Abstract

Magnesium is one of microminerals playing pivotal role in glucose homeostasis and insulin activities. Magnesium playsa very important role in phosphorylation of insulin receptor. This is an analytical cross-sectional study to determinethe prevalence of hypomagnesium and correlation between magnesium level with insulin resistance, performed inPedawa Village Bali at February 2010. Subjects for this study were comprised of 111 people, 45 males (40.5%) and 66females (59.5%). Prevalence of hypomagnesium is 17.12%. The prevalence of hypomagnesium is higher in geriatricsubjects compare with non geriatric subjects (38.48% vs 10.98%, p = 0.004), is higher in diabetes subjects compare withnon diabetes (60% vs 15.1%, p = 0.035), and higher in subject with metabolic syndrome compare with non metabolicsyndrome (34.78% vs 12.5%, p = 0.025). Because lack on sensitivity of insulin measurement, 87 subjects (78.38%) thathave insulin level < 2  IU/ml can not measured by machine so the level of HOMA-IR of this subjects can not calculated,and just 24 subjects (21.62%) have insulin level > 2  IU/ml which can measured. From this 24 subjects, there is nosigniÞ cant correlation between magnesium level and insulin resistance (HOMA-IR) (r = -0.1, p = 0.642). In this study,we found a high prevalence of hypomagnesium in residents of Pedawa Village. The prevalence of hypomagnesium ishigher in geriatrics, diabetes mellitus, and metabolic syndrome subjects. There is no signiÞ cant correlation betweenmagnesium and insulin resistance (HOMA-IR), may be because lack of subject in this study. Thus further studies withhigher sensitivity of insulin measurement and enough sample of diabetic or metabolic syndrome patients in hospitalsettingare needed to determine correlation between magnesium level and insulin resistance.

Abstract

Hyponatremia is defined as a decrease in the serum sodium concentration to a level below 135 mmol perliter. Hyponatremia can be associated with low, normal, or high tonicity. One cause of hyponatremia is adrenalinsufficiency. Serum sodium concentration is regulated by stimulation of thirst, secretion of ADH, feedbackmechanisms of the renin-angiotensin-aldosterone system, and variations in renal handling of filtered sodium.Increases in serum osmolarity above the normal range (280-300 mOsm/kg) stimulate hypothalamicosmoreceptors, which, in turn, cause an increase in thirst and in circulating levels of ADH. ADH increases freewater reabsorption from the urine, yielding urine of low volume and relatively high osmolarity and, as a result,returning serum osmolarity to normal. Aldosterone, synthesized by the adrenal cortex, is regulated primarily byserum potassium but also is released in response to hypovolemia through the renin-angiotensin-aldosterone axis.Aldosterone causes absorption of sodium at the distal renal tubule.In this report, patient is male, 64 years old, with probable adrenal insufficiency. Patient have very lowrespond to sodium teraphy. The sodium level increased and have good respond after corticosteroid teraphy.Patient have low level of cortisol serum (18,60 ?/dl) in critically ill condition.The possibility of adrenal insufficiency is of crucial importance in critically ill patients. If the diagnosisis missed, the patient will probably die. In such patients, a blood sample for the measurement of plasma cortisoland corticotropin should be obtained, a short corticotropin test (see below) should be performed, and immediatehigh-dose cortisol therapy should be considered or instituted. A plasma cortisol value in the normal range doesnot rule out adrenal insufficiency in an acutely ill patient. On the basis of a recent study of plasma cortisolconcentrations in patients with sepsis or trauma, a plasma cortisol value of more than 25 ?g per deciliter in apatient requiring intensive care probably rules out adrenal insufficiency, but a safe cutoff value is unknown.

Abstract

Diabetes Mellitus (DM) merupakan salah satu penyakit tidak menular yang prevalensinya terus meningkat, sehingga memerlukan upaya pengelolaan yang efektif untuk mencegah komplikasi dan meningkatkan kualitas hidup penderita. Puskesmas Ubud 1 melalui program Pemberdayaan Kelompok Prolanis Gempita berupaya mengembangkan kemandirian pengelolaan DM dengan melibatkan pasien dalam pendidikan kesehatan, pelatihan keterampilan, dan pembentukan dukungan sosial. Program ini bertujuan untuk meningkatkan pengetahuan, keterampilan, serta perubahan perilaku peserta dalam mengelola diabetes secara mandiri, termasuk dalam pemantauan gula darah, pengaturan pola makan, olahraga, dan manajemen obat. Kegiatan ini dilaksanakan pada bulan Oktober 2024 di Puskesmas Ubud 1 dan diikuti oleh 23 peserta Prolanis. Dari kegiatan ini, seluruh peserta (100%) mengalami peningkatan pengetahuan setelah penyuluhan tentang manajemen DM yang diketahui dari hasil pre-test dan post-test. Kelompok prolanis yang saat ini berjalan menjadi sarana bagi peserta untuk saling berbagi pengalaman dan motivasi. Melalui pendekatan yang terintegrasi, diharapkan peserta dapat meningkatkan kemandirian dalam mengelola DM dan meminimalisir risiko komplikasi. Monitoring dan evaluasi yang dilakukan secara berkala memastikan efektivitas program ini dalam meningkatkan kemandirian peserta dalam pengelolaan DM. Program ini diharapkan dapat meningkatkan kualitas hidup penderita diabetes dan menjadi model pemberdayaan kelompok kesehatan lainnya di masyarakat

