Abstract

Latar Belakang: Discharge planning adalah proses penting dalam transisi perawatan yang bertujuan meningkatkan kualitas hidup pasien, mengurangi rehospitalisasi, dan memperkuat kesiapan pasien serta keluarga. Namun, implementasinya di berbagai fasilitas kesehatan belum optimal. Metode: Studi ini merupakan systematic review berdasarkan pedoman PRISMA. Pencarian artikel dilakukan pada database Google Scholar, ProQuest, PubMed, ScienceDirect, dan SpringerLink dengan kata kunci: “discharge planning” OR “hospital discharge” OR “patient outcomes” OR “readmission” OR “rehospitalization”, dalam rentang tahun 2020–2024. Dari 294 artikel, 10 artikel dipilih setelah melalui proses seleksi. Hasil: Discharge planning berdampak positif terhadap kualitas hidup, kesiapan pulang, dan penurunan rehospitalisasi. Faktor pendukungnya meliputi motivasi perawat, supervisi, dan kepemimpinan. Hambatan utama adalah beban kerja tinggi dan kurangnya dokumentasi. Kesimpulan: Discharge planning yang efektif membutuhkan dukungan sistem, keterlibatan perawat, dan pendekatan yang responsif terhadap kebutuhan pasien. Implementasi yang terstruktur dapat meningkatkan luaran pasien dan efisiensi layanan kesehatan. Kata Kunci: Discharge planning, rehospitalisasi, kesiapan pulang, luaran pasien, supervisi keperawatan Abstract Introduction: Discharge planning is a critical process in care transition aimed at improving patients’ quality of life, reducing rehospitalization, and enhancing the readiness of both patients and families. However, its implementation remains suboptimal across various healthcare facilities. Methods: This study is a systematic review conducted based on the PRISMA guidelines. Article searches were performed in Google Scholar, ProQuest, PubMed, ScienceDirect, and SpringerLink using the keywords: “discharge planning” OR “hospital discharge” OR “patient outcomes” OR “readmission” OR “rehospitalization”, with a publication range from 2020 to 2024. Out of 294 identified articles, 10 were selected after screening and eligibility assessment. Results: Discharge planning has a positive impact on quality of life, discharge readiness, and reduced rehospitalization rates. Supporting factors include nurse motivation, supervision, and leadership. Main barriers are high workload and lack of documentation. Conclusion: Effective discharge planning requires system support, active nurse involvement, and a patient-centered approach. Structured implementation can improve patient outcomes and healthcare service efficiency. Keywords: Discharge planning, rehospitalization, discharge readiness, patient outcomes, nursing supervision.

Abstract

Penelitian ini bertujuan untuk menganalisis lanskap ilmiah keperawatan terkait gangguan metabolik melalui pendekatan bibliometrik. Dengan menganalisis 391 dokumen dari total 657 publikasi yang teridentifikasi dalam periode 1953–2025, studi ini mengungkap tren pertumbuhan tahunan sebesar 3,25%, dengan rata-rata sitasi 16,51 per dokumen. Analisis ko-okurensi kata kunci menggunakan VOSviewer menghasilkan tiga klaster utama: (1) aspek klinis akut, (2) hubungan gangguan metabolik dengan penyakit kronis, dan (3) dimensi psikososial dan farmakologis. Hasil overlay dan density visualization menunjukkan bahwa tema seperti metabolic disorder dan water-electrolyte imbalance mendominasi, sementara beberapa topik strategis seperti insulin sensitivity dan neuroleptic agent masih kurang mendapat perhatian. Analisis kontribusi penulis dan negara menyoroti peran signifikan United States, Brasil, dan negara-negara Eropa dalam pengembangan literatur ini. Selain itu, artikel yang paling berpengaruh umumnya berasal dari jurnal bereputasi tinggi (Q1), dengan fokus pada intervensi keperawatan dalam penanganan pasien lansia dan kritis. Temuan ini menunjukkan perlunya revitalisasi riset keperawatan yang lebih kontekstual, integratif, dan berbasis teknologi. Studi ini memberikan arah strategis bagi penguatan kapasitas ilmiah, pengembangan praktik keperawatan berbasis bukti, serta kolaborasi global dalam menghadapi tantangan gangguan metabolik secara holistik.

Abstract

Introduction: Patients undergoing hemodialysis in hospitals are often faced with a variety of complex physical, psychological and social challenges. Family support has been identified as a key factor in improving patients' quality of life and adherence to therapy. Family support interventions for hemodialysis patients can help address issues related to the decline in quality of life. Objective: This study aims to examine the relationship between family support interventions and self-care behaviors and the quality of life of chronic kidney patients undergoing hemodialysis. Methods: This study was conducted using nine articles obtained between April 2 and May 18, 2025. Results: A total of nine relevant research articles were identified and analyzed, revealing a positive relationship between family support interventions and both self-care behavior and the quality of life of chronic kidney patients undergoing hemodialysis. Several factors were found to influence the effectiveness of these interventions, including patient characteristics, family conditions, the type and intensity of the intervention, socioeconomic factors, and the role of healthcare professionals. However, some aspects remain underexplored, such as family emotional intelligence, multigenerational family dynamics, the role of technology in family support, family spirituality, caregiver burnout, and family perceptions and expectations regarding treatment outcomes. Conclusion: Family support interventions can enhance self-care behavior and improve the quality of life of hemodialysis patients.
