Abstract

Latar belakang: Pandemi COVID 19 selama kurun waktu dua tahun ini telah menghentikan banyak aktivitas manusia, termasuk remaja yang harus belajar secara online di rumah. Aktivitas fisik yang kurang adekuat dan juga sikap kerja tubuh selama beraktivitas di rumah yang tidak memperhatikan prinsip ergonomis dalam jangka waktu yang lama, memberikan dampak terhadap kesehatan terutama munculnya gangguan muskuloskeletal mulai dari yang ringan-sedang pada remaja. Tujuan: melakukan pendidikan kesehatan tentang sikap tubuh ergonomis dan pola hidup sehat untuk mencegah terjadinya musculoskeletal disorders (MSDs). Metode: Pengabdian kepada masyarakat berbasis riset dilakukan dengan metode pendidikan kesehatan dengan topik sikap tubuh ergonomis dan pola aktivitas/olahraga dalam rangka mencegah gangguan tulang dan sendi. Peserta yang hadir sebanyak 50 orang. Hasil : Tingkat pengetahuan remaja tentang bagaiman pola aktivitas dan olahraga yang ideal untuk remaja berada pada tingkat sedang yaitu sebanyak 41 anak (82%) dan tingkat pengetahuan tentang sikap ergonomis tubuh dalam aktivitas sehai-hari dalam mencegah musculoskeletal disorder sudah mencapai 96% tingkat pengetahuan yang tinggi. Kesimpulan: Pendidikan kesehatan tentang sikap tubuh ergonomis sangat penting dilakukan pada remaja untuk membekali pengetahuan, sikap dan perilaku sikap tubuh ergonomis yang baik dan benar dalam aktivitas sehari-hari sehingga mencegah terjadinya MSDs. Dengan terjaganya kesehatan otot, tulang, dan sendi maka kualitas hidup dan produktivitas remaja akan terpelihara bahkan meningkat. Abstract

Latar belakang: Standar prosedur penanganan pada pasien paska bedah di RSUP Dr Kariadi belum cukup efektif untuk mengurangi nyeri tenggorok paska operasi dengan pamasangan endotrakeal tube pada General Anesthesia (GA). Penggunaan tablet hisap terbukti mampu menurunkan nyeri tenggorok, akan tetapi bukti penelitian masih terbatas. Penelitian ini untuk menguji keefektivitasan pemberian tablet hisap dan relaksasi napas dalam untuk mengurangi derajat nyeri tenggorok paska operasi. Metode: Penelitian dilakukan dengan desain quasi experimental pre-posttest pada 11 Juli – 30 Oktober 2019. Subyek penelitian berjumlah 60 pasien paska operasi dengan general anesthesia yang dikelompokan menjadi 30 kelompok intervensi dan 30 kelompok kontrol. Nyeri tenggorok paska operasi diukur dengan menggunakan Visual Analogue Scale (VAS) pada 6 jam paska operasi dan dievaluasi tiap 12 jam selama 48 jam. Hasil: Perbedaan rerata skor VAS antara kelompok kontrol dan kelompok intervensi secara signifikan terjadi pada penilaian nyeri 36 jam paska operasi (t = -3,70, p = 0,002) dan 48 jam paska operasi ( t = -4,28, p = 0,000). Skor VAS pada kelompok intervensi lebih rendah secara signifikan dibandingan dengan skor VAS pada kelompok kontrol. Simpulan: Pemberian tablet hisap dan relaksasi napas dalam secara signifikan menurunkan tingkat nyeri tenggorak paska operasi. Pemberian tablet hisap dapat dijadikan sebagai intervensi alternatif untuk penangangan pasien dengan komplikasi nyeri tenggorok paska operasi. Key words: Nyeri tenggorok, Endotracheal Tube, Tablet hisap, Relaksasi napas dalam, Visual Analogue Scale (VAS) Background: The standard procedures of post operative patients at RSUP Dr Kariadi Semarang was ineffective to reduce post operative sore throat with Endotracheal Tube insertion in General Anesthesia. Lozenges was proven may reduce sore throat while the evidences were still limited. Methods: The research design was quasi experimental pre-post test on 11 july - 30 october 2019. The study subjects were 60 post operative patients with General Anesthesia grouped into 30 intervention grups and 30 control groups. Post operative sore throat was measured using Visual Analogue Scale (VAS) at 6 hours after surgery then evaluated every 12 hours for 48 hours. Results: Mean difference VAS score between intervention group and control group significantly in the assessment of pain 36 hours (t = -3,70, p = 0,002) and 48 hours ( t = -4,28, p = 0,000) post operatively. VAS score in intervention group was significantly lower than VAS score in control group. Conclusion: Lozenges and deep breath relaxation reduce significantly post operative sore throat. Lozenges could be used as an alternative intervention to treat patients with post operative sore throat. Abstract

