Abstract

Objective: Tooth fractures are mostly caused by traumatic injuries that can involve the pulp and surrounding periodontal tissue. Root fracture is an unusual injury (0,5-7%) but requires a complex healing pattern, so the treatment can require many specialists. Fractures at the third level have a good result if the proper diagnosis and there is no mobility and the teeth are asymptomatic. Case Report: Treatment of this third-apical fractures is endodontics treatment in the coronal and apical resection of the apical fragmen with MTA placement to get an apical plug. The treatment is continued by prosthodontic to obtain the aesthetic result. Results: Endodontic treatment and apical resection presented good results with no symptoms of clinical pathology for about 3 months followed up. Conclusion: Fractures at the third level have a good result if the proper diagnosis, the right treatment planning and procedures are performed.

Abstract

Introduction: Traumatized immature permanent teeth can cause pulp necrosis, leading to imperfect root formation, and thus resulting an open apex. The apical opening of the immature teeth is mostly large and thus may lead to difficulties in performing a root filling. In such cases, apexification is mandatory before doing root canal treatment. The aim of this case report was to inform that apexification and direct veneer was suitable to treat Ellis class IV fracture with necrotic pulpCase Report: 21-year-old female came to the Dental Conservation Clinic RSGM Prof. Soedomo Faculty of Dentistry UGM with chief complaint of right central incisive maxilla fracture and tooth discoloration. Radiographic examination showed an open apex of tooth 11 with a diffuse apical radiolucent area. Root canal treatment and apexification were carried out using circumferential technique and Mineral Trioxide Aggregate (MTA). Vertical warm obturation was performed, followed by intra coronal bleaching in the next visit. Final restoration was carried out using direct composite veneer. Abstract

Preparation of the root canal system is a crucial step in root canal treatment. Endodontically treated teeth differ structurally from healthy, untreated teeth. This can lead to root cracking by creating pressure on the canal wall, reducing the fracture resistance of the tooth. Different designs, including cross-sectional shape, tip, taper, flute, radial land, helix angle, rake angle, and pitch, can influence the outcome of the root canal preparation and the risk of root fracture. The purpose of this study was to evaluate the fracture resistance of teeth after root canal preparation using three different NiTi rotary files: ProTaper Next (PN), ProTaper Gold (PG), and HyFlex CM (HC). Thirty premolars with a single and straight root canal were decoronated at the cementoenamel junction, leaving 14 mm of the root. The subjects were divided into 3 groups. Group 1 (n = 10) was prepared using PN, group 2 (n = 10) was prepared using PG, and group 3 (n = 10) was prepared using HC. After instrumentation and irrigation, the specimens were subjected to a continuous vertical compressive force (crosshead-speed of 2.28 mm/sec) in order to record the force (in newtons) until root fracture. To describe the surface characteristic of the dentinal root after the preparation, a section fragment from the apical third of the specimens was observed using a scanning electron microscope (SEM) at 2500x magnification. The micrographs were analyzed according to the Hulsman’s method. The results demonstrated that there was a statistically significant difference in root fracture resistance among the three groups. (p = 0.043). The LSD post hoc test indicated that HC showed a higher root fracture resistance (p < 0.05) compared to both PN and PG. In conclusion, the different file systems of PN, PG, and HC instruments generate different root fracture resistance of teeth. In comparison to PN and PG, HC instruments tend to result in increased fracture resistance.

Abstract

The geometric design of an endodontic file includes cross-sectional shape, taper, and pitch. NiTi endodontic files of OneCurve (MicroMega) are made of two variations of cross-sectional shape, that is, double-s shaped and triple helix. Various kinds of modifications were made to increase the resistance of the NiTi endodontic files from fracture. Cyclic fatigue is the most common factor that causes a fracture of the endodontic file. Objective: The purpose of this study was to obtain a NiTi endodontic file design that has the best cyclic resistance value based on its taper size and number of pitch. Methods: This study used OneCurve endodontic file size 25.06, then scanned with MicroCT Scan and modified the geometric design. The number of file modifications was nine. Taper size modifications were 4%, 6%, and 8%; the number of pitch modifications was reduced by three, fixed, and added by three. Cyclic fatigue measurement simulations were carried out using the finite element method three times each. The result data were analyzed using a two-way ANOVA test and then continued with LSD. Results: The results of the statistical test showed that there was an effect of the size of the taper on the cyclic fatigue values. Endodontic files with a size of a taper 4% had the lowest cyclic fatigue value. Conclusion: The conclusion of this research was that the taper size affects the value of cyclic fatigue in continuous endodontic files made of nickel-titanium with variable cross sections. The number of pitch did not show a significant effect on the cyclic value.

Abstract

Background: Endodontic treated teeth are more susceptible to fracture. Correct restoration can minimize fracture risk, for example crown and post-core systems. Based on its manufacturing, Fiber Reinforced Composite (FRC) Post was divided into prefabricated and fabricated. The objective of this study was to evaluate the differences of fracture resistance between Prefabricated and Fabricated FRC Post systems on different post canal width. Method: The samples of this study were 28 premolars that were cut with 15 mm in length. The samples were treated with conventional endodontic method using gutta percha and resin sealer. The samples were divided into two groups and two sub-groups. The first two group was based on canal width consisting of 14 teeth each group with two different canal width (1.4 mm and 2.1 mm). Then, the first group was divided again into two subgroups based on the dowel use (Prefabricated post and Fabricated Post). Universal Testing machine was used to test the samples resistance. Result: This research showed that there were differences in fracture resistance on different post canal width. However, there were no fracture resistance differences between prefabricated post and fabricated post.
