Abstract

Introductions : Uncontrolled blood pressure and persistant overload extracelluler fluid caused by the positif balance of sodium and water, is the contributor of left venticular hipertrophy in hemodialytic patients and will influeate to cell integrity. Sodium dialysate level modification is one of the method to solve this matter. Phase angle is the method to examine the cell integrity in hemodialysis patients. The aim of this study was to determinethe correlation betweenmodifications of sodiumdialysatewith phase angle in regularhemodialysispatientsMethods : An experimental studydesignfromMay untilDecember 2014in28regularhemodialysis patients. By comparing phase angle at pre hemodialysis before and after sodium dialysate modificationResults : Of the 28 subjects, we found 16 male patients (57.1%) and female 12 patients (42.9%) with mean phase angle after sodium dialysate modification higher compare than before modification (5.83 vs 5.80). But there is no significant difference in statistic (p=0.145)Conclusion : There is no significant difference in phase angle between before and after sodium dialysate modification. Abstract

Introduction : Reticulocyte Hemoglobin Equivalent (RET He) is a new parameter for evaluating iron status. RET He is not influenced by chronic inflammatory conditions thus could provide better information of iron status in patients with chronic kidney disease (CKD) with regular hemodialysis. The aim of this study was to investigate the changes in RET He after iron supplementation to determine the usefulness of this marker as iron status parameter in CKD patients with regular hemodialysis.Methods : Uncontrolled clinical study was conducted at Hemodialysis Center in Adam Malik Hospital Medan during April-May 2011. A total of 15 regular hemodialysis patients with iron deficiency anemia received intravenous iron supplementation (100mg Fe sucrose) every hemodialysis session (2x/weeks) for 4 weeks. The changes in hemoglobin, hematocrit and RET He before and after the intervention were analyzed using paired t test.Results : Mean hemoglobin, hematocrit and RET He before intervention were 8.6 (SD 1.09) g/dl, 28.0 (SD 4.25)% and 29.9 (SD 3.85) pg, respectively. After intervention they were 9.9 (SD 1.77) g/dl, 31.7 (SD 5.31)% and 32.6 (SD 3.24) pg, respectively (P=0.023, P=0.049 and P=0.019).Conclusions : RET He were significantly higher after administration of intravenous iron supplement in iron deficient anemia in CKD patients with regular hemodialysis. These changes were also found in hemoglobin and hematokrit.Key words : anemia; chronic kidney disease; iron deficiency; reticulocyte hemoglobin equivalent

Abstract

Background : Malnutrition in hemodialysis patients is common and it’s a regular independent factors affecting morbidity and mortality. This relates to the level of uremic toxins clearance during the process of hemodialysis is less than optimal. Clinical Application of hemoperfusion combination with hemodialysis (HD/HP) demonstrated the optimal clearance in molecular uremic toxins clearance. Kidney Disease Outcomes Quality Initiative (KDOQI) recommends assessment nutritional status of regular hemodialysis patients with a combination of valid assessment tools. such as 7-point SGA (Subjective Global Assessment) and albumin. This aim of the study to determine correlation between the combination of HD/HP with the nutritional status of regular hemodialysis patients.Method : This prospective cohort study from December 2013 to March 2014 on 20 regular hemodialysis patient and do history. measurements of height. weight. body mass index. nutritional status assessment by 7-point SGA and examination of serum albumin.Results : At the end of observation. 20 subjects were observed one subject out of the study. There is a significant difference between the mean of 7-point SGA score before and after combination hemodialysis/hemoperfusion (p<0.05). There is a significant difference between the mean albumin before and after first combination Albumin HD/HP (p<0.05). There is a strong relationship between the 7-point SGA with BMI after combination (r=0.636.p<0.01).Conclusion : The combination of HD / HP is good clinical application for the treatment of malnutrition in regular hemodialysis patients. Regular assessment of nutritional status of regular hemodialysis patients is necessary. with a combination of a valid assessment tool (7-point SGA. albumin) is expected to help decrease morbidity and mortality.Key word : Hemodialysis; haemoperfusion; 7-point SGA; albumin

Abstract

Background : Urinary tract infection (UTI) is the most frequent infectious diseases occur and took second place after respiratory tract infections, where his first-line therapy is an antimicrobial. Many sufferers are not improved after treatment due to the onset of bacterial resistance to antimicrobials specific types and also the discovery of bacteria producing Extended-Spectrum BetaLactamase (ESBL). The aim of study is to determine the differences in patterns of antimicrobial resistance urinary tract infections caused by ESBL-producing bacteria and Non-ESBL.Methods : This study was an observational study with cross-sectional method included 50 outpatients in Departments of Internal Medicine Haji Adam Malik Hospital. Urinalisa examination found nitrites(+) or hematuria(+) or leukocytes> 10/lpb and continued urine cultures, Double Disk Synergy Test (DDST) and sensitivity test.Results : Antimicrobial resistance in ESBL groups were ampicillin (94.7%), and trimethoprim/sulfamethoxazole, ceftriaxon (84.2%), followed by cefotaxime, cefpodoxime and tetracyclin (78.9%) while in Non-ESBL group were ampicillin (88%), and trimethoprim/sulfamethoxazole (84%), and tetracyclin (76%). Differences patterns of antimicrobial resistance in E.coli ESBL producing group and Non-ESBL found cefotaxime significant (P = 0.021) and aztreonam significant (P = 0.031). Conclusion : ESBL-producing bacteria have an increased resistance to several antimicrobial mainly cephalosporin class although not statistically significantly different.Keywords : antimicrobial resistance; urinary tract infection; extended spectrum beta lactamase
