Abstract

Latar belakang. Sakit perut berulang merupakan suatu keluhan yang sering datang pada praktik dokter anak usia sekolah. Seringkali penyebab tidak terdeteksi dan diyakini sebagai keluhan fungsional. Tujuan. Mengetahui prevalensi dan hubungan faktor risiko dengan kejadian sakit perut berulang pada anak usia 7-12 tahun di Kecamatan Padang Timur.Metode. Studi potong lintang dilakukan pada Januari – April 2019 terhadap 153 anak berusia 7-12 tahun di tiga Sekolah Dasar Kecamatan Padang Timur, Kota Padang. Data dikumpulkan dari pengisian kuesioner sesuai Kriteria Roma III yang diisi oleh orang tua siswa yang sebelumnya sudah menandatangani lembaran persetujuan orang tua.Hasil. Nyeri perut berulang dialami 6,5% dari 153 anak. Hasil uji chi-square terdapat hubungan bermakna antara paparan kejadian yang dapat menyebabkan stres dari rumah, sekolah, dan tingkat pendidikan orang tua dengan kejadian sakit perut berulang pada anak usia 7-12 tahun dengan kejadian sakit perut berulang. Hasil analisis regresi logistik didapatkan paparan kejadian dalam hidup yang dapat menyebabkan stres dari rumah merupakan faktor risiko kejadian pada anak usia 7-12 tahun di Kecamatan Padang Timur. Kesimpulan. Paparan kejadian dalam hidup yang dapat menyebabkan stres dari rumah merupakan faktor risiko kejadian sakit perut berulang pada anak usia 7-12 tahun di Kecamatan Padang Timur.

Abstract

Pendahuluan: Glomerulonefritis akut pasca Streptococcus (GNAPS) merupakan bentuk glomerulonefritis terbanyak pada anak dengan angka kejadian tinggi di negara berkembang. Manifestasi klinis GNAPS pada anak bisa beragam, mulai hipertensi ringan sampai krisis, dan berbagai komplikasi seperti gangguan ginjal akut dan hipertensi ensefalopati.Tujuan penelitian: Untuk mengetahui faktor-faktor yang berhubungan dengan kejadian hipertensi pada pasien GNAPS di Bagian Ilmu Kesehatan Anak RSUP DR M.Djamil Padang tahun 2015-2017  Metode: Penelitian dilakukan pada Desember 2017 – Maret 2018 di Bagian Ilmu Kesehatan Anak RSUP Dr. M Djamil Padang. Penelitian deskriptif analitik retrospektif dengan desain cross sectional dan teknik total sampling. Sampel penelitian berjumlah 27 pasien. Analisis yang digunakan yaitu uji Fisher’s Exact dan regresi logistik. Hasil: Hasil penelitian mendapati rasio laki-laki dan perempuan 1,5:1. Umur, jenis kelamin, status gizi, hematuria, proteinuria, LFG, ASTO positif tidak signifikan berpengaruh terhadap kejadian hipertensi (p=0,64; p=1,00; p=1,00; p=0,05; p=0,30; p=0,58; p=0,61, berurutan). Edema terdapat pada 74,1% pasien, dengan 90% yang mengalami hipertensi (p=0,02). Analisis multivariat terhadap edema dan hematuria menunjukkan edema adalah faktor yang paling berpengaruh terhadap kejadian hipertensi (p=0,033). Kesimpulan: Edema adalah faktor yang signifikan dan paling berpengaruh terhadap kejadian hipertensi pada pasien GNAPS. Diperlukan kewaspadaan akan terjadinya hipertensi pada pasien GNAPS yang datang dengan edema.

Abstract

Background: Adolescent obesity is a global epidemic that initiates subclinical atherosclerosis. While lipid profiles are traditional markers, their correlation with structural vascular changes in the pediatric population remains a subject of intense academic debate. This study evaluates the relationship between traditional lipid parameters and carotid intima-media thickness (CIMT) in obese high school students. Methods: An observational cross-sectional study was conducted between January 2025 and January 2026 involving 45 obese adolescents aged 15 to 18 years in Padang, Indonesia. Nutritional status was determined using the CDC 2000 growth charts, specifically targeting those with a body mass index at or above the 95th percentile. CIMT was measured via high-resolution B-mode ultrasonography. Lipid profiles, including total cholesterol, triglycerides, HDL, and LDL, were analyzed through laboratory testing. Statistical analysis utilized Fisher’s exact test through SPSS version 22. Results: The subjects, of whom 68.9 percent were female, had a median BMI of 29.34 ± 2.03 kg/m². Remarkably, 68.9 percent exhibited CIMT thickening above the 75th percentile. Bivariate analysis revealed no significant association between CIMT and total cholesterol (p=0.402), triglycerides (p=0.696), HDL (p=0.563), or LDL (p=1.000). Conclusion: Vascular remodeling in obese adolescents occurs independently of circulating lipid levels, suggesting that chronic inflammation may drive early atherosclerosis before clinical dislipidemia manifests.

