Abstract

Abstract: Lower urinary tract symptoms (LUTS) often occur among old-aged population. One of the causes of LUTS in aged males is benign prostatic hyperplasia (BPH). Males with severe LUTS will experience decreased libido, difficulty in enducing erection, and decreased sexual satisfaction. This study aimed to find out the number of LUTS occurence, caused by BPH at Prof. Dr. R. D. Kandou Hospital Manado during period 2009-2013. This was a descriptive retrospective sudy using secondary data from the hospital. The results showed that 8 cases (15.1%) in 2009, 16 cases (30.2%) in 2010, 12 cases (22.6%) in 2011, 11 cases (20.8%) in 2012, and 6 cases (11.3%) in 2013. Totally, there were 53 cases during that period of time. The data showed that LUTS caused by BPH most often occured among males aged ≥65 years. Conclusion: In this study, the number of LUTS occurence caused by BPH at Prof. Dr. R. D. Kandou Hospital in 2010 was the highest.Keywords : LUTS, BPHAbstrak: Lower Urinary Tract Symptoms (LUTS) sering terjadi pada populasi usia lanjut. Salah satu penyebab yang paling sering ialah Benign Prostatic Hyperplasia (BPH). Pria dengan LUTS yang berat akan mengalami penurunan libido, kesulitan mempertahankan ereksi dan penurunan tingkat kepuasan seksual. Penelitian ini bertujuan untuk mengetahui angka kejadian LUTS yang disebabkan oleh BPH di RSUP Prof. Dr. R. D. Kandou Manado periode 2009-2013. Penelitian ini bersifat deskriptif retrospektif dengan menggunakan data sekunder dari RSUP Prof. Dr. R. D. Kandou Manado periode 2009 - 2013. Hasil penelitian menunjukkan angka kejadian BPH pada tahun 2009 8 kasus (15,1%), tahun 2010 ditemukan 16 kasus (30,2%), tahun 2011 ditemukan 12 kasus (22,6%), tahun 2012 ditemukan 11 kasus (20,8%) dan tahun 2013 ditemukan 6 kasus (11,3%), dengan total 53 kasus. Penderita LUTS yang disebabkan oleh BPH tersering muncul pada umur ≥65 tahun. Simpulan: Pada penelitian ini angka kejadian LUTS yang disebabkan oleh BPH di RSUP Prof. Dr. R. D. Kandou Manado periode 2009 – 2013, paling tinggi pada tahun 2010.Kata kunci: LUTS, BPH

