Abstract

Patellar malalignment is the imbalance relationship between patella and trochlea, in which clinical findings most of the time are obscured; hence diagnosis is often challenging. Magnetic resonance imaging (MRI) is the most sensitive tool to detect subtle patellar malalignment features, so diagnosis can be made early. However, there has been no clear consensus on the normal value of patella morphology until today. This study aims to determine patellofemoral morphology values in Indonesian using MRI. This was a retrospective study of 202 patients aged 18-40 years old with knee problems without patellar instability. Patellar morphology parameters including Insal Savati ratio (IS ratio), patellar tilt angle (PTA), sulcus angle (SA) and tibial tubercle-trochlear groove distance    (TT-TG) were evaluated and recorded for statistical analysis. There was no significant correlation between anthropometric values and patellar morphology values. There were significantly higher PTA, SA and TT-TG values in females compared to males. The mean value of the IS ratio in the Asian population using MRI was 0.99 ± 0.14, PTA was 9.09 ± 6.88, SA was 139.20 ± 6.38, and TT-TG distance was 8.00 ± 5.25. Further studies with larger samples and multi-center results are required.

Abstract

Exosomes from Mesenchymal Stem Cells (MSC) is currently one of the highlighted research due to its more specific action on the target tissue. Exosome is one of the MSC Extracellular Vesicle (EVs) that acts as a mediator in the cellular communication. However, not so many literatures have succeed in elaborating its isolation and characterization process. This study aimed to explain the method used in exosomes isolation from Adipose-Derived MSC (ASC) and elaborate its characteristics. This is an in-vitro study performed in the Stem Cell and Tissue Engineering (SCTE) Indonesia Medical Education and Research Institute (IMERI) laboratory. Ultracentrifugation method was performed to isolate exosome from ASC. The exosome were characterized based on its particle size, morphology, and CD63 and CD81 expression for its purity. We were able to isolate sterile exosome from ASC by performing differential ultracentrifugation. The mean size of exosome was 88.7 nm ± 40 nm SD and showed expression of CD63 and CD81. Exosome was successfully isolated from ASC using ultracentrifugation method and characterization following the MISEV standard should be implemented in order to meet the therapeutic efficacy and safety issues as a regenerative agent.

Abstract

Objective: Bone marrow mesenchymal stem cells are promising in cell therapy but the low frequency of this subpopulation necessitates their in vitro expansion and cryopreservation prior to clinical use. The aim of this study was to analyze the passage effect on the aging profile of previously successive cryopreserved bone marrow mesenchymal stem cells.  Methods: The samples of this in-vitro observational analytic study (April - September 2016) were iliac crest bone marrow mesenchymal stem cells that were cryopreserved in passage one and a second group was cryopreserved after being subcultured. It was obtained from UPT TK Sel Punca RSCM-FKUI. Samples were analyzed for each passage in terms of cell size, viability, population doubling time (PDT), and percentage of senescent cells by ANOVA test and independent t-test for the inter-cryopreservation group.Results: The remarkable manifestation of senescence appeared at passage six in both cryopreserved groups, but tend to appear earlier in the twice cryopreservation group. The senescent positive cell size became bigger along with the increasing number of passage. There were significant differences in PDT, 30% confluent cell size, viability, in passage six between the two cryopreservation groups (p<0.001, p<0.001, p=0.022), respectively, but no significant difference in percentage of senescent cells (p= 0.052). The cells became senescent in P6 in both groups (P=0.000).Conclusion: The onset of senescence of bone marrow mesenchymal stem cells was in passage six, while PDT, cell size and viability differ significantly due to successive cryopreservation.

Abstract

Introduction. Osteomyelitis management with debridement, guttering, and systemic antibiotics often do not give satisfactory results due to the inability of antibiotics to reach the infection site. Another method of antibiotic delivery in the form of beads are deemed ineffective as it requires two separate surgeries. Combination of local antibiotics with absorbable porous hydroxyappetite scaffolding  and antibiotic carriers have not been studied. Methods. We conducted pre-test and post-test control group in a rabbit model of osteomyelitis. Ten rabbits divided in control group and the treatment group (n=5). We injected Staphylococcus aureus in the rabbit tibia, forming the osteomyelitis model, and subsequently performed treatment for osteomyelitis. In the control group, we performed debridement and administered ceftriaxone injection for 4 weeks, whereas in the treatment group, we added the combination of porous hydroxyapatite and gentamicin. Afterwards, we performed clinical assessment, x-ray examination, culture, and histopathology.Results. Radiologically, tibia cortical thickening scores improved in the treatment group compared to the control group (p=0.48) as well as histopathological osteomyelitis evaluation score (p=0,009). Clinically, there were improvements in the swelling scores (n=5) of the treatment group compared to control group, but no significant statistically (p=0.053). In culture, there were no significant difference between the two groups (p=1.00).Conclusion. Combination of porous hydroxyapatite and gentamicin as a local treatment of osteomyelitis of the rabbit tibia osteomyelitis models improved radiological and histopathological scores and also clinically compared to existing standard treatment procedures for chronic osteomyelitis