Caregivers should have adequate caring roles knowledge of children to increase the quality of life of children with congenital heart disease (CHD). Aim: To develop and evaluate the effect of a home-based continuing care program on the role of caregivers for children with CHD. This pilot study was a quasi-experiment with one group pre-posttest without control group design and conducted between October 2018 and September 2019 in Narathiwat, Pattani, and Yala. Thirty caregivers were recruited using a multistage random sampling technique. The program covered teaching, demonstration, and feedback equipped with a manual of CHD for six weeks. The self-developed caregiver role questionnaire was provided to evaluate caregiver care roles. The paired t-test was applied to evaluate the different scores of the caring roles before and after the program with the significant p value < 0.05. The scores of the caregivers’ roles for children with CHD after receiving the program were significantly higher than the scores before receiving the program (t = 6.20, p < .001). Continuing care programs enhance caring roles related knowledge of children with CHD. This program is recommended to apply continuously for caregivers of children with CHD at the outpatient or as part of discharge planning

Abstract

Inadequacy of current practices of routine thermal care for heat loss prevention in preterm infants leads to an additional need for immediate hypothermia prevention following delivery and during NICU admission. Plastic bag wrapping has been proven effectively to prevent the hypothermia in preterm infants. A review aimed to investigate the effectiveness of plastic bag wrapping toward hypothermia prevention in preterm infants after birth and during NICU admission. A review was performed by searching of RCT trials using CINAHL, Pubmed, Medline, and EBSCOHost and published in English between January 2013 and August 2023 related to plastic bag or plastic wrap or vinyl bag or polyethylene bag or polyethylene covering, core temperature, and hypothermia. Quasi-experimental studies and randomized controlled trials (RCTs) are included in this study. This study included 10 trials involving 1704 preterm infants: 8 trials reported on poststabilization temperature and incidence of hypothermia, 2 trials showed hypothermia, and one trial in poststabilization temperature for the outcome. Most studies conducted plastic wrap on preterm neonates < 28 weeks of gestational age and for 1-2 hours. Nine studies revealed that plastic bag wrapping was associated with a decline in hypothermia insidences, the higher temperature on baseline and poststabilization temperature compared with routine care or other thermal care. A plastic bag is considered an effective and safe additional intervention for preventing hypothermia in preterm infants less than 28 weeks of gestational age. However, further study could be ensured by performing better-designed studies and a longer follow-up period.

Abstract

Background: Early adolescence is a period of growth and development marked by the onset of puberty, where important changes occur in psychological development and social roles. Excessive internet use, especially in adolescents at this time, causes health problems such as postural disorders, sedentary lifestyles, and unbalanced nutrition. Objective: This study was planned to investigate the level of activity, body mass index and habits and awareness of posture in early adolescents. Method: Measurements of body weight, height were carried out, participants filled out a questionnaire on knowledge of posture, knowledge of ergonomics and a questionnaire on postural habits awareness. Results: Participants were 33 children aged 11-17 years. Knowledge of posture at a high level was 17 adolescents (51.5%). Knowledge of musculoskeletal disorders at a moderate level was 26 adolescents (78.78%). Posture habits were mostly at a good level as many as 29 adolescents (87.9%). Conclusion: The level of activity of adolescents was mostly at a moderate level and nutritional status based on BMI was mostly in the thin category. Knowledge of posture, ergonomics and habits and awareness of posture is at medium and high levels. Indicating adequate information has been obtained by participants.