Abstract

Abstract: Benign prostatic hyperplasia (BPH) is a disease that is rarely life-threatening, but resulting in annoying complaints and decreasing the quality of life of the patients. BPH is found in older men. BPH patients who come for treatment are usually accompanied with lower urinary tract symptoms (LUTS). BPH patients with LUTS symptoms usually come to the clinic when they feel that the symptoms are bothering their daily-lives, therefore, decreasing the quality of life (QoL). BPH is caused by many factors, as follows: knowledge, economic, social, or cultural factors. To determine the severity of BPH with LUTS, we used the International Prostate Symptom Scoring Score (IPSS) and 1 question about the quality of life. Based on the results of cross-sectional study in December 2014 we obtained 37 patients as samples. Most patients belonged to the age group of 70-79 years (62.2%). The majority of respondents had severe degree of IPSS score (20 patients, 54.1%). The value of QoL scores showed that 10 patients (27%) were not happy with their quality of life. The Spearman correlation test was used to find whether there was a correlation between the IPSS score and Quality of Life (QoL) of BPH patients with LUTS symptoms. There was a significant correlation (p < 0.005) between IPSS-QoL (r = 0.481). The reason that patients with symptoms of BPH LUTS came the surgical clinic of Prof. Dr R. D. Kandou Hospital Manado with severe symptoms and decreases QoL was their lack of knowledge about BPH.Keywords: BPH, LUTS, IPSS, QoL, ManadoAbstrak: Hiperplasia prostat jinak atau Benign Prostatic Hyperplasia (BPH) merupakan penyakit yangjarang mengancamjiwa tetapi cukup memberikan keluhan yang menjengkelkan dan menurunkan kualitas hidup penderitanya dan sering ditemukan pada pria usia lanjut. Pasien BPH yang datang berobat biasanya disertai dengan LUTS (lower urinary tract symptoms). Pasien BPH dengan gejala LUTS yang mencari pengobatan medis biasanya datang pada saat dirinya merasa bahwa gejala yang dirasakan sudah cukup menggangu aktivitas sehari-hari atau dengan kata lain, mengganggu kualitas hidupnya. Keadaan seperti ini disebabkan oleh banyak faktor, antara lain faktor pengetahuan, ekonomi, atau sosial budaya. Untuk menentukan derajat berat ringannya BPH yang disertai dengan gejala LUTS, digunakan skoring International Prostate Symptom Score (IPSS) dan 1 pertanyaan mengenai kualitas hidup. Berdasarkan hasil penelitian potong lintang yang dilakukan selama bulan Desember 2014 didapatkan 37 sampel. Insiden BPH terbanyak berada pada kisaran umur 70-79 tahun sebanyak 23 pasien (62,2%). Derajat BPH yang paling sering ialah derajat berat sebanyak 20 pasien (54,1%). Nilai skor kualitas hidup yang sering dirasakan pasien menunjukkan bahwa 10 pasien (27%) merasa tidak senang dengan kualitas hidupnya. Hasil uji korelasi antara IPSS-QOL didapatkan bermakna (p<0,005), dengan nilai r = 0,481. Faktor yang menjadi alasan mengapa pasien-pasien BPH dengan gejala LUTS yang berobat di Poli Bedah RSUP Prof. Dr. 490Mandang, Monoarfa, Salem: Hubungan antara skor...R. D. Abstract

Abstract: Gastric perforation is the most common disease and a surgical emergency with high morbidity and mortality; therefore, early and precise management is needed. However, no consensus has been accepted regarding the best surgical treatment for gastric perforation closure so far. Although surgery methods are varied, laparotomy, and omental patch repair are still the gold standard. The risk of leakage after primary closure accompanied by omentopexy is still common, resulting high morbidity and mortality. This study aimed to discuss about the modified method by means of primary closure with seromuscular omentum insertion in a gastric perforation case. We modified the usual method using primary closure with seromuscular omentum insertion, to get the best way for perforation closure and reduced risk of leakage. After a week of surgical perforation closure, the patient was discharged and further observation resulted in no leakage. In conclusion, modified method by using primary closure with seromuscular omentum insertion in gastric perforation has good result without any leakage and recurrence. Abstract

Abstract: In colorectal cancer, surgical resection is the main curative therapeutic modality for localized, non-metastatic colorectal cancer at any age with good performance status and controlled comorbidities. Nutritional status is known to be a very important factor in the treatment of all types of surgery including preoperative and postoperative aspects. This study aimed to assess the ability of prognostic nutritional index (PNI) to predict the healing time of surgical wounds after laparotomy in patients with colorectal cancer. Samples were patients with colorectal cancer that underwent laparotomy at Prof. Dr. R. D. Kandou Hospital, Manado. The results showed that mean age of patients was 55 years and the mean body mass index (BMI) was 22. Patients with comorbids were 18.2%. The most common duration of wound healing according to Southampton wound grade system was grade 0 (63.6%), followed by grade 1 (18.2%), grade 2 (4.5%), and grade 3 (13.6%). The Spearman test showed a positive correlation between pre-operative PNI and duration of wound healing (r=0.582), and the 2-tailed significancy of 0.000. In conclusion, prognostic nutritional index can predict the duration of post-laparotomy wound healing in colorectal cancer patients. Keywords: colorectal cancer;  prognostic nutritional index; wound healing   Abstrak: Pada kanker kolorektal reseksi pembedahan merupakan modalitas terapi utama yang bersifat kuratif untuk stadium non metastasis yang masih terlokalisasi pada usia berapapun dengan status performance yang masih baik dan komorbiditas terkontrol. Status gizi diketahui menjadi faktor yang sangat penting dalam penanganan seluruh jenis bedah termasuk aspek pre operatif dan post operatif. Penelitian ini bertujuan untuk menilai kemampuan prognostic nutritional index (PNI) dalam memrediksi lama penyembuhan luka operasi paska tindakan laparotomi pada pasien dengan kanker kolorektal. Sampel penelitian ialah pasien dengan kanker kolorektal yang menjalani laparatomi di RSUP Prof. Dr. R. D. Kandou Manado. Hasil penelitian mendapatkan rerata usia pasien ialah 55 tahun dan rerata indeks massa tubuh (IMT) ialah 22. Pasien yang memiliki penyakit penyerta sebesar 18,2%. Lama penyembuhan luka operasi menurut (Southampton Wound Grade System) terbanyak ialah grade 0 (63,6%), diikuti grade 1 (18,2%), grade 2 (4,5%), dan grade 3 (13,6%). Hasil uji korelasi Spearman terhadap korelasi antara PNI pre-OP dengan lama penyembuhan luka operasi bernilai positif 0,582, dengan nilai Sig. (2-tailed) sebesar 0,000. Simpulan penelitian ini ialah prognostic nutritional index dapat memrediksi lama penyembuhan luka operasi paska tindakan laparotomi pada pasien kanker kolorektal. Abstract

