Abstract

Diabetes melitus (DM) merupakan  penyakit metabolik  yang menyebabkan kerusakan sel termasuk pankreas. Kuersetin danomega-3 telah diteliti sebagai terapi tambahan pada DM karena dapat menurunkan stres oksidatif dan mencegah kerusakan sel. Tujuan penelitian ini adalah mengetahui efek kombinasi kuersetin dan omega 3 dalam melindungi sel β pankreas dari kerusakan pada tikus DM. Subyek terdiri dari 28 ekor tikus, dibagi menjadi 1 kelompok sehat dan 6 kelompok DM tipe 2 (4 ekor perkelompok). Bahan uji adalah kuersetin dosis 5, 20 dan 80 mg/kgBB/hari dan omega-3 dosis 25, 100 dan 400  mg/kgBB/hari peroral selama 4 minggu. Setelah perlakuan, jaringan pankreas diambil untuk pengecatan hematoksilin-eosin dan dihitung persentasekerusakannya.Pada penelitian ini, kombinasi kuersetin 80 mg/kgbb/hari dan omega-3 400 mg/kgbb/hari menurunkan tingkat kerusakan sel Β pankreas paling baik secara bermakna (p<0,05) dari pada kombinasi dosis lebih kecil maupun kuersetin saja atau omega-3 saja. Jadi kombinasi kuersetin 80 mg/kgbb/hari dan omega-3 400 mg/kgbb/hari menurunkan tingkat kerusakan sel β pankreas paling baik secara bermakna jika dibandingkan kombinasi dosis lebih kecil maupun tanpa kombinasi.Kata Kunci: Kuersetin, omega-3, diabetes melitus tipe-2, histologi sel β pankreas, hematoksilin-eosin.

Abstract

BACKGROUNDDiabetes mellitus (DM) is a group of metabolic diseases which are characterized by hyperglycemia, resulting in various complications. A major macrovascular complication of DM is cardiac failure due to cardiomyopathy. Hyperglycemiaincreases oxidative stress, so an oxidative-stress reducing therapeutic agent is required, e.g. the antioxidant quercetin. The aim of this study was to assess the effects of quercetin in reducing damage to cardiomyocytes of type 2 diabetic rats.METHODSThis research is an experimental study using 40 rats. With simple randomallocation, rats were divided into eight groups, then type 2 diabetes mellituswas induced using streptozotocin (5 rats per group). The test material wasquercetin given at doses of 5, 20 and 80 mg/kgBW/day orally for 4 weeks.Each single dose of quercetin was given in combination with glibenclamide 5mg/ kgBW/day. After 4 weeks the rats were decapitated and the cardiac tissuestaken to quantify the percentage of cell damage after hematoxylin-eosin staining(HE).RESULTSQuercetin at a dose of 80 mg/kgBW/day can lower cardiomyocyte damagebetter than quercetin at doses of 5 or 20 mg/kgBW/day. A combination ofquercetin and glibenclamide can significantly lower levels of cardiomyocytedamage better than quercetin without glibenclamide (p<0.05).CONCLUSIONQuercetin at a dose of 80 mg/kgBW/day with or without glibenclamide canlower damage to cardiomyocytes of type 2 diabetic rats. Thus quercetin mightserve as a valuable protective agent in cardiovascular inflammatory diseasesin diabetic rats.

Abstract

Type 2 diabetes mellitus is characterized by hyperglycemia. Hyperglycemia increases oxidative stress that damage cells including in heart muscle. Oral hypoglycemic drugs cant reduce the expression of nuclear factor kappa B (NFkB) which plays an important role in inflammation and cell damage. Radish leaves (Raphanus sativus) are known to have compounds that can reduce the expression of NFkB.This study aimed to determine the effect of radish leaves on the expression of NFkB in heart muscle. The study design was an experimental laboratory post-test control group design. The subjects were DM male Wistar rats (Rattus norvegicus), weight 150-300 grams, 3-4 months old. Rats were divided into four groups and took the treatment orally for 28 days. The 1st group got plain water. The 2nd group got glibenclamide 5 mg/kg BW/day. The 3rd and 4th groups got 50% and 100% ethanol extract of radish leaves. After the end of treatment, the heart's tissue was taken for immunohistochemical (IHC) staining to measure the proportion of NFkB. The intervention of ethanol extract of radish leaves 50% and 100% for 28 days reduced the percentage of NFkB in the heart muscle cell of rats than placebo and glibenclamide 5 mg/kg BW ((p=0.000). There was no difference between radish leaf extract concentration of 50% and 100% for 28 days in reducing the expression level of NFkB in the heart muscle cell of rats (p=0.876).This research has not been done before and is important for the treatment of diabetes mellitus in the future.