Abstract

Pelvic ring injury is a rare condition with the incidence of hip fracture about 3% to 8% of all fractures. However, the majority of pelvic fracture patients have a high morbidity rate that could threaten their survival. The majority cause of hip fracture is high energy blunt trauma and is related to polytrauma incident.  The main goals of management for pelvic ring injury are to restore the form and stability of the pelvic. The main goal is to prevent nonunion and malunion, which lead to chronic pain and disability. This review is focused on the strengths and weaknesses of the management option, approach and complication for pelvic ring fracture surgery. Complications of pelvic ring fracture surgery are bleeding, nerve damage, DVT, pulmonary embolism, soft tissue injury, sepsis, and malunion of the pelvis and nonunion. Management for pelvic ring fracture surgery is used based on the location and configuration of the fracture, implant availability, surgeon’s preference, and experience.

Abstract

Background. Computer-assisted navigation TKA has just been adapted in Indonesia for the last decade. The method is aiming to achieve more precise mechanical alignment in TKA. However, controversies still exist especially in terms of functional outcome and radiological outcome.Method. Fifty elderly patients aged more than 50 years old with severe knee osteoarthritis (Kellgren Lawrence stage III or IV) who scheduled for TKA surgery in the first half of 2019 were consecutively sampled. All patients were measured for objective functional status with Western Ontario Mac Master University Index (WOMAC) and Oxford Knee Score (OKS). Weight-bearing, long-leg follow-up radiographs of both lower limbs were taken, and coronal tibiofemoral angle (CTFA), coronal femoral component angle (CFCA) and coronal tibial component angle (CTCA) were measured.Results. Group of computer-assisted navigation TKA surgery was found to have significantly lower WOMAC score (p0.05) and higher OKS score (p0.05). Although not statistically significant, greater angle of deviation is observed in the conventional TKA surgery group. Proportions with deviation of less than 3º were found more in the group using computer navigation.Discussion. The results of this study indicate the advantage of using a computer assisted  system in TKA procedure, in terms of better functional outcome. However, our study only measures mechanical alignment from coronal plane and short term follow up time. Other parameter measurement, such as mechanical alignment on sagittal plane, rotational alignment, and soft tissue balance are also important to determine the success rate of TKA procedure.Conclusion. Patients who had TKA surgery under computer-assisted navigation would benefit a better functional outcome if compared with conventional TKA surgery, as measured with WOMAC and OKS. However, there is no significant difference in terms of radiographic value of coronal-plane mechanical deviation on both methods.

Abstract

The annual incidence of acetabulum fractures is estimated to be 8.1 per 100.000 patients. In young adults, high energy trauma is the primary mechanism, but in the elderly, low energy trauma is the primary mechanism. Comorbidities, poor bone quality, and delayed presentation are all linked to a poor outcome. From diagnosis to treatment, the goal of this study is to describe the required procedures in reaching a satisfactory result.It is essential to be able to correctly classify acetabular fractures using the Joudet and Letournel classifications, especially when planning definitive therapy. Non-surgical and operative treatment methods are available for acetabulum fractures. All acetabulum fractures that result in hip joint instability and/or incongruity require surgical treatment. Although the prognosis is lower than in younger patients, Open Reduction Internal Fixation (ORIF) remains the mainstay of therapy for most elderly acetabular fractures. Central fracture dislocation of the hip with medial migration of the quadrilateral plate is common in patients with both column, transverse, T-shaped, and anterior column-posterior hemi-transverse fractures. Failure to repair the quadrilateral plate will result in an incongruent hip and a poor outcome since it has a thin medial wall that fractures with less force than the superior weight-bearing portion of the acetabulum.The goals of postoperative care are to maximize the patient's function, allow a quick return to function, and detect early problems. Complications with acetabulum fractures might occur with any treatment option. Post-traumatic arthritis is the most common complications, followed by Deep Vein Thrombosis (DVT), heterotrphic ossification, infection, and iatrogenic sciatic nerve damage.

