Abstract

Introduction: Caries is a multifactorial disease that is influenced by saliva and teeth as the host, microorganism, substrate, and time. Children with stunting experiencing saliva gland atrophy which decrease saliva flow, buffer function, immunity component, and its antimicrobial activity which increase the risk of caries. The research was done on children aged 6-12 years old in Puskesmas Andalas, Padang, aims to find out the relationship between the number of bacteria and DMF-T index based on stunting cases. Methods: The research method was observational with the cross-sectional study. Salivary sampling was selected by stratified random sampling for 30 stunting kids and 30 normal ones. Result: The result showed that based on the research, the amount of Streptococcus mutans ATCC 25175 bacteria is higher in children with stunting than the normal ones, but not significant statistically; p=0,659 (p>0,05). DMF-T index is higher in children with stunting than in normal ones. Statistical test analysis resulted in p= 0,004 (p<0,005), which shows the relationship between DMF-T and stunting. Conclusion: This research showed that there is no relationship between the number of Streptococcus mutans ATCC 25175 with DMF-T index based on stunting cases.

Abstract

Latar Belakang :Stunting merupakan kondisi gagal tumbuh pada anak, dimana tinggi badan menurut umur dibawah standar pertumbuhan menurut World Health Organization (WHO). Stunting dipengaruhi banyak faktor, diantaranya penyerapan nutrisi yang kurang, baik makro maupun mikronutirien. Kondisi stunting dapat berpengaruh negatif terhadap pertumbuhan fisik dan kognitif. Di rongga mulut kelenjar saliva pada anak stunting mengalami atropi akibat defisiensi protein, hal ini berpengaruh pada jumlah produksi saliva dan kualitas saliva seperti fungsi self-cleansing, kapasitas buffer, komposisi saliva dan komponen imunitas saliva. Perubahan ini memengaruhi profil mikrobioma oral pada anak stunting. Mikrobioma oral dan usus merupakan yang paling komplek dalam tubuh manusia. Mikrobioma oral dan usus tidak hanya saling terkait namun disbiosis mikrobioma oral akan memodulasi mikrobioma usus. Tujuan: Secara umum literrature review ini bertujuan untuk mengetahui perubahan mikrobioma oral sebelum dan sesudah terapi oral pada anak stunting. Metode: Metode yang digunakan adalah literature review. Hasil: Adanya hubungan mikrobioma oral terhadap absorpsi nutrisi pada anak stunting. Kesimpulan : Diperlukan penanganan yang holistik di rongga mulut, dengan menghilangkan bakteri patogen yang ada di rongga mulut.Kata kunci : Stunting, Mikrobioma oral, mikrobioma usus, suplemen, terapi oral

Abstract

Introduction: Stunting has long-term effects on child growth and development and may be associated with oral health and immune responses. Gingivitis is an inflammatory periodontal condition characterized by the activation of various inflammatory mediators, including interleukin-6 (IL-6). This study was conducted among children aged 6–12 years in the working area of Lubuk Kilangan Community Health Center, Padang City, Indonesia.Objective: To compare the periodontal healing response following scaling and root planing (SRP) between children with stunting and those with normal nutritional status based on clinical parameters and salivary IL-6 levels.Methods:This experimental study employed a pretest–posttest group design involving 60 children aged 6–12 years with gingivitis who were selected using simple random sampling, comprising 30 children with stunting and 30 children with normal nutritional status. Clinical parameters included the Oral Hygiene Index (OHI) and Gingival Index (GI), while the molecular parameter was salivary IL-6 level. All parameters were assessed before SRP and four weeks after SRP.Results: Significant improvements in clinical periodontal parameters were observed after SRP in both groups. Salivary IL-6 levels also decreased significantly in the stunting and normal nutritional status groups, with mean reductions of 2.37 pg/mL (p<0.001) and 3.36 pg/mL (p=0.002), respectively.Conclusion: SRP resulted in improvements in clinical periodontal parameters and reductions in salivary IL-6 levels in both children with stunting and those with normal nutritional status. However, the clinical improvement in the stunting group was less pronounced than in the normal nutritional status group. These findings suggest that periodontal healing responses may be influenced by both local and systemic factors. Periodontal management in children with stunting should consider systemic conditions, including nutritional status and oral health education.

Abstract

Introduction: Composite resin is a restorative material that bonds to tooth structure through micromechanical interlocking. Etching removes the smear layer and creates microporosities, enhancing mechanical retention. Although 37% phosphoric acid is the gold standard, it may cause inflammatory cell infiltration. Averrhoa bilimbi L. contains citric, oxalic, and lactic acids, which may serve as a natural alternative etching agent for enamel. This study aimed to analyze the effect of Averrhoa bilimbi L. extract gel as a natural enamel etching agent on the tensile bond strength of composite resin restorations. Methods: This was an in vitro laboratory true experimental study that employed a post-test-only control group design. Twenty-eight extracted human premolars were divided into four groups (n = 7): three experimental groups etched with 2.5%, 5%, and 7.5% Averrhoa bilimbi L. extract gel, and one control group etched with 37% phosphoric acid. After etching, bonding agent and microhybrid composite resin were applied. Tensile bond strength was tested using a Universal Testing Machine, and data were analyzed using the Shapiro–Wilk, Levene’s, and Kruskal–Wallis tests. Results: The highest mean tensile bond strength was observed in the 37% phosphoric acid group (7.56 ± 3.87 MPa), followed by 7.5% (6.71 ± 1.71 MPa), 5% (6.17 ± 1.39 MPa), and 2.5% (6.04 ± 3.01 MPa) Averrhoa bilimbi L. extract groups. The Kruskal-Wallis test showed p=0.874, indicating no statistically significant difference between the groups. Conclusion: Averrhoa bilimbi L. extract gel demonstrated a tensile bond strength comparable to that of 37% phosphoric acid. The extract gel demonstrated promising potential as a natural etching agent; however, further laboratory and clinical studies are required before it can be recommended for clinical application.