Abstract

Introduction: Critical thinking and clinical reasoning are essential competencies in delivering safe and effective nursing care. However, identifying effective learning strategies to foster these skills remains a challenge. This study aimed to examine the effectiveness of a case-based concept map method in improving critical thinking and clinical reasoning abilities among newly enrolled nurse professional students.Methods: This quasi-experimental study employed a one-group pre-test–post-test design without a control group. A total of 18 newly admitted nurse professional students were recruited using total sampling. The intervention involved developing concept maps based on clinical cases over one week. Clinical reasoning was assessed using quizzes with clinical vignettes, while critical thinking was measured using the Critical Thinking Disposition Inventory. Paired t-tests were used to evaluate the differences in scores before and after the intervention.Results: There was a significant improvement in total critical thinking scores following the intervention (t = 2.25, p = 0.038), particularly in the analytical and systematic thinking domains (t = 2.30, p = 0.023). Clinical reasoning scores also showed a marked increase post-intervention (t = 11.93, p < 0.001), with a mean difference of 9.56.Conclusion: The case-based concept map method is effective in enhancing both clinical reasoning and critical thinking skills among nurse professional students. Integrating this approach into nursing education may better prepare students for clinical practice by promoting deeper understanding and structured decision-making.

Abstract

Background: Adequate understanding of pathophysiology is essential for nursing students to provide competent care and demonstrate caring behavior towards patients. In addition, caring behavior is at the core of the nursing care process. Concept maps, a visual learning strategy, have shown promise in facilitating the understanding of pathophysiology and may positively impact on improving caring behavior among nursing students. Purpose: To analyze the effectiveness of the case-based concept map method in improving the understanding of pathophysiology and caring behavior of nursing students. Method: A quasi-experimental study with a pre-posttest with control group approach on 149 nursing students (intervention group: 81 participants, control group: 68 participants) in May - July 2024. The application of the concept map was carried out for 5 weeks in the intervention group. This study used the Caring Behavior Inventory (CBI) questionnaire to measure students' caring behavior and a quiz to measure students' understanding of pathophysiology which was filled out online. The analysis in this study used paired t-test and independent t-test. Results: The mean score difference in pathophysiology understanding between the intervention group and the control group was significantly higher in the intervention group (p = 0.000, t = 3.882). In contrast, the mean score of caring behavior did not show a significant difference between the intervention and control groups (p = 0.37, t = 0.899). Conclusion: The case-based concept map is effective in enhancing the understanding of pathophysiology; however, this method remains inconsistent in promoting the caring behaviors of nursing students. Suggestion: Future research is expected to develop an extended case-based concept map application in nursing process planning (formulation of nursing diagnoses, determination of outcome criteria, and selection of nursing interventions) in nursing students at the final level or nursing students who are carrying out learning in clinical practice.   Keywords: Concept Map; Caring Behavior; Nursing Students; Pathophysiology.   Pendahuluan: Pemahaman yang adekuat tentang patofisiologi sangat penting bagi mahasiswa keperawatan untuk memberikan perawatan yang kompeten dan menunjukkan perilaku caring kepada pasien. Selain itu, perilaku caring merupakan inti dari proses asuhan keperawatan. Concept map merupakan sebuah strategi pembelajaran visual, terbukti menjanjikan dalam memfasilitasi pemahaman patofisiologi dan mungkin berdampak terhadap peningkatan perilaku caring pada mahasiswa keperawatan. Tujuan: Untuk menganalisis efektivitas metode case-based concept map dalam meningkatkan pemahaman patofisiologi dan perilaku caring mahasiswa keperawatan. Metode: Penelitian studi quasi eksperimental dengan pendekatan pre-posttest with control group pada 149 mahasiswa Jurusan Keperawatan (kelompok intervensi: 81 partisipan, kelompok kontrol: 68 partisipan) pada bulan Mei – Juli 2024. Penerapan concept map dilakukan selama 5 minggu pada kelompok intervensi. Penelitian ini menggunakan kuesioner Caring Behavior Inventory (CBI) untuk mengukur perilaku caring mahasiswa dan kuis untuk mengukur pemahaman patofisiologi mahasiswa yang diisi secara online. Paired-t-test dan independent t-test digunakan untuk analisis dalam penelitian ini. Hasil: Perbedaan rerata skor pemahaman patofisiologi pada kelompok intervensi lebih tinggi secara signifikan dibandingkan skor rerata pada kelompok kontrol (p = 0.000, t = 3.882). Sedangkan, rerata skor perilaku caring antara kelompok intervensi dan kontol tidak berbeda secara signifikan (p = 0.37, t = 0.899). Simpulan: Case-based concept map efektif dalam meningkatkan pemahaman patofisiologi. Namun, metode ini masih belum inkonsisten dalam meningkatkan perilaku caring mahasiswa keperawatan. Saran: Penelitian selanjutnya diharapkan dapat mengembangkan aplikasi case-based concept map yang diperluas dalam perencanaan proses keperawatan (perumusan diagnosa keperawatan, penentuan kriteria luaran, dan pemilihan intervensi keperawatan) pada mahasiswa keperawatan.   Kata Kunci: Concept Map; Mahasiswa Keperawatan; Patofisiologi; Perilaku Caring.