Abstract

Background: The most troubling complication of joint replacement procedures is periprosthetic joint infection (PJI). Pathogen identification increases the likelihood of successful treatment. The data of PJI are mostly unpublished in Indonesia. In this study we aimed to investigate the characteristics of PJI of a national hospital in Indonesia. Material and methods: This research was a cross-sectional study. The inclusion criteria were patients diagnosed with PJI confirmed by examination of fluid exudate culture, sedimentation rate and C-Reactive Protein, which was accompanied by dislocation, fracture and/or loosening of the prosthetic joint and diagnosed between January 2019 to July 2020. Result: The mean age of patients who underwent revision surgery for PJI was 50.0 ± 14.69 years-old with a slightly higher proportion of men (63.6%). Of the 21 patients, some patients had a history of surgery more than once. The clinical outcome was assessed with the Harris Hip Score (HHS) and the Visual Analog Scale (VAS). The mean HHS of all hip replacement surgeries increased from 61.4 (48.15 to 68) before surgery to 82.5 (75.7 to 84) at the 12-month follow-up post last surgery (p=0.03). Likewise, the VAS value decreased from 3 (2 to 4) before surgery to 2 (1 to 2). Conclusion: Gram positive bacteria were more prevalent compared to gram negative bacteria. The difference in the microorganism spectrum could be one of several factors. This pattern should be explored in the future by adding more samples to the study. Overall, the clinical outcome was better in both total hip and knee arthroplasty patients, measured by the Harris hip score, visual analog scale, and the knee society score

Abstract

Background: Revision total hip arthroplasty (rTHA) rate has increased until 12-20% in the past few decades, despite the 95% success rate of THA approach in 10 years and 80% in 20 years. The most common causes of rTHA are dislocation, periprosthetic fracture, aseptic loosening, and periprosthetic joint infection. This study is aimed to describe the outcome of rTHA in Indonesia where there are limited types of revision implant and funding. Material and Methods: An analytic cross-sectional study was conducted on 31 rTHA in the Indonesian national referral hospital from January 2014 to December 2019. Data on the causes of rTHA was extracted. All subjects met the criteria underwent examination for functional outcome (Harris hip score) and radiological outcome (Harris or Engh criteria). All complications and outcomes after rTHA were identified. Results: Sixteen subjects met the criteria with the mean age of 48.13 (18.74). The most common causes of rTHA were dislocation, aseptic loosening, and perirosthetic joint infection (five cases each, 31.25%). The Harris hip score after complete rTHA was 79.42 (SD 6.14, range 70.50 – 91) with the mean follow up of 29.50 (SD 16.88, range 7 – 70 months). Only one possible loosening was identified in hybrid prosthesis (femoral component) from radiological exam. Three complications were observed, i.e. drop foot, recurrent dislocation, and extension knee contracture. Conclusion: Revision THA produces fair to good results in terms of functional outcome and no loosening in radiological exam. Revision THA is still a reliable technique to manage complications of THA.

Abstract

Background: This study examines the adverse effects of COVID-19 pandemic on pediatric orthopedics globally, including decreased patient admissions. The adverse effects of pandemic on pediatric orthopedic patients and surgeries have not been determined. This study aims to conduct a comprehensive systematic review and meta-analysis to investigate the specific impact of COVID-19 pandemic on pediatric orthopedic admissions and surgical cases.Methods: A thorough examination of published literature was performed by searching databases such as PubMed, ScienceDirect, and Cochrane Library, using PRISMA guidelines, and limiting the search to articles published up to January 2022. The primary focus of this review was to inspect the ratio of admission and surgical cases, expressed as a risk ratio (RR) with a 95% confidence level. The study also evaluated any variation among outcomes using the I2 test and examined potential bias using the Egger test. The study findings were calculated using Review Manager 5.4.Results: Fifteen studies were reviewed, revealing a 70% decrease in pediatric orthopedic admissions during the pandemic but no significant difference in admissions between the pandemic and pre-pandemic eras (RR 1.08, 95% CI 0.76-1.54, p=0.66). Furthermore, in the perspective of the pediatric orthopedic surgical case, the pooled analysis also did not show any significant difference between both periods (RR 1.05, 95% CI 0.95 to 1.17, p=0.35). Egger's test result showed no evidence of small-study effects (p = 0.22).Conclusion: This study found that the number of children admitted for orthopedic surgeries and other orthopedic treatments was not greatly impacted during the COVID-19 pandemic.