Abstract: Liver metastases are common in patients with colorectal cancer; almost 70% will develop liver metastases during the course. The recommended treatment for colorectal liver metastasis (CRLM) is multidisciplinary, including liver resection and chemotherapy. We reported a 52-year-old female with stage 3B distal third rectal adenocarcinoma which eight months earlier underwent Mile’s procedure plus total mesorectal excision (TME) followed by adjuvant radiotherapy (50gy). During surveillance, liver metastases was found at segments 4B-5. A delayed anatomical staged hepatectomy segments 4B-5 was performed. Intraoperative USG findings suggested liver metastases at segments 7, 8, and 3, and non-anatomical liver resection was performed in accordance with parenchymal liver sparing principles. No post-hepatectomy liver failure (PHLF) was detected, but billoma occured at 1-month post hepatectomy. USG guided percutaneous drainage was performed to resolve the billoma. Colorectal metastasis (CRLM) was detected at six months post-hepatectomy, and the patient underwent adjuvant chemotherapy with improvement in survival rate. In conclusion, delayed staged hepatectomy for CRLM is a safe and beneficial procedure, though there is still no guideline regarding the sequence of resection. Abstract

Pendahuluan: Penyakit virus corona 2019 (COVID-19) adalah penyakit menular yang disebabkan oleh sindrom pernapasan akut coronavirus-2 (SARS-CoV-2). Keadaan hiperkoagulasi COVID-19 dapat meningkatkan risiko komplikasi trombosis dan tromboemboli. Trombosis mesenterika adalah komplikasi vaskular yang umumnya terkait dengan hiperkoagulabilitas, yang mengakibatkan nyeri perut dan iskemia usus. Presentasi kasus: kami melaporkan 3 kasus pasien yang datang kepada kami setelah terinfeksi Covid-19, yang datang dengan keluhan dan gejala berupa nyeri perut hebat, kembung dengan tanda-tanda obstruksi, dengan hasil D-dimer yang tinggi. Dilakukan laparotomi darurat dan ditemukan trombosis mesenterika, dilakukan reseksi anastomosis dan heparinisasi pasca operasi. Diskusi: Trombosis Mesenterika setelah Covid 19 biasanya muncul dengan gejala abdomen akut dan pemeriksaan d-dimer yang meningkat. Laparotomi darurat dilakukan dengan reseksi anastomosis usus, diikuti dengan heparinisasi pasca operasi. Dua pasien menunjukkan hasil yang baik dan menjalani rawat jalan, tidak ditemukan adanya kebocoran anastomosis maupun sindrom usus pendek. Gegar otak: Diagnosis trombosis mesenterika dapat dicurigai pada pasien pasca Covid-19 dengan gejala abdomen akut di mana terdapat peningkatan D-Dimer.