Abstract

Introduction: One of the most common clinical manifestations in CKD patients is leg oedema. Oedema that is not treated promptly leads to several complications such as ischaemia due to reduced tissue function, reduced mobility, skin infections such as cellulitis or ulcers in chronic oedema. Oedema can be managed with non-pharmacological therapies such as a combination of ankle pumping and warm compresses. Purpose: This study aims to describe the reduction in the degree of leg oedema in patients with CKD after administration of ankle pump exercise and warm compresses. Methods: This research is a case study with a nursing approach to CKD patients with oedema. Participants in this study were 3 CKD patients with leg oedema. Interventions were carried out on 3 consecutive days within 15-20 minutes of each session. Pre- and post-measurements of the degree of oedema were made at each session. Discussion: Oedema is a condition resulting from an increase in hydrostatic pressure in the blood vessels, which causes the veins to become blocked and plasma fluid to accumulate in the interstitial spaces. The results of the measurement of the degree of oedema showed that two participants experienced a decrease in the degree of oedema from degree 3 to 2 and one participant experienced a decrease from degree 2 to 1. Conclusion: A combination of ankle pump exercise and warm compresses can reduce the degree of leg oedema in patients with CKD and can be used independently by patients and families.

Abstract

Introduction: Rheumatoid arthritis is a disease that attacks the joints which can cause a decrease in muscle strength. To avoid a decrease in muscle strength, non-pharmacological treatment can be done by doing Range of Motion (ROM) exercises. Purpose: This case study aims to determine the increase in muscle strength in patients with rheumatoid arthritis after active ROM exercises. Methods: The method used is a case study using descriptive research. The subject of the study is one patient with rheumatoid arthritis. The focus of this study is on the application of evidence-based nursing in the form of active ROM exercises for 3 times in 3 consecutive days with a movement frequency of 10 times to patient with rheumatoid arthritis. Muscle strength assessment is carried out using Manual Muscle Testing (MMT) to see the results of the pre-test and post-test of the patient's muscle strength. Discussion: The results obtained after the patient underwent active ROM exercises were increased muscle strength. Active ROM exercises was able to increase muscle strength from 4/4/3/3 to 5/5/4/4. This shows that ROM exercises can be used as a therapy to increase muscle strength in patients with rheumatoid arthritis. Conclusion: Providing active ROM exercises can increase muscle strength in patients with rheumatoid arthritis.