Abstract

Background. Long-term antiseizure-medication (ASM) therapy can accelerate vitamin D catabolism via hepatic cytochrome P450 induction, predisposing children with epilepsy to hypovitaminosis D and its skeletal consequences; Indonesian tertiary-centre data remain scarce. Methods. This cross-sectional study examined the association between ASM class, number and duration and serum 25-hydroxyvitamin D [25(OH)D] in children aged 1–18 years at Dr. M. Djamil General Hospital, Padang, West Sumatera, between April and October 2025. Of 82 records screened, 77 were eligible; 25(OH)D was measured by enzyme-linked fluorescent assay, with hypovitaminosis D defined as <30 ng/mL. Associations were tested with Fisher–Freeman–Halton exact and chi-square tests, odds ratios, ANOVA, multivariable logistic regression and ROC analysis. Results. Hypovitaminosis D affected 48 children (62.3%; 95% CI 51.2–72.3), with mean 25(OH)D of 18.3±6.7 versus 41.6±11.2 ng/mL in deficient versus replete children. ASM class was significantly associated with vitamin D status (exact p=0.037; Cramér's V=0.283): all nine enzyme-inducing users were deficient, versus 56.0% non-enzyme-inducing and 58.1% combination (ANOVA p=0.045, η²=0.080). Neither ASM number (p=0.642) nor duration (p=0.348) was associated. Enzyme-inducing exposure carried the largest adjusted odds (adjusted OR 5.66, 95% CI 0.62–52.06), and the model discriminated moderately (AUC 0.685). Conclusion. Hypovitaminosis D is prevalent in Indonesian children with epilepsy and is most strongly linked to enzyme-inducing ASMs, supporting early routine 25(OH)D monitoring and supplementation from treatment initiation.

Abstract

Stunting remains a significant public health challenge, particularly in developing countries. Vitamin D and calcium play crucial roles in linear growth and bone mineralization during early childhood. This study aimed to analyze the levels of serum vitamin D and calcium in stunted and severely stunted children aged 12–23 months in the working area of Pauh Health Center, Padang City, West Sumatra. This cross-sectional analytical study was conducted from February to May 2025, involving 43 children aged 12–23 months (33 stunted and 10 severely stunted). Data were analyzed using the chi-square test and independent-samples t-test. Most children exhibited hypovitaminosis D (62.7%), with 9.3% deficiency and 53.4% insufficiency. Serum calcium levels were predominantly normal (85.1%). No significant differences were found in vitamin D levels (p > 0.05) or calcium levels (p > 0.05) between stunted and severely stunted children. The mean vitamin D level was 12.14 ± 1.97 ng/mL in stunted children and 12.16 ± 1.63 ng/mL in severely stunted children. The mean serum calcium levels were 9.14 ± 1.97 mg/dL in stunted children and 9.16 ± 1.63 mg/dL in severely stunted children, respectively. Despite the high prevalence of vitamin D insufficiency, there were no significant differences in serum vitamin D and calcium levels between stunted and severely stunted children aged 12–23 months. The body's homeostatic mechanisms maintain normal serum calcium levels despite low vitamin D status, potentially at the expense of bone density. Further research is needed to identify other contributing factors to stunting severity.

Abstract

Latar Belakang: Hipertensi merupakan penyakit tidak menular yang menjadi masalah kesehatan masyarakat di dunia. Faktor yang dapat memengaruhi terjadinya hipertensi salah satunya adalah obesitas. Obesitas pada remaja ditandai dengan indeks massa tubuh (IMT) yang berlebih dan rasio lingkar pinggang-tinggi badan (RLPTB) yang tinggi. Objektif: Tujuan penelitian ini adalah untuk mengetahui hubungan antara indeks massa tubuh dan rasio lingkar pinggang-tinggi badan dengan tekanan darah pada remaja SMA Adabiah Kota Padang. Metode: Penelitian dilakukan dengan desain cross sectional menggunakan teknik pengambilan sampel proporsionate stratified random sampling pada 135 responden dengan melakukan pengukuran indeks massa tubuh, rasio lingkar pinggang-tinggi badan dan tekanan darah. Data dianalisis menggunakan uji Chi-Square. Hasil: Hasil penelitian kepada 135 responden, didapatkan responden terbanyak merupakan remaja kelas X (51.1%), berusia 16 tahun (51.1%), dan berjenis kelamin perempuan (60%). Sebanyak 92 responden (68,1%) memiliki IMT normal, 29 responden (21,5%) IMT lebih, dan 14 responden (10,4%) IMT kurang. Sebanyak 108 responden (80%) memiliki RLPTB normal dan 27 responden (20%) RLPTB tinggi. Hasil uji Chi-Square menunjukan terdapat hubungan bermakna antara indeks massa tubuh dan rasio lingkar pinggang-tinggi badan dengan tekanan darah pada remaja SMA Adabiah Kota Padang dengan p-value masing-masing 0,000 (p<0,05). Kesimpulan: Kesimpulan pada penelitian ini terdapat hubungan yang bermakna antara indeks massa tubuh dan rasio lingkar pinggang-tinggi badan dengan tekanan darah pada remaja SMA Adabiah Kota Padang.
