of 2012. Abstract

Magnesium is one of microminerals playing pivotal role in glucose homeostasis and insulin activities. Magnesium playsa very important role in phosphorylation of insulin receptor. This is an analytical cross-sectional study to determinethe prevalence of hypomagnesium and correlation between magnesium level with insulin resistance, performed inPedawa Village Bali at February 2010. Subjects for this study were comprised of 111 people, 45 males (40.5%) and 66females (59.5%). Prevalence of hypomagnesium is 17.12%. The prevalence of hypomagnesium is higher in geriatricsubjects compare with non geriatric subjects (38.48% vs 10.98%, p = 0.004), is higher in diabetes subjects compare withnon diabetes (60% vs 15.1%, p = 0.035), and higher in subject with metabolic syndrome compare with non metabolicsyndrome (34.78% vs 12.5%, p = 0.025). Because lack on sensitivity of insulin measurement, 87 subjects (78.38%) thathave insulin level < 2  IU/ml can not measured by machine so the level of HOMA-IR of this subjects can not calculated,and just 24 subjects (21.62%) have insulin level > 2  IU/ml which can measured. From this 24 subjects, there is nosigniÞ cant correlation between magnesium level and insulin resistance (HOMA-IR) (r = -0.1, p = 0.642). In this study,we found a high prevalence of hypomagnesium in residents of Pedawa Village. The prevalence of hypomagnesium ishigher in geriatrics, diabetes mellitus, and metabolic syndrome subjects. There is no signiÞ cant correlation betweenmagnesium and insulin resistance (HOMA-IR), may be because lack of subject in this study. Thus further studies withhigher sensitivity of insulin measurement and enough sample of diabetic or metabolic syndrome patients in hospitalsettingare needed to determine correlation between magnesium level and insulin resistance.

Abstract

Hyponatremia is defined as a decrease in the serum sodium concentration to a level below 135 mmol perliter. Hyponatremia can be associated with low, normal, or high tonicity. One cause of hyponatremia is adrenalinsufficiency. Serum sodium concentration is regulated by stimulation of thirst, secretion of ADH, feedbackmechanisms of the renin-angiotensin-aldosterone system, and variations in renal handling of filtered sodium.Increases in serum osmolarity above the normal range (280-300 mOsm/kg) stimulate hypothalamicosmoreceptors, which, in turn, cause an increase in thirst and in circulating levels of ADH. ADH increases freewater reabsorption from the urine, yielding urine of low volume and relatively high osmolarity and, as a result,returning serum osmolarity to normal. Aldosterone, synthesized by the adrenal cortex, is regulated primarily byserum potassium but also is released in response to hypovolemia through the renin-angiotensin-aldosterone axis.Aldosterone causes absorption of sodium at the distal renal tubule.In this report, patient is male, 64 years old, with probable adrenal insufficiency. Patient have very lowrespond to sodium teraphy. The sodium level increased and have good respond after corticosteroid teraphy.Patient have low level of cortisol serum (18,60 ?/dl) in critically ill condition.The possibility of adrenal insufficiency is of crucial importance in critically ill patients. If the diagnosisis missed, the patient will probably die. In such patients, a blood sample for the measurement of plasma cortisoland corticotropin should be obtained, a short corticotropin test (see below) should be performed, and immediatehigh-dose cortisol therapy should be considered or instituted. A plasma cortisol value in the normal range doesnot rule out adrenal insufficiency in an acutely ill patient. On the basis of a recent study of plasma cortisolconcentrations in patients with sepsis or trauma, a plasma cortisol value of more than 25 ?g per deciliter in apatient requiring intensive care probably rules out adrenal insufficiency, but a safe cutoff value is unknown.

Abstract

Diabetes Mellitus (DM) merupakan salah satu penyakit tidak menular yang prevalensinya terus meningkat, sehingga memerlukan upaya pengelolaan yang efektif untuk mencegah komplikasi dan meningkatkan kualitas hidup penderita. Puskesmas Ubud 1 melalui program Pemberdayaan Kelompok Prolanis Gempita berupaya mengembangkan kemandirian pengelolaan DM dengan melibatkan pasien dalam pendidikan kesehatan, pelatihan keterampilan, dan pembentukan dukungan sosial. Program ini bertujuan untuk meningkatkan pengetahuan, keterampilan, serta perubahan perilaku peserta dalam mengelola diabetes secara mandiri, termasuk dalam pemantauan gula darah, pengaturan pola makan, olahraga, dan manajemen obat. Kegiatan ini dilaksanakan pada bulan Oktober 2024 di Puskesmas Ubud 1 dan diikuti oleh 23 peserta Prolanis. Dari kegiatan ini, seluruh peserta (100%) mengalami peningkatan pengetahuan setelah penyuluhan tentang manajemen DM yang diketahui dari hasil pre-test dan post-test. Kelompok prolanis yang saat ini berjalan menjadi sarana bagi peserta untuk saling berbagi pengalaman dan motivasi. Melalui pendekatan yang terintegrasi, diharapkan peserta dapat meningkatkan kemandirian dalam mengelola DM dan meminimalisir risiko komplikasi. Monitoring dan evaluasi yang dilakukan secara berkala memastikan efektivitas program ini dalam meningkatkan kemandirian peserta dalam pengelolaan DM. Program ini diharapkan dapat meningkatkan kualitas hidup penderita diabetes dan menjadi model pemberdayaan kelompok kesehatan lainnya di masyarakat
