Abstract

Introduction : Reticulocyte Hemoglobin Equivalent (RET He) is a new parameter for evaluating iron status. RET He is not influenced by chronic inflammatory conditions thus could provide better information of iron status in patients with chronic kidney disease (CKD) with regular hemodialysis. The aim of this study was to investigate the changes in RET He after iron supplementation to determine the usefulness of this marker as iron status parameter in CKD patients with regular hemodialysis.Methods : Uncontrolled clinical study was conducted at Hemodialysis Center in Adam Malik Hospital Medan during April-May 2011. A total of 15 regular hemodialysis patients with iron deficiency anemia received intravenous iron supplementation (100mg Fe sucrose) every hemodialysis session (2x/weeks) for 4 weeks. The changes in hemoglobin, hematocrit and RET He before and after the intervention were analyzed using paired t test.Results : Mean hemoglobin, hematocrit and RET He before intervention were 8.6 (SD 1.09) g/dl, 28.0 (SD 4.25)% and 29.9 (SD 3.85) pg, respectively. After intervention they were 9.9 (SD 1.77) g/dl, 31.7 (SD 5.31)% and 32.6 (SD 3.24) pg, respectively (P=0.023, P=0.049 and P=0.019).Conclusions : RET He were significantly higher after administration of intravenous iron supplement in iron deficient anemia in CKD patients with regular hemodialysis. These changes were also found in hemoglobin and hematokrit.Key words : anemia; chronic kidney disease; iron deficiency; reticulocyte hemoglobin equivalent

Abstract

Visceral and subcutaneous adipose tissue (VAT and SAT), and excess body mass index (BMI) act as cardiometabolic risk factors. BMI only describes the ratio of body weight to height in general. Abdominal CT scan is the gold standard in the examination of VAT and SAT volumes. The purpose of this study is to analyze the correlation between BMI with VAT and SAT in patients that performed CT scans of the abdomen. A total of 60 patients who underwent CT scans of the abdominal region at the Radiology Department of the RSUP H. Adam Malik Medan from July to August 2022 participated in this cross-sectional study. The BMI was calculated from measurement and volume of VAT and SAT were obtained from CT scans of the abdominal region at the fourth lumbal level, using 3D smart segmentation. Among of samples was 51.7% male. Average of age and BMI are 49.3 12.18 years and 25.58 kg/m2. Average of VAT and SAT volume value are 678.13 436.09 cm3 and 1708.76 1068.99 cm3. There is a moderate positive correlation between BMI and VAT (r=0.687) and BMI and SAT (r=0.669) with p0.001.

Abstract

Inferior myocardial infarction is frequently accompanied by right ventricular infarction. This report presents two cases of right ventricular infarction accompanied by hemodynamic disorder and atrioventricular block complications. These patients were diagnosed with right ventricular infarction based on history, clinical presentations, and electrocardiography findings. The management approach for these patients involved optimizing volume and fluid status, using temporary pacemakers, and urgent percutaneous coronary intervention. This report highlights the need to promptly identify right ventricular involvement in inferior myocardial infarction, as the treatment strategies for right ventricular infarction are different from left ventricular infarction.

Abstract

Background. Diabetes is a major health problem worldwide including in Indonesia. There is a close relationship between diabetes and cardiovascular disease, with cardiovascular events being the most prevalent cause of morbidity and mortality in diabetic patients. Therefore, it is important to assess cardiovascular risk in patients with diabetes in order to conduct appropriate preventive strategies. Various methods have been developed to assess the degree of cardiovascular risk, one of which is the Jakarta Cardiovascular Score, a scoring method that has been adapted for the Indonesian community. This study aims to describe the profile of cardiovascular risk in diabetes patients at Universitas Sumatera Utara Hospital. Methods. A descriptive, cross-sectional study was conducted in the internal medicine outpatient clinic at Universitas Sumatera Utara Hospital. All subjects were diabetic patients who had not developed any cardiovascular complications. The risk of cardiovascular disorders was assessed using the Jakarta Cardiovascular Score. Data were collected through medical records, interviews, and physical examinations. Results. There were 64 subjects in this study. The majority of subjects (81.3%) were classified as high risk for cardiovascular disease. Conclusion. Most patients with diabetes have a high degree of risk of cardiovascular disease.

Abstract

Introduction. The Get with the Guidelines-Heart Failure (GWTG-HF) risk score is a widely utilized and validated tool for assessing prognosis in patients with heart failure. Inflammation significantly contributes to the pathophysiology of heart failure, evidenced by platelet activation and a reduction in lymphocyte counts. Consequently, the platelet-to-lymphocyte ratio (PLR) may function as a prognostic biomarker in this population. This study aims to investigate the correlation between the GWTG-HF risk score and PLR as an inflammatory marker in patients hospitalized with heart failure. Methods. This cross-sectional study utilized medical records of heart failure patients admitted to Adam Malik Hospital in Medan from January 2022 to July 2023. Total GWTG-HF scores and PLR values were recorded at the time of patient admission. Correlation analyses were conducted to assess the relationship between the two variables. Results. The study evaluated 106 patient records, with male patients comprising 72.64% and a mean age of 55.84 (SD 14.09) years. The median GWTG-HF score was 37.5, while the median PLR was 137.14. Correlation analysis yielded a coefficient of r = 0.249 (p < 0.05), indicating a statistically significant association.Conclusion. A weak positive correlation exists between the GWTG-HF score and PLR in patients with heart failure, suggesting that PLR may serve as a supplementary inflammatory marker in prognostic assessments.