Abstract

Diabetes can cause various chronic complications, one of the most feared being diabetic nephropathy. Diabetic nephropathy is characterized by proteinuria and/or decreased glomerular filtration rate. Diabetic nephropathy is a major cause of chronic kidney disease, which can lead to end-stage renal disease requiring hemodialysis. Candesartan is an angiotensin receptor blocker antihypertensive drug. In addition to lowering blood pressure, candesartan is also thought to decrease renal vascular resistance, resulting in a decrease in microalbuminuria or proteinuria in patients with diabetes mellitus. Diabetic nephropathy increases with age, and geriatric patients have a lower response to therapy than adults. To determine the effect of candesartan on creatinine and albuminuria in geriatric patients with diabetic nephropathy. This was a cross-sectional study of 25 geriatric patients diagnosed with diabetic nephropathy who had been given 16 mg of candesartan as an antihypertensive at the Puri Raharja Hospital Denpasar outpatient clinic. Subjects who met the inclusion and exclusion criteria were recruited using consecutive sampling. The diabetic nephropathy subjects were evaluated for creatinine and proteinuria levels at the time of diagnosis and 3 months after receiving 16 mg of candesartan. All data were obtained from medical records. The study involved 25 patients consisting of 13 men (52%) and 12 women (485%). Subjects aged between 60-82 years with initial creatinine levels of 0.80-2.60 mg/dL, final creatinine levels of 1.395 ± 0.50 SD, initial positive proteinuria levels of 0-2, and final positive creatinine levels of 0-1. Based on the T-test, there was no significant difference between initial and final creatinine levels with a p value = 0.892 (95% CI -0.045-0.051). Based on the Wilcoxon test, there was a significant difference between proteinuria levels before and after candesartan administration with a p value p <0.05. This study did not find a significant decrease in creatinine after candesartan administration. However, candesartan significantly reduced proteinuria in geriatric patients with diabetic nephropathy.

Abstract

Diabetic nephropathy (DN) is a major complication of diabetes, leading to proteinuria and declining kidney function. Candesartan, an angiotensin receptor blocker, may offer renal benefits beyond blood pressure control. Its specific effects on creatinine and proteinuria in geriatric DN patients remain underexplored. A cross-sectional study was conducted with 25 geriatric DN outpatients at Puri Raharja Hospital. Using consecutive sampling, subjects receiving 16 mg candesartan daily had their serum creatinine and proteinuria levels evaluated from medical records at baseline and after three months. The cohort (60-82 years) comprised 13 men and 12 women. Initial creatinine ranged from 0.80-2.60 mg/dL, with a final mean of 1.395 ± 0.50 mg/dL. Statistical analysis (paired T-test) showed no significant change in creatinine levels (p=0.892). However, proteinuria significantly decreased post-treatment (Wilcoxon test, p=0.005). Candesartan administration did not significantly reduce serum creatinine in geriatric DN patients over three months. However, the significant reduction in proteinuria indicates a beneficial effect on renal pathology, potentially slowing DN progression in this population.

Abstract

Chronic kidney disease (CKD) is still a world health problem, that showed by an increasing prevalence every year. CKD causes kidney damage which is assessed by increased levels of creatinine and urea. One of the health problems that can occur in CKD patients is anemia. The purpose of this study is to determine the relationship between creatinine and urea levels with the degree of anemia, by using analytical research methods with a cross-sectional study design. The data collection method used secondary data from medical records. The research samples are 80 CKD patients at the Sanjiwani Hospital, Gianyar who qualified the inclusion and exclusion criteria with consecutive sampling techniques. Data were analyzed by univariate and bivariate analysis. The results showed that patients with CKD were dominated by men (60%), with an age range of 50-59 years (55%), and dominated by CKD stage V (78.8%). The highest average creatinine and urea levels were found in CKD stage V, which was 11.41 mg/dL and 143.72 mg/dL, while the lowest average of hemoglobin level was found in CKD grade V (9.07 g/dL). The results of the analysis of the One way annova test showed that the value of p=0.003 for creatinine and p=0.001 for urea (p<0.05). Based on these results, it can be concluded that there is a significant relationship between creatinine levels with moderate anemic patients (p=0.003) and severe anemic patients (p=0.003) using the Bonferroni post hoc test, and also there is a significant relationship between urea levels with moderate anemic patients (p=0.022) and severe anemic patients (p=0.022) using the Games-Howell post hoc test.

