Abstract

Highlights: Arthroplasty surgeries were reduced significantly during the COVID-19 pandemic. The number of operations gradually increased between the first and second year of the pandemic. Patients' median age and hospital stays were reduced during the pandemic.   Abstract Introduction: The COVID-19 pandemic has had significant effects on arthroplasty surgeries worldwide. Various studies have described decreased arthroplasty procedures among the most common orthopedic surgeries. This study aimed to analyze the effects of the pandemic on the epidemiology and demography of hip and knee arthroplasty in a tertiary referral general hospital in Indonesia. Methods: This was a retrospective study of medical records data conducted on hip and knee arthroplasty cases from the pre-pandemic (April 2019–March 2020), first-year pandemic (April 2020–March 2021), and second-year pandemic (April 2021–March 2022) periods. Each period was compared for epidemiology and demography data, which included the annual number of arthroplasties, female-to-male ratio, age, duration of hospitalization, and in-hospital post-operative rehabilitation participation. All statistical data calculations were performed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) version 26. Results: This study observed a statistically significant decrease of 232.00% for hip and 371.43% for knee arthroplasty performed during the first-year pandemic and an increase of 41.86% for hip and 74.07% for knee arthroplasty in the second-year pandemic. A decrease in median age and fluctuation of the female-to-male ratio were observed. The median duration of hospitalization was 80.00% shorter between the pre-pandemic and first-year pandemics and 11.11% shorter between the first and second-year pandemics. Participation in rehabilitation fluctuated during each period. There were four positive cases of COVID-19 and two cases of mortality during the first- and second-year pandemics. Conclusion: The COVID-19 pandemic caused a significant reduction in arthroplasties performed during the first year and a slight increase during the second year. Several changes to the demographics of the patients were also observed between the pre-pandemic and the pandemic periods.

Abstract

Abstract: Cases of maternal death during childbirth are very high in the world. Maternal health is important to children's health, especially for pregnant women. It is hoped that this activity can carry out outreach activities related to fulfilling nutrition to reduce stunting in toddlers and improve the health and nutritional conditions of pregnant women and toddlers, delivered through promotional media using pocketbooks and practice. This service provides training as a method with a participatory approach to improve soft skills (knowledge) and hard skills (how to process nutritious food). This service carries out training as a method with a participatory approach. Participants are cadres at the Puskesmas and civilians, especially pregnant/nursing women, mothers, and toddlers, with 26 participants. The results of outreach activities regarding nutritional requirements to reduce stunting in toddlers and improve the nutritional status of pregnant and breastfeeding mothers and mothers of toddlers are conveyed through promotional media using pocketbooks and practice. Monitoring and evaluating this activity is via WhatsApp and Zoom meetings where the puskesmas cadres submit reports regarding food menu counselling to pregnant/nursing mothers and mothers with toddlers. The success rate for this activity is 90% due to weather constraints. It is hoped that the knowledge provided can foster awareness and positive attitudes toward breastfeeding pregnant women, mothers, and toddlers by applying the knowledge gained in their daily activities. With this activity, it is hoped that Puskesmas Sarang 2 employees will understand the importance of paying attention to nutritional needs to reduce stunting.

Abstract

Background: Ankle injuries are the most common injuries in sports and often cause chronic pain, swelling, and functional instability. Ankle injuries (sprains) are acute injuries that athletes often experience. This acute injury is caused by a pressure force, for example, when making a sudden turning motion. Sprains can affect the ankle's side and damage the outside (lateral) ligament; ankle injuries were reported at the Sports Clinic RSUD Dr. Soetomo Surabaya . Aims: To understand the profile of ankle injuries suffered by East Java Puslatda athletes and investigate the risk factors for these injuries. Material and methods: The study was conducted using a retrospective approach with an analytic observational design in the form of a cross-sectional study because the researchers did not provide intervention to the research subjects and only made observations and secondary data profiling. Result: The data collection and analysis found that ankle injuries in East Java Puslatda athletes most often occur in the late adolescent age range (17-23 years), which is as high as 35.8%. Conclusions: There is no relationship between extrinsic and intrinsic factors on the risk of ankle injury for Puslatda East Java athletes.

Abstract

Backgrounds: Tibial plateau fracture is the most frequent type of fracture caused by motor-vehicle collision. This injury may or may not be associated with soft tissue injury, such as the knee's ligaments. Posterior tibial plateau fractures are a rare subtype that requires surgical intervention. This case report describes the outcome of posterior plating in treating a posterior tibial plateau fracture. Case report: A 41-year-old female patient came to the emergency room (ER) after being involved in a traffic collision. The patient underwent a thorough medical examination, which revealed the tibial eminence avulsion associated with the fracture of the head of the fibula. Surgical treatment was performed using a posterior approach through an inverted L-shaped incision. Discussion: Tibial plateau fractures are serious injuries that often require surgery. However, the decision to operate depends on several factors, including the morphology of the fracture, the patient's profile, the condition of the soft tissues, and the expected recovery of the articular surface and the axis of the limb. Conclusion: A surgical technique using a posterior approach through an inverted L-shaped incision with a small medial T-plate, lateral bone graft, and roof K-wire fixation demonstrates positive outcomes for treating closed posterior tibial plateau fractures and avulsion fractures of the tibial eminence.
