Abstract

PrEP is an effective additional prevention strategy at trial settings. Decision to adopt PrEP as a public health intervention must be made based on the need and capacity of current systems. Instead of allocating substantial resources for PrEP, the limited resources could be wisely allocated to scale-up the existing prevention strategies including access to ART, as well as to ensure equality of access of people to full-range of HIV prevention and treatment measures.

Abstract

Background and purpose: Increasing of HIV+ cases resulted in the increasing demand for antiretroviral (ARV) and access to ART services. This study aims to explore barriers of integrating ART services into primary health centres in Badung District. Methods: Explorative interviews were conducted with CST officers at Badung District Hospital, puskesmas (health centres), NGOs, policy makers, community leaders and PLWHA. Data were collected by conducting indepth interviews, focus group discussion and observation. Data were thematically analysed. Results: The study found that there were variations in internal barriers across different puskesmas in Badung District to be a satellite site. The main internal barriers perceived by health officers particularly in relation to issues around the availability of human resources, lack of competencies and increase of workload. Internal capacity of puskesmas which also perceived as barriers including the availability of laboratory facilities, absent of operational procedures, limited information system and source of funds that predominantly still from donor agencies. External barriers perceived by respondent were stigma and discrimination. Conclusions: Barriers to integrate ART services into puskesmas consist of structural barriers from internal puskesmas as well as external barriers such as stigma and discrimination towards PLWHA.

Abstract

Given the fact that drug use is a complex problem, public intervention to change and address its determinants are crucial. Social and economic capitals should be channelled via several options, ranging from ‘pro to the poor’ policy, micro economic enterprise, diversion from informal economy, community engagement, social health insurance for poor people, economic development strategy for community and national level and employment initiatives or providing wider access for job opportunity. National public health policy strategies regarding drug user treatment are also necessary to facilitate future interventions.

Abstract

The findings from SR on ‘male circumcision for prevention of heterosexual acquisition of HIV in men’ are warranted to be introduced. Male circumcision can reduce HIV transmission among sexually active men by 38 – 66%. This finding is highly relevant to Indonesian context and partially transferable because of: (1) the high prevalence of HIV/AIDS in Indonesia, ranging from concentrated and generalised HIV epidemics; (2) heterosexual contact is a major route of HIV transmission; and (3) the demographic feature of high risk population. These facts alone, however, do not sufficiently address applicability issues to justify implementation. There are three major applicability issues that should be taken into consideration: (1) HIV/AIDS epidemic and the centrality of sex workers in the transmission of HIV in Indonesia; (2) cultural and social reluctance; and (3) health system readiness. Complete community readiness assessment and a complete applicability-transferability attributes assessment are essential prior to adoption. This requirement mandates a new field of research agenda in Indonesia. In the current situation, the SR finding on male circumcision potentially impacts national policy and regulation - particularly in disseminating information strategy. The findings provide immediate justification to start promoting the health benefits of male circumcision to the wider community.

Abstract

Background and purpose: The approach of pharmacy service provision has shifted from rudimentary medicine distribution to becoming more patient centered. The objective of this study was to explore the factors that affect the quality of pharmaceutical care in pharmacist owned and managed (APA) pharmacies in Denpasar. Methods: Study was cross-sectional with 68 samples collected by systematic random sampling among 214 pharmacies at Denpasar City. Data was collected by interview with pharmacists, selfadministered- questionnaires to their staff and observation regarding the quality of services. Bivariate data analysis was conducted using chi square test and multivariate analysis was carried out using logistic regression. Results: Findings indicated that 48.5% (33 pharmacies) provide poor quality services. The presence of APA, the status of APA and the ownership of pharmacy affected the quality of pharmacy service. Multivariate analysis indicated that ownership of pharmacy was the most dominant variable impacting upon quality of pharmaceutical care (AOR=7.04; 95%CI: 1.5-33.8). Conclusion: The ownership of pharmacy affected the quality of pharmaceutical service provision in Denpasar.

Abstract

Background and purpose: Karangasem Hospital has adopted balanced scorecard method in preparation of the strategic plan. The performance evaluation has been done based on accountability report only with the three other perspectives have never been analysed before. This study was conducted to assess those four perspectives. Methods: Cross-sectional survey was carried out among 110 inpatients and 95 employees which were selected conveniently. The data were collected by interviews using questionnaire. The data were analyzed with kartesius graph to get each item of patients and employees satisfaction that need to be priority. Secondary data analysis conducted to know the tendency of those four perspectives. Results: Based on the assessment of the four perspectives showed that the overall score is 44.7%. It is defined as sufficient category. This also can be observed that each perspective of finance, customers and internal business process included in sufficient category. Their scores are 16.65%, 12.5% and 15.62%. Meanwhile, for the learning and growth perspective showed as weak category (0). Kartesius graph analysis showed that two items of patient’s satisfaction have to do with procedure and quick service need to be concerned. However, the employees’ satisfaction showed that there are five sub items which need to be concerned. They are quick response, management support and working environment. Conclusion: The performance of Karangasem Hospital based on scorecard is in the sufficient category and patients/employee’s satisfaction still need to be concerned on certain items.

Abstract

The construction sector is a contributor to increase number of accidents and occupational illness. Unsafe behavior in the workplace is directly related to work accidents. One of the factors that can influence unsafe behavior is the worker's perception of the safety climate. This study aims to analyze the correlation between perceptions of safety climate and unsafe behavior of construction workers on the renovation project of PT X in Nusa Dua. Quantitative research was conducted using an analytical cross-sectional approach. Respondents in the study are 200 people who were selected by the block sampling method. This study found that the proportion of respondents behaved unsafely was 60.5%. Unsafe behavior was found to be significantly related to safety empowerment management (p=0.042), worker commitment (p=0.012), priority and risk tolerance (p=0.007), and communication and trust in implementation (p=0.040). In multivariate analysis, respondents who have good perceptions of communication and trust in implementation had a 2.63 times chance of reducing unsafe behavior compared to who had sufficient perceptions. As an effort to increase safety behavior, workers are expected to be more concerned with the risk of hazards in the workplace by complying with occupational safety health programs.