Abstract

Faculty of Medicine has a variety of learning methods that can cause stress which can trigger a functional disorders in the digestive tract, namely irritable bowel syndrome (IBS). This research aims to determine the relationship between stress and the incidence of IBS in first year students of the Faculty of Medicine and Health Sciences, University of Warmadewa. This research was conducted from February to March 2022 using a cross-sectional study and simple random sampling technique. The subjects of this study were students of the Faculty of Medicine and Health Sciences, University of Warmadewa batch 2021 who met the inclusion and exclusion criteria, totaling 73 people. The instruments in this study were the Rome IV questionnaire for the incidence of IBS and the Perceived Stress Scale (PSS) questionnaire for stress levels. Data analysis in this research is Mann-Whitney test with a p value that considered significant, namely <0.05. Based on this study, the prevalence of mild stress was 9 people (12.3%), moderate stress was 61 people (83.6%), and severe stress was 3 people (4.1%). The prevalence of irritable bowel syndrome was 7 people (9.6%). Through the Mann-Whitney analysis, a significant p value of 0.015 (p<0.05) was obtained. The results showed that there was a relationship between stress and the incidence of IBS in first year students of the Faculty of Medicine and Health Sciences, Warmadewa University. Medical students are advised to care and be aware of the symptoms and body conditions that can occur due to stress.

Abstract

Abstract The majority of cardiovascular diseases in community, especially the elderly are hypertension. One of the effective non-pharmacological of hypertension is physical activity. The purpose of this study to determine the relationship of daily physical activity with the degree of hypertension in the pre-elderly and elderly in ​​Puskesmas I East Denpasar. This research used observational analytic research method with cross sectional and consecutive sampling technique. The instrument used in this study were the GPAQ questionnaire, sphygmomanometer and stethoscope. Data analysis was carried out in this research Spearman rank test. The research sample amounted to 64 samples. The results showed that there were 10 samples (15.6%) with low physical activity experienced hypertension, 1 of them (1.6%) had first degree hypertension, 3 samples (4.7%) had second degree hypertension and third degree hypertension was 6 samples (9.4%). A total of 35 samples (54.7%) who were physically active were experiencing hypertension including first degree hypertension as many as 21 samples (32.8%), second degree hypertension as many as 11 samples (17.2%), and third degree hypertension as many as 3 (4,7%). Of the total 19 samples (29.7%) who had high physical activity, 16 samples (25%) had first degree hypertension, 2 samples (3.1%) had second degree hypertension and 1 sample had third degree hypertension (1.6%). Based on the results of the Spearman rank, it is known that there is a significant relationship between daily physical activity and the degree of hypertension in the pre-elderly and elderly at Puskesmas I East Denpasar (p-value 0.0001) with a non-unidirectional relationship (r=-0.489). The increasing physical activity, the lower the degree of hypertension on the sample.

Abstract

Knowledge of Basic Life Support (BLS) is considered basic knowledge for the medical team because it is needed to provide first aid to victims who experience cardiac arrest. The Baswara Prada Medical Assistance Team (TBM) is an organization engaged in medical emergencies under the auspices of the BEM-LM FKIK Warmadewa University. This study aims to analyze and describe the level of basic life support knowledge of the Baswara Prada Medical Assistance Team (TBM) at the Faculty of Medicine, Warmadewa University. This research is a descriptive study using a cross sectional approach. This research was conducted on 101 respondents of members of the Baswara Prada Medical Assistance Team, Warmadewa University. Data was collected by giving a questionnaire in the form of a google form containing questions about Basic Life Support (BLS) to members of the Baswara Prada Medical Assistance Team. The results of the research data were analyzed descriptively and presented in the form of tables or graphs. The results showed that the knowledge level of Basic Life Support (BLS) of the Baswara Prada Medical Assistance Team (TBM) of the Faculty of Medicine and Health Sciences, Warmadewa University was at a good level with a percentage of 95.1% and at an adequate level with a percentage of 4.9%.

Abstract

Rendahnya kadar trombosit dan hemokonsentrasi merupakan indikator penting dalam diagnosis infeksi virus dengue. Beberapa temuan sebelumnya menunjukkan perbedaan hasil mengenai hubungan antara kadar trombosit dan peningkatan hematokrit pada pasien dengue. Penelitian ini bertujuan untuk menganalisis hubungan antara kadar trombosit dan peningkatan hematokrit pada pasien infeksi virus dengue di RSUD Tabanan periode 1 Januari 2021 – 31 Desember 2022.
